1.A case of Spontaneous Gastric Perforation of th4e NewBorn.

A full term nomal baby girl who born by C.S. due to C.P.D. had been done well until the age of 5 days when she suddenly developed vomiting & abdominal distension & became not doing well? Immediate abdominal X-ray revealed free air under the both diaphragm without gastric air bubble & air fluid level. Emergency operation was performed under the tentative diagnosis of stomach perforation & found a laceration, 6cm in length in the great curvature. She died 6 hrs after operation. We reported this case with a brief review of literature.

4.Immunomodulators Extracted from Korean - style Fermented Soybean Paste and Their Function . 1 . Isolation of B Cell Mitogen from Korean - style Fermented Soybean Paste.

Responses of mouse lymphocytes to the soybean paste fermented by Korean traditional fashion was examined to clarify its effects in cytokine production in vitro. A fraction of the soybean paste (KFSP-100) was prepared by precipitation with ammonium sulfate and by filtration through ultrafiltration membrane. KFSP-100 were added into cultures of fresh mouse splenic cells in vitro. KFSP-100 significantly enhanced the amount of IL-6 and TNF-a produced by macrophages and IL-6 and IFN-r produced by lymphocytes. Production of IL-12 by macrophages was not much affected by KFSP-100 treatments. The most noticeable finding was the fact that lymphocytes treated with KFSP-100 proliferated to an exceeding numbers (more than 10 times to the control) in 72 hours. The KFSP-100-induced proliferative response was specific to B cells since almost all of the KFSP-100-induced cells in the cultures of splenic cells were B cells. Furthermore, such a proliferative responses were equally observed only in cultures of purified B cells but not in cultures of T cells. In thermostability test, the biologically active components of the KFSP-100 is assumed to be either linear protein or glycoprotein. KFSP-100 did not induce agglutination of lymphocytes demonstrated by lectins in the same cells. These observations suggest that KFSP-100 may be a novel mitogen for B lymphocytes. The component (s) responsible for the B cell proliferation in KFSP-100 might be a factor gained by natural fermentation. None of the fractions of not fermented soybean paste prepared by the same methods demonstrate the same effect.

Congentally corrected transposition of the great vessels can best be defined as a malformation in which the aorta and pulmonary artery are transposed in relation to each other but in which the flow of blood is in the physiologic direction. And in addition to the malposition of great vessels, there is an inversion of the ventrice,. Unfortunately this basic malformation seldom exists withot complicating cardiovascular anomalies. We had experienced a case of congenitally corrected transposition of the great vessels associated with itral?insufficiency in 13year-old boy, who presented the most above mentioned findings. We have made diagnosis by cardiac catheterization and selective angiocardiographic study.

7.Assessments of Activities of Daily Living for Disabled Persons in Some Rural Communities.

OBJECTIVE: Aims of this study are to assess the activities of daily living (ADL) for disabled persons in some rural communities using an appropriately developed ADL scale and to analyse the relationship between severity of impairment and ADL score. METHOD: ADLs of 98 disabled persons in two myeons of Kyunggido were assessed from May 1996 to August 1997. The newly developed ADL index composed of eight basic ADL (B-ADL) and two instrumental ADL (I-ADL) parameters were used. Impairment defined by the National Reparation Law was divided into a severely disabled group, grades 1 to 6 and a mildly disabled group, graded 7 to 14. Correlations between the impairment grade of each body part and the assessed ADL score were studied. RESULTS: 1) The most dependent ADL item of ten ADL parameters was performance in the public office (37.8%) and, the most independent ADL item was feeding (98.0%). 2) With severe impairment of the upper or lower limb, all ADL scores except for feeding were significantly low (p<0.05). In addition, with severe hearing impairment, scores of performance in public office were accessed low (p<0.05). 3)With severe lower limb impairment, average scores of I-ADL and B-ADL were low (p<0.05). 4) With severe language impairment, the average I-ADL score was also low (p<0.05). CONCLUSION: The task of performance in the public office was the most difficult ADL item for disabled persons. All ADL parameters except feeding were affected for disabled persons with lower or upper limb impairment. We think this pilot study may be used to develop more appropriate ADL scales for community based rehabilitation in the future.

8.A Case of Nephrotic Syndrome in Congenital Syphilis.

9.Nonsimultaneous Bilateral Internal Oblique Muscle Rupture in a Professional Baseball Batter: A Case Report

Abdominal oblique muscle injuries are relatively common in professional baseball players and can result in substantial loss of playing time. It is usually caused by a sudden movement of the torso in sports involving repetitive activity requiring trunk rotation and it tend to occur on the contralateral side of the dominant arm. We report a unique case of sequentially occurred bilateral abdominal internal oblique muscle rupture in a right-handed professional baseball batter over two seasons. Each internal oblique rupture had a different mechanism of the injury, the non-dominant side occurred during bat swing and the dominant side during bent leg sliding. After rest and rehabilitation, each injury was recovered to play in about 1 month.
10.Latarjet Procedure Screw Guide Designed with a 3.5 mm Locking Compression Plate Piece: Technical Note

Traditional soft tissue Bankart repair has high failure rates in the setting of a critical glenoid bone defect in recurrent shoulder dislocation.Hence, coracoid transfer, particularly the Latarjet procedure, can be performed to reduce the recurrence. A commercialized guide can be used during the Latarjet procedure. On the other hand, it is difficult to handle the guide parallel to the glenoid articular surface because of the predetermined fixed direction of the guide, and the guide pin is so thin that it can be broken in the wrong direction of insertion. Screw holes can also be drilled using the free hand technique without a guide, but it is inaccurate in maintaining a parallel direction or constant spacing between the screw holes. This study devised an instrument by cutting a 3.5 mm locking compression plate used in fracture surgeries, and the Latarjet procedure was performed relatively easily.
