1.Treatment of Post-traumatic Scapho-lunate Dissociation

We have analyzed 27 cases(38.8%) of scapho-lunate(SL) dissociation which were the most com- mon type of total 72 cases of post-traumatic wrist instabilities since 1985. As a result, SL dissociations associated with wrist fracture were 23 cases, more common than simple dynamic SL dissociation(4 cases). Early diagnosis and treatment influenced on their prognosis, and both wrist PA roentgenograms were useful for the confirmative diagnosis of SL dissociation. In 16 cases with early diagnosis, closed reduction, 2 or 3 K-wires fixation under C-arm image and short arm cast for 8 weeks were effective. But in 11 cases with neglected diagnosis and with wrist fractures showed poor results. Excessive wrist ligamentous reconstruction in chronic cases should be avoided because of osteoporosis and wrist stiffness. Conclusively, clinical wrist symptoms are more important than radiological changes in wrist insta- bilites. So if the symptoms are not so severe, we must consider the patient's age, occupation and their social activities etc. to decide the proper way of treatment.

2.Postoperative Changes of Herniated Intervertebral Disc: Normal and Discitis MR Findings.

OBJECTIVE: To describe normal postoperative MR findings and MR findings of postoperative discitis in patients who underwent operation due to herniated intervertebral disc. MATERIALS AND METHODS: We retrospectively reviewed normal postoperative MR findings and MR findings of discitis in 30 patients(21-61yrs.) (13 cases dignosed as discitis and 17 cases as normal) who previonsly underwent laminectomy and discectomy, or bony fusion. We analyzed signal intensity of end plate and disc, end plate destruction, and enhancement of end plate and disc on T1- and T2-weighted images(WI) of 1.5T MRI. RESULTS: Among 14 out of 17 patients with no evidence of discitis, 7 patients showed high signal of the posterior portion of disc on T1- and T2-WI and 11 patients revealed enhancement at the same sites. In all 13 patients suspected of having discitis, end plate and disc showed low signal on T1-WI, high signal on T2-WI, heterogeneous enhancement, and irregular destruction of end plate. Meanwhile, 3 cases with no evidence of postoperative discitis clinically who underwent bony fusion showed similiar findings to those of the above 13 patients, except for homogeneous enhancement of end plate and vertebral body. CONCLUSION: The MR findings of postoperative discitis were low signal on T1-WI, high signal on T2-WI, and heterogeneous enhancement of and plate and disc, and destruction of end plate.

3.Development of a Questionnaire for Dietary Habit Survey of Korean Adults.

OBJECTIVES: The purposes of the study were to develop a questionnaire for dietary habit survey for Korean adults, and unify the terms related to dietary habits. METHODS: The Delphi method by 43 professionals on dietary habit research was applied to unify the terms. RESULTS: With regard to results on terms related to dietary habit, the respondents recorded the highest percentage (90.7%) of selecting the term "dietary habits" and also the highest percentage (76.7%) of choosing "dietary habits" for English. The biggest percentage of the respondents chose "individual dietary behaviors repeatedly formed and habitualized under the social, cultural, and psychological influence in the group" as the concept of dietary habits. The Delphi survey for the development of a questionnaire resulted in the first questionnaire of 31 items, the second one of 27 items, and the third one of 25 items. The validity of questionnaire items was tested with content validity ratios (CVR). The items whose CVR value was 0.29 or lower were eliminated or revised, because the minimum CVR value needed to test validity was 0.29. To test the reliability of questionnaire items, test-retest method was performed in 163 adults. According to the Kappa coefficient in the range of 0.314-0.716, all of the 25 items were in the reliability scope. A survey was taken with 702 adults to finally revise and supplement the third questionnaire whose validity and reliability were tested. CONCLUSIONS: Through those processes, a questionnaire for adults' dietary habit survey was finally completed. The significance of the study lies in the development of the first questionnaire on dietary habits equipped with both validity and reliability in South Korea.

4.Clinical Observation on Pediatric Urolithiasis.

A clinical observation was made on 13 cases of pediatric urolithiasis admitted to the Department of Urology. Han Il Hospital during the period from Jan., 1971 to Dec., 1975, and the following results were obtained. 1) Among 78 cases of pediatric in-patient, 13 cases were pediatric urolithiasis, 10 cases were male and 3 cases were female. 2) Age distribution was between 2 and 13 years, showing the highest incidence in 7yrs (23.0%). 3) The etiology of urinary tract stone: 2 cases in congenital anomaly, 8 cases in idiopathic, 1 case in foreign body, 1 case in immobilization, 1 case in recurrent infection. 4) Locational distribution of urinary tract stone was 5 cases in ureter, 4 cases in bladder, 3 cases in urethra, 2 cases in kidney. 5) The largest stone extracted was 2.0cm in size. 6) The common clinical manifestation of upper urinary tract stone included flank pain (71.4). hematuria (57. 1%) and lower urinary tract stone included painful urination (85. 7%), frequent urination (71.4%). 7) The most frequent method of treatment was lithotomy(57%). 8) The results of chemical analysis of 9 urinary calculi showed ca. oxalate in 33.3%, cal. Phosphate + oxalate in 33.3%.

