1.Arterial Embolization for Management of Hemoptysis.

PURPOSE: Our purpose in this study is to evaluate the effectiveness of bronchial and nonbronchial systemic arteries for the control of hemoptysis. MATERIALS AND METHODS: Seventy patients with massive or recurrent hemoptysis underwent percutaneous transcatheter embolotherapy between 1991 and 1993. Retrospectively we reviewed 77 cases of bronchial artery embotization and 32 cases of nonbronchial systemic artery embolization in the 70 patients. RESULTS: Immdiate control of hemoptysis was achieved in 33 of 39 patients with massive hemoptysis(84.6%) and 20 of 24 patients with recurrent hemopt ysis(83.3%). In 32 cases, nonbronchial systemic arteries contributed significantly to areas of pathologic pulmonary tissue and frequently were the major arterial supply. CONCLUSION: Bronchial artery embolization is an effective and life saving procedure in non-surgical candidates. Recognition and occlusion of nonbronchial systemic arteries that feed to hypervascular pulmonary lesions is essential for successful percutaneous embolotherapy of hemoptysis.

5.Surgical Management for Incidental Finding of an Intrapulmonary Foreign Body : A case report.

A 48-years-old woman was visited to our hospital because of incidental finding of intrapulmonary foreign body. Chest X-ray showed a 4cm sized foreign body in left upper lung field without adjacent fibrosis. Chest CT showed a sewing needle shaped foreign body of metallic density, located in the left upper lobe. The foreign bodies including the needle were removed surgically using a wedge resection. The extracted needle was divided into three 4 cm pieces. Patient was discharged without other respiratory symptoms after surgery. We report a case of wedge resection in a patient with intrapulmonary needle in the left upper lobe, with review of literatures.

6.Statistical Observation for Admitted Patients during the Years of 1974 to 1977 at Ped. Dept. of Han-Il Hospital.

Statistical analysis according to W.H.O. classification for the patients admitted the Ped. Dept. of Han-Il Hospital was carried out during 4 years from Jan. 1974 to Dec. 1977. The following results were observed. 1. Total No. of patient during 4 years period were 1955, of which 1199 were male(61.33%, 756 were female(38.67%) and male to femal ratio was 1.57:1. 2. There was no significant variation. 3. According to age, preschool aged group as the most frequent group consistin of 488 cases(24.49%), school aged group and adolescent in the order of frequency. 4. On monthly distribution, there was no significant variation 5. Most frequent diseases in pediatric age group were respiratory tract one, 851 cases(39.29%) : infectious and parasitic ones, 553(25.53%) and neonatal disease, 204 cases(9.42%) in the order of frequency. 6. Among respiratory tract disease, pneumonia was the most frequent one, 587 cases(27.10%). 7. Among infectious and parasitic disease group, gastrointestinal infectious one was the most frequent, 224 cases(10.34%) : other viral disease and tuberculosis in the order of frequency. 8. Among neonatal diseases, prematurity was the most frequent one, 82 cases(3.79%).

7.A Case of Uhls Anomaly.

Congenital hypoplasia of the right ventricular myocardium, also known as parchment heart or Uhl's anomaly, is a rare congenital heart defect. It was first described in Oslers principles and Practice of Medicine in 1905 and reviewed by Segall We found 29 reported cases, but there was no reported case in Korea. Recently, we experienced a case of Uhls anomaly associated with patent ductus arteriosus, atrial septal defect, pulmonary atresia and tricuspid abnormality in a 3 month-old infant, which was confirmed by autopsy. We present a case with a brief review of litteratures.

Of 7 cases of Guillain-Barre syndrome reviewed, which was admitted to the Department of Pediatrics Yonsei University, College of Medicine during 3 months period from July through September 1980, serologic surveys for an infectious agent were found to be without demonstrable causes such as cytomegalovirus and measles virus. We reviewed 87 acses of Guillain-Barre syndrome, which was admitted to our hospital during 10 years from January 1970 through December 1979. In age incidence, 57% of patients were under 5 years of age. Males were affected more often than females; the ratio was 2:1. In seasonal incidence, 59% of patients occured in summer time.

10.The Significance of Serum Carcinoembryonic Antigen in Curative Surgery of Colorectal Cancer.

Carcinoembryonic antigen (CEA) in now the most widely used and the most useful marker for many cancers, including those of the colon, lung, pancreas, and breast. Also CEA is widely used for detection, staging, recurrence, and assessing the response to therapy in colorectal cancer. METHODS: From 1992 to 1998 the clinical value of the pre- and postoperative serum levels of CEA who underwent curative surgery at Department of General Surgery, College of Medicine, Dongguk University KyungJu Hospital, in 140 s patient with colorectal cancer with abnormal levels of CEA (>or=5 ng/ml) was investigated. RESULTS: The results are as follows: 1) The positive rate of preoperative CEA level was 47%, so preoperative CEA level measurement was not useful as screening test for colorectal cancer. 2) There was no significant association between abnormal CEA level and the location of tumor. 3) There was significant association between increased levels of preoperative serum CEA and lymph node metastases. 4) The incidence of preoperatively elevated CEA levels in Dukes stages A, B, C, and D was 0%, 27%, 63%, 71%, respectively. There was significant association between increased levels of the preoperative serum CEA and the progressive stages of colorectal cancers. 5) There was no significant association between abnormal CEA level and histologic differentiation of tumor. In addition, there was no significant association between abnormal CEA level and ploidy status of tumor. 6) The recurrence rate was 20% and 77% in patients with preoperative levels of CEA<5 ng/ml and >5 ng/ml, respectively. 7) The recurrence rate was 11% and 64% in patients with postoperative levels of CEA <5 ng/ml and >5 ng/ml, respectively. 8) Considering as normal CEA levels up to 5.0 ng/ml, sensitivity was found to be 77%, specificity, 80%, and predictive value of an elevated CEA concentration, 77%. CONCLUSIONS: In conclusion, it is suggested that measurement of preoperative and serial postoperative CEA is very useful in assessing the prognosis and in detecting recurrences in colorectal cancer.
