1.Decision making for management of acute abdominal pain in children.

BACKGROUND: Acute abdominal pain in children is one of the most common problems encountered in our daily practice. The children often are not able to tell us their exact complain symptoms and differential diagnosis of acute abdominal pain is difficult in its first encounter. When family physicians encounter patients with acute abdominal pain in the ambulatory care settings, they must make an appropriate decision for proper management such as admission, referral, discharge and follow-up without firm diagnosis. If it is possible to predict the course or prognosis of acute abdominal pain by clinical data concerning patient, it will be helpful to make a decision for the management of acute abdominal pain. So we tested for hypothesis that patients with acute intermittent abdominal pain and normal simple abdomen X-ray finding is not serious and shows favorable outcome. METHOD: 82 patients with acute abdominal pain were assessed from Oct. in 1996 to Sep. in 1997 at the emergency department of Kyungju Hospital, Dongkuk University. 69 patients among 82 showed normal simple abdomen X-ray findings. 67 patients were discharged and 2 patients were admitted. 64 patients were contacted by phone call after 1 week and reported their outcome of the previous abdominal pain. RESULTS: Among 64 subjects 36 patients were male and 28 patients female. 57 patients complained of intermittent abdominal pain and 7 patients complained continous abdominal pain. Positive predictive value of intermittent abdominal pain and normal simple abdominal x-ray was 98%. Outcome of patients with intermittent abdominal paln was more favorable than that of continuous pain. CONCLUSIONS: Acute intermittent abdominal pain and normal simple abdomen x-ray findings in children shows favorable outcome and can be managedim ambulatory care settings.

2.Effect of Tending Diancibo Pu on Burn Treatment.

PURPOSE: Recently, in an experiment using animals, radiation therapy using a laser or Tending Diancibo Pu (TDP) has been shown to be effective in treating scars on the skin by increasing the production of fibroblast cell and collagen and by accelerating the process of epithelization. The purpose of this study is to examine the effectiveness of TDP radiation therapy in treating human burn injuries in terms of the frequency of treatment, the timing of eschar separation, and the duration of treatment. METHODS: In the treatment group, the burn area of the patients was first sterilized with saline solution and potadin solution and covered with one vaseline gauze. Then, the burn area was radiated every other day by using TDP for 20 minutes at distance of 20-25 cm, at radiant plate temperature of 250-280oC. In the cases of control group, the burn area of the patients was first sterilized with saline solution and potadin solution and covered with one vaseline gauze. Then, the area was covered with one burn gauze and bandaged. The treatment was conducted every other day. RESULTS: In cases of superficial second degree burn injuries, the difference between the treatment and the control groups was 1.34 in terms of the frequency of treatment. In the cases of both superficial and deep second degree burns, the differences between the treatment and the control groups were 3.47 in terms of the frequency of treatment, 0.63 weeks in terms of the timing of eschar separation, and 6.03 days in terms of the duration of treatment. All these differences were statistically significant. CONCLUSION: From the experiment, it can be concluded that TDP radiation therapy is more effective in treating human burn injuries than conventional treatment in terms of the of the frequency of treatment, the timing of eschar separation, and the duration of treatment.

3.Phenotypic Changes of Intermediate Filament Proteins during Degeneration and Regeneration of Rat Leg Muscles following Sciatic Nerve Injury.

