2.Neuroblastoma: Computed Tomographic Findings.

PURPOSE: To evaluate the characteristic CT findings of neuroblastoma, we studied neurobalstomas. METHODS AND MATERIALS: We analysed CT findings of available 25 cases among pathologically proved 51 neurobalstomas from Jan. 1983 to Sept. 1990. RESULTS: The most frequent site of origin is adrenl gland (40%) and the second is retroperitoneum (32%) and the third ismediastinum (16%). Characteristic CT findings are as follows:Calcifications within the tumor is detected in 86% of abdominal neuroblastomas and 50% of mediastinal origin. Hemorrhagic and necrotic changes within the tumor is noted at 86% in the tumor of abdominal origin and 25% in mediastinal neuroblastomas. Contrast enhanced study showed frequently septated enhanced appearance with/without solid contrast enhancement. Encasements of major great vessels such as aorta and IVC with/without displacement by metastatic lymphnodes or tumor are frequently seen in 90% of abdominal neuroblastomas. Multiple lymphadenopathy are detected in 95% of abdominal neurobalstomas and 25% of mediastinal neuroblastomas. The most common organ of contiguous direct invasion is kidney in 6 cases and the next one is liver but intraspinal canal invasion is also noted in 2 cases. CONCLUSION: We concluded that diagnosis of neuroblastoma would be easily obtained in masses of pediatric group from recognition of above characteristic findings.

3.The Value of Computerized Tomography in Head Injury Patients with No Neurological Deficits.

Computerized tomography(CT) has proven to be far superior to other diagnostic studies in the detection of intracranial lesions after head injuries. The procedure is especially appropriate for early and accurate diagnosis because it can be done quickly with no discomfort and minimal risk. CT was utilized as a screening diagnostic procedures in 100 consecutive patients of head injuries who were alert and neurologically no deficits in a 8 months period. 11 abnormal CT Scans found at 10 patients:six epidural hematomas, two intracerebral hematomas, one sudural hygroma and one temporal contusion. The cases presented are discussed in light of pertinent literature.

5.Clinical Outcomes of Advanced Surface Ablation with Smoothing in High Myopia.

PURPOSE: To investigate the results of Advanced Surface Ablation (ASA) coupled with "smoothing" to smooth the ablation surface after covering masking fluid. METHODS: ASA was performed in 61 eyes with ablation depth of more than 75 microm. The mean refractive error was -5.88 +/- 1.27 D and mean ablation depth was 102.93 +/- 12.06 microm. Smoothing was performed in all patients (mean depth 16.79 +/- 2.43 microm, mean diameter 8.77 +/- 0.16 mm). Customized Aspheric Transition zone (CATz) was used in the laser algorithm. RESULTS: The mean refractive error was -0.29 +/- 0.41 D at postoperative 6 months and 97% of patients had an UCVA of 1.0 or better at postoperative 6 months. There was no statistically significant difference in magnitude of high-order aberrations at postoperative 6 months. The magnitude of total spherical aberrations increase was 0.084 microm at postoperative 6 months (p<0.05, paired t-test). The cornea was maintained clear in the majority of eyes. CONCLUSIONS: Excellent results were obtained by ASA coupled with "smoothing" in high myopia patients with an ablation depth greater than 75 microm.

6.Treatment of Acute Retinopathy of Prematurity with Argon Indirect Laser Opthalmoscope.

Twenty two eyes of twelve infants with threshold or prethreshold retinopathy of prematurity were treated by using the Argon laser through an indirect ophthalmoscopic delivery system. Eyes that had persistent plus disease were retreated to the missed areas. Mean follow-up period was 8.1 months. 20 of 22 eyes were regressed and all of prethreshold ROP were regressed. Our results suggest that argon laser photocoagulation is as effective as cryotherapy in the treatment of ROP and excellent results can be obtained by treating prethreshold zone I or posterior zone II ROP. And laser treatment is better tolerated by infant, accurate, easy, less destructive and not requires general anesthesia.

7.A Case of Congenital Cataract in Down's Syndrome.

In most cases of chromosomal anomaly of congenital cataract, the cause is known as Down's syndrome. We experienced a case of bilateral congenital cataract in Trisomy 21 (Down's syndrome). A 3 year-old female who had been born from 37 year-old mother was visited to our hospital, with clinical diagnosis of bilateral congenital cataract. Her face was pathognomonic to Down's syndrome. Chromosomal study of peripheral blood showed that the total number of chromosome was 47, and karyotype was 47, XX, +21. The lens showed nuclear opacity. Aspiration and Discission of the cataract on both eyes were performed uneventfully.

Spontaneous coronary artery dissection is a rarely identified entity whose exact incidence, etiology, pathogenesis, medium-term evolution and optimal treatment have not yet been firmly estabilished. The cause of spontaneous disection remains unclear but theories of etiology include a medial eosinophilic angiitis, pregnancy induced degeneration of collagen and rupture of the vasovasoum. Most paients die suddenly, but a clinical spectrum is seen including and unstable angina, myocardial infarction and cardiogenic shock. We experienced 4 cases with spontaneous coronary artery dissection found angiographically which caused myocardial infarction and unstable angina. Our patients were treated medically.

9.Effect of the Illumination and the Types of the Lenses on Near Visual Acuity in Low Vison Patients.

To obtain adequate information of illumination and lens types for visual acuity test of low vision patient, the relation between various illumination and vision was evaluated. Visual acuity for near and effective lens types were examined. Among the 28 patients (of 36 eyes), intensities of illumination were set at 200, 500, 1000 and 2000lux and the lenses of biconvex, aspheric and aplanatic were tested. The best visible range of illumination for the low vision patients was 500-1000lux, with lower power diopter for the same visual acuity and with aspherin and aplantic lenses. Results attained by our study may be applied for appropriate condition of illumiation and type of lens in the low vision care.

10.Periarticular Osteoporosis Is a Prominent Feature in Early Rheumatoid Arthritis: Estimation Using Shaft to Periarticular Bone Mineral Density Ratio.

We aimed to quantify periarticular osteoporosis and investigate its significance in 45 patients with rheumatoid arthritis (RA) and 106 controls. Dual-energy X-ray absorptiometry (DXA) was used to determine the ratio of shaft to periarticular bone mineral density (BMD) as an index of periarticular demineralization. Periarticular osteoporosis was measured by conventional radiography. The BMDs of shaft and periarticular regions in eight designated areas on proximal phalanges were quantified. Clinical variables were examined to identify risk factors for periarticular osteoporosis. The assessment of periarticular osteoporosis on X-ray images reached a moderate degree of interobserver agreement among four physicians (k = 0.47). For BMD quantification, we designed three types of mathematical formulae: the ratio of shaft to periarticular BMD, the mean of the ratios, and the ratio of the sums. These ratios were significantly higher in the patients with early RA (disease duration < or = 3 yr) than in controls (P < 0.01). The findings were not as distinctive in patients with established RA. Body mass index, cumulative dose of corticosteroid, and C-terminal telopeptide were correlated with BMD ratios. Conclusively, DXA-assisted localized quantification and BMD ratio calculations are feasible for assessing periarticular demineralization. Periarticular osteoporosis is a relatively distinctive feature of early RA.