7.Treatment of Unstable Fracture of the Thoraco

The spinal injuries were classified into bursting fracture, fracture-dislocation, seat-belt injury, compression fracture according to the three column theory by Denis. The bursting fracture and fracture-dislocation required the most careful planning. So, myelography, computerized tomography enabled us to diagnose the spinal fracture including retropulsed bony fragment into the spinal canal. There was much controversy as to appropriate treatment af unstable thoraco-lumbar fractures. The frequent surgical treatment of thoraco-lumbar spinal fractures was still posterior spinal instrumentation including Harrington rod system. Since 1964, the use of anterior spinal instrumentation had been started by Dwyer, Dunn, Kostrik, Slot, and Zielke used anterior spinal instrumentation in unstable thoraco-lumbar spinal fractures, but not popular. This study suggested the experience to accomplish the decompression of the neural elements and stabilization of the spine by using Kandea device in unstable thoraco-lumbar spinal fractures. 12 patients treated with this Kaneda device in unstable thoraco-lumbar spinal fractures were analyzed from Dec. 1988 to May, 1989 at the Department of Orthopaedic Surgery, Ewha Womans University Hospital. We obtained the following conclusions. The results were as follows. 1. The common injury mechanism was falling down in 5 cases, the frequent injury site was 1st lumbar vertebra. 2. According to Denis classification, the bursting fractures were in 8 cases(68% ), the fracture-dislocations were in 4 cases(33%). 3. The average preoperative kyphotic angulation was 24.5 degrees(range 8 to 45) and postoperative angulation was 5 degrees(range 2 to 15), the correction rate was 79.6% and correction degree was average 19.5 degrees. 4. The advantages were the fixation of one level above and below the injury site, sufficient neural decompression, firm spinal stabilization, early mobilization with a brace and elimination of the 2nd posterior procedure. 5. The fixation of vertebral plate was difficult due to the invariable size of vertebral plates, especially, in upper thoracic spinal fracture and children's fracture.

8.Research Trend of Nutrition through Analysis of Articles Published in 'Korean Journal of Community Nutrition'.

The purpose of this study was to examine the research trend of nutrition for the recent 12 years from 1996 to 2007 by analyzing 734 articles published in the Korean Journal of Community Nutrition. The majority of the articles (61.4%) were classified as survey types in terms of data collection methods. Most of the subjects used in the articles were adults (28.8%), and the subject whose research has been increased at the highest rate was "patients". The most frequent keywords in the title of articles were "nutrient intake" (231times), "food service" (92times), "dietary habits" (69times), and "obesity" (69times). The keywords that have appeared more frequently with the years were "osteoporosis" (450.0%), "menopause" (350.0%) and "dietary attitudes" (208.3%). As for research interests, "nutrient intake" was dominant in the early stage of research while "disease", "dietary habits", "dietary attitudes" and "nutrition education" have increased in recent years. Some of the most common methods of nutrition assessment were "dietary intake" (41.2%), "anthropometric" (34.0%) and "biochemical test" (14.7%). The most common methods of dietary intake were "24-hours recall" (28.6%) and "dietary habits" (23.3%). The results of this study showed some biases in data collection methods, gender of the subjects, and study areas. Moreover, inconsistent terminologies, questionnaire contents, and measures were used for the researches on dietary behaviors, dietary habits, dietary attitudes, which made it difficult to compare their results for each research. Therefore, standardized research methods and terminologies need to be developed regarding dietary practices.

9.Endocrine Study on Menstrual Irregularities in Wilson's Disease.

We have investigated endocrine functions of 2 patients with Wilson's disease who showed oligomenorrhea or amenorrhea, The serum basal levels of hypothalamic, pituitary, thyroid, adrenal cortical and ovarian hormones were determined. Then stimulation tests were performed with GnRH(50 microgram/m2), TRH(7 microgram/Kg) and insulin(0.l U/Kg) in one intravenous bolus, Levels of LH, FSH, prolactin, GH, TSH, total testosterone, l7 beta-estradiol, free T3, T4, DHEA-S and cortisol were measured by standard radioimmunoassays. The endocrine profiles which represent functions of hypothalamus, pituitary, thyroid and adrenal cortex was all proved to be normal. Serum estradiol and total testosterone levels were 59.0 microgram/ml ( normal ; 60-130 microgram/ml) and 2.9 ng / dl (normal ; 0.l5-1.1 ng/dl) in one case, and 20.5 microgram / ml and 0.69 ng / dl in the other respectiviely. Low estradial and high total testosterone levels seen in these cases suggested ovarian dysfuncton. Interference of ovarian follicular aromatase activity due to passible copper intoxication could explain these findings as the cause of the menstrual irregularity of patients with Wilson's disease.

10.MR Imaging of the Dural Ligaments' Cadaveric and Clinical Study.

OBJECTIVE: The purpose of this study was to describe the MR imaging appearance of the dural ligaments which is connected between the anterior dura mater and the posterior longitudinal ligament. MATERIALS AND METHODS: In 10 cadavers with formalin fixation, the dural ligaments were evaluated and photographed after removal of the posterior element of the spinal canal. MR studies of 12 patients who had distinct dural ligaments were evaluated. Spin-echo MR images were obtained on a 1. 5T unit. RESULTS: In seven of the ten cadavers, the midline dural ligament was observed; the lateral dural ligament was observed in 3 cadavers. On MR, the dural ligaments were imaged as thin lines of low signal intensity between the anterior dural margin and the posterior longitudinal ligament. Dural ligaments were well-visualized at the level of the lower lumbar spine which had a more prominent anterior epidural space than that of the upper level of the spine. Eight of 12 cases had well developed midline dural ligaments. Two cases showed distinct lateral dural ligaments and the remaining 2 cases had distinct midline and lateral dural ligaments. CONCLUSION: MR images of the dural ligament demonstrate a thin low signal line at the anterior epidural space.