PURPOSE: The intermediate filament proteins, desmin and vimentin, are specific components of the cytoskeleton of striated muscle fibers and of mononuclear cells of mesenchymal origin including myoblasts, respectively. Desmin has also been found in presumptive myoblasts of mammals. The aim of this experiment was attempted to observe the phenotypic changes of intermediate filaments in skeletal muscle fibers during early stages of sciatic nerve crushing injury. MATERIALS AND METHODS: The sciatic nerves of rats were surgically crushed by hemostat and serial cryosections of soleus and extensor digitorum longus(EDL) muscles were prepared at 2, 4, 6, 8, 10, 15, 20 and 27 days after nerve injury. Serial cryosections were immunolabelled with desmin, vimentin and laminin and were histochemically reacted with NADH-TR. RESULTS: 1) Firstly, desmin positive fibers were appeared in fast-twitch type C fibers of both muscles at 6 days after nerve crushing, but were not reacted for vimentin. 2) Co-expressions of desmin and vimentin were firstly detected in fast-twitch type A fibers of EDL muscles at 8 days after nerve injury. In soleus muscles, co-expressions of desmin and vimentin were firstly seen in slow-twitch type B fibers at 10 days after nerve injury. Many atrophic fibers, that contained several central nuclei like myotubes and co-expressed desmin and vimentin, were appeared in EDL muscles at 10 days after nerve injury. Although whole regions of fibers were regenerated in EDL muscles, only peripheral regions of fibers were regenerated in soleus muscles at 15 days after nerve injury. Many atrophic fibers, co-expressed of desmin and vimentin, were appeared in EDL muscles at 20 days after nerve injury. These whole fibers represented various degrees of regenerating stages. Most of mature fibers containing several central nuclei, only expressed vimentin slightly, were seen in soleus muscles at 20 days after nerve injury. Most fibers of both muscles were matured at 27 days after nerve injury, but some fibers in EDL muscles were still in processing of degeneration and regeneration. No expressions of desmin and vimentin indicated that muscle fibers were almostly matured in soleus muscles at 27 days after nerve injury. 3) Targetoid or target fibers which informed reinnervation, were appeared firstly in soleus muscles at 20 days and were seen in both muscles at 27 days after nerve injury. All targetoid and target fibers were type B fibers. CONCLUSION: Desmin was revealed in processes of degeneration and regeneration and vimentin was appealed in regeneration process. At the same time, positive immunoreactivity of desmin and vimentin showed specific differences in degree of degeneration and regeneration according to different muscles and muscle fibers.

BACKGROUND: Hepatitis B vaccination schedule commonly used in Korea is divided largely into 0, 1, 2 month scheduled vaccination group(0, 1, 2 group) and 0, 1, 6 month scheduled vaccination gorup(0, 1, 6 group). The only difference bet.ween two groups is the interval from 2nd dose to 3rd dose. This st,udy had been carried out, to find whether t.he difference of vaccination interval influence the vaccination complet,ion rate or not. METHODS: Study objects are 135 persons over 20 years old who had heptitis B vaccination more than once in injection room of Dongkuk University Kyong-ju Hospital from Jan. 1st in 1996 to Dec. 31th in 1996. Data about vaccination completion were gathered from record book of injection room and telephone interview. RESULTS: Hepatitis B vaccination completion rate is 73.8% in 0, 1, 2 group and 72.5% in 0, 1, 6 group. The reasons for incomplete vaccination are forgetting vaccinat.ion date(36.4% in 0, 1, 2 group and 50% in 0, 1, 6 group), having no time to spare for vaccination(54.5% in 0, 1, 2 group and 43% in 0, 1, 6 group) and knowing positive HBsAb before completion of scheduled vaccination(9.1% in 0, 1, 2 group and 7% in 0, 1, 6 group). CONCLUSIONS: There is no difference in Hepatitis B vaccination complet,ion rate bet.ween 0, 1, 2 group and 0, 1, 6 group.

9.THE STUDY OF MUTATION SPECTRUM OF HEMOPHILIA

Background:
Hemophilia is life-threatening and hereditary bleeding disorder caused by deficiencies of pi coagulation VIII. IX, XI factor, and that is inherited by X-linkcd recessive.
However, hemophilic treatment is getting increase in our country, but still insufficient or poor because of high cost, which is about 50000-70000$ per patient per year in other countries. Now, we need to detect hemophilia heterozygote (XAX*) or no symptomatic carrier among hemophiliac siblings (including mother, younger sister, sister) that is essential for prevention of hemophilia. We need to screen the mutation spectrum of FS and F9 gene among patients with hemophilia in order to detect hemophilia carriers.
Objective:
to detect a mutation 1'Ó ø! f9 gene among patients with hemophilia Methods:
The blood samples were collected from paffeflf* who had been hospitalizing in department ot hematology of the First Clinical Hospital and the National Center for Maternal and Child Health from 2010 to 2011. We have carried out the screening of most common mutation named int- 22 inversion by Long-Range PC'R method is previously described by Lui Q et al.. 1998. Direct sequence method was used to detect the SNP and small deletion in patients who had no int-22 inv and large deletion.
Results:
In total, If pillitfhis with hemophilia (ÈË-14, IIB=1) participated in this study. The 9 patients positive for an int-22 inversion mutation, while the one patient had a multiple exon deletion (exon 1-13) which was demonstrated by repeated PC'R amplification failure. I wo missense mutation and 1 frame shift mutation were detected. The one patient had nonsense mutation but he was diagnosed as a severe hemophilia-A patient.
Conclusion:
We have to urgently adopt the molecular diagnosis and carrier detection ol hemophilia in our rnuntrv
