Primary aldosteronism, characterized by hypertension, hypokalemia, and hyperaldosteronemia resulting from chronic oversecretion of aldosterone independent of normal renin-angiotensin regulatory system, is due mostly to aldosteronoma or to bilateral cortical nodular hyperplasia. We report two cases of primary aldosteronism due to adrenal cortical adenoma, which were diagnosed by clinical data and abdominal computed tomographic scan. Clinical symptoms and laboratory data returned to normal after surgical adrenalectomy.

5.The role of mammography on breast cancer patients in a hospital setting.

From Oct. 1, 1983 to May 31, 1988, 6198 mammograms were taken and 443 mastectomies were performed at the Yonsei University Hospital, Seoul, Korea. Of these, 191 cases had preoperative mammography performed prior to surgery (study group) and 252 cases underwent mastectomies without preoperative mammography (control group). There were no significant differences between the two groups in pathological stages, axillary lymph node metastasis and size of the tumor, suggesting that little benefit is derived from the preoperative mammography in the hospital setting. However, there were a number of other findings, including a 14% incidence of multiple foci and localization of non-palpable cancer in 3% of cases, that could help us to make therapeutic plans and strategies for these patients.

6.Expression of Heme Oxygenase-1 in Ischemic Colitis.

BACKGROUND/AIMS: Ischemic colitis is a vascular condition of inadequate blood flow in the colon which leads to colonic inflammation and can cause significant morbidity and mortality. Oxidative stress is an early initiating event in ischemia and reperfusion injury. Heme oxygenase (HO) is considered to be an antioxidant enzyme that catabolizes heme to carbon monoxide, free iron and biliverdin. The aim of this study was to evaluate the expression patterns of HO-1, inducible form of HO, in ischemic colitis. METHODS: We analyzed the twelve cases of clinically and pathologically diagnosed ischemic colitis without surgical intervention compared with normal colon (n=10) and psedomembranous colitis (n=5). Immunohistochemical stainings for HO-1 were performed in paraffin-embedded tissues. RESULTS: The age of the patients ranged from 56 to 84 years (mean: 67 years) in ischemic colitis. Eight patients (66.7%) were female. The most common presenting symptom was bloody stool (66.7%) and rectosigmoid area (91.7%) of the large intestine was the most common ischemic site. Expression of HO-1 in ischemic colitis was high in contrast to normal colonic mucosa or psedomembranous colitis. CONCLUSIONS: Ischemic colitis usually involves the rectosigmoid area in elderly female patients with a history of bloody stool. High expression of HO-1 in ischemic colitis may be responsible for a protective mechanism to ischemia or heme injury.

7.Two Cases of Primitive Neuroectodermal Tumors in Infancy.

The authors are reporting two cases of huge primitive neuroectodermal tumors involving cerebral hemisphere. These are highly malignant neoplasm, both clinically and histologically. They are rapidly growing tumors, with a brief duration of symptoms and a rapidly progressive course. The patients were discharged after subtotal resecton of tumor mass with an uneventful course, but one was dead within 2 months after the operation. These tumors usually occur in children and young adult. We have experienced two cases surgically treated in infancy.

8.Clinical and electrophysiological characteristics in Korean patients with WPW syndrome.

This study was designed to investigate the clinical and electrophysiologic characteristics of WPW syndromes in Korea. A total of 400 symptomatic WPW syndrome patients were consecutively recruited. The most common documented symptomatic tachyarrhythmia was orthodromic atrioventricar reentrant tachycardia (75.3%), followed by atrial fibrillation (31.3%), and antidromic atrioventricular reentrant tachycardia (6.2%). There was a higher incidence of multiple bypass tract in patients with antidromic tachycardia than in those with orthodromic tachycardia (30.4 vs 4.3%, P < 0.001). The inducibility of tachyarrhythmia with electrophysiologic study in this study population was 95.8%. The most frequent location of the accessory pathway was the left free wall (48.0%), followed by the right free wall (29.1%), posterior septum (17.5%) and anterior septum (3.5%). These results indicated that 1) clinical and electrophysiological characteristics of Korean patients with WPW syndrome were similar to those of western countries and 2) the electrophysiologic study was important in the evaluation of patients with WPW syndrome.

10.A Case of Duodenal GIST Accompanied with Neurofibromatosis-1, Presenting with Gastrointestinal Bleeding.

Gastrointestinal stroma tumors (GISTs) are CD117- positive primary mesenchymal tumors of the gastrointestinal tract and are noted to have a possible non-random association with neurofibromatosis-1 (NF-1, Von Recklinghausen disease). We report a case of a duodenal GIST presenting with gastrointestinal bleeding in a 74-year-old female, and this condition was accompanied with NF-1. A upper gastrointestinal endoscopy and abdominal computed tomography scan revealed several submucosal tumors in the duodenum, jejunum and ileum. Histological and immunohistochemical studies on the surgical resection specimen revealed gastrointestinal stromal tumors of an uncommitted type. The patient was treated with local excision of the tumors and is now in a favorable state.
