2.Study on the Death Rates and Causes of Death.

To use basic data for health planning and evaluation of Kangwha community health project of Yonsei University, a study on death rates and causes of death were investigated in two townships (Naega and Sunwon Myuns) in Kangwha County from April 1, 1975 to March 31, 1977). All death was identified and reported by family health workers who are living in each village and 2 trained public health nurses confirmed the death. The causes of death were investigated by 2 public physicians. Total number of deaths for 2 years was 230 and the followings are brief summary of the study. 1. Age-adjusted crude death rates of study area were 8.69 per 1,000 population in 1975 and 7.18 per 1,000 population in 1976. Age-adjusted crude death rates for male were 9.18 in 1975 and 6.38 in 1976 and for female were 8.33 and 7.80 per 1,000 population. 2. Age specific death rate curves by year and sex showed "U" shapes. 3. Infant and neonatal death rates were 30.08 and 22.56 per 1,000 live births in 1975, and the rates in 1976 were 18.18 and 13.64. 4. The most common cause of death was cerebrovascular disease and average cause specific death rate for the disease was 215.5 per 100,000 population. 5. Four leading causes of death were non-infectious origin ; cerevrovascular disease, malignant neoplasms, senility and suicide. Pulmonaty tubeculosis and pneumonia occupied 5th and 9th causes of death. 6. Stomach cancer and hepatoma occupied 61.3% of total death due to malignant neoplasms. 7. Most frequent cause of neonatal death was birth injury. Two deaths due to tetanus were found in 1975, but no death due to this disease was found in 1976. 8. About half of deceased received care from physician before death.

6.Study on the frequency distribution of the metacercarial density of Clonorchis sinensis in fish host, Pseudorasbora parva.

Since the pattern of the frequency distribution of a parasite within a host showed an overdispersed pattern, various statistical models such as Poisson, negative binomial and lognormal distributions have been applied on the population dynamics in host-parasite relations. The observed data on the number of metacercariae of Clonorchis sinensis in a suitable intermediate host, Pseudorasbora parva which were collected from an endemic area, Juk-Rim River, Kim-Hae Goon, South Kyong-Sang Do were applied on the statistical models. The results obtained are as follows: By the calculation of the raw data, 258(94.9 percent) out of 272 fish showed positive to the metacercarial infection of C. sinensis. The mean number of metacercaria in a fish was 335.1. The standard deviation was 250.6 and the mode was observed between 250 and 299. The frequency distribution pattern of metacercariae of C. sinensis within the fish host in surveyed area was fitted to the lognormal distribution (0.05

7.A mathematical approach to the mode of transmission of clonorchiasis in the inhabitants of Nak-dong and Han River basin.

To understand the mode of transmission in clonorchiasis, a survey was made in Kim-hae Goon, South Kyong-sang Do (=Province). The mathematical analysis of the age prevalence was done on the egg positive rates. And another analysis for the comparison was also made to the cited data from two areas, North Kyong-sang Do and Ko-yang Goon, Kyong-gi Do. Some catalytic models of H. Muench (1959) were applied to the observed age prevalence. Because the both parameters, such as force of infection(a) and loss of positivity(b) were considered to be constant for a long period in the surveyed area, the two stage catalytic model by Muench was chosen to the analysis. In the surveyed area, Kim-hae Goon where the egg positive rates were 56.2 percent and 61.2 percent (by Kim, 1974), the constant values of 'a' were found to be 0.051 and 0.089 respectively. In other words, the force of infection was 51, 89 per 1,000 susceptibles. The values of 'b' were found to be 0.006 and 0.005. This means that the rates of disappearance from egg positive cases to negative were 6 and 5 per annum per l,000 positive cases in above area. Therefore, the two catalytic curves were expressed by the following equations, y = 1.133 {e(-0.006t) - e(-0.051t)} and y = 1.047 {e(-0.005t) - e(-0.089t)} respectively. In the cases of North Kyong-sang Do and Ko-yang Goon, Kyong-gi Do where the egg positive rates of clonorchis shown as 27.7 percent and 15.2 percent by Shin (1964) and Kim (l974), the curves were expressed by y = 1.769 {e(-0.010t) - e(-0.034t)} and y = 2.857 {e(-0.020t) - e(-0.027t)} respectively. From the above mathematical analyses by age prevalence in clonorchiasis, it was considered that the mode of transmission of clonorchiasis in the surveyed area, Kim-hae Goon presented more rapid pattern than those of North Kyong-sang Do and Ko-yang Goon, Kyong-gi Do.

8.Echocardiographic Analysis of Left Ventricular Function in Hypertensive Patients.

The echocardiographic results obtained from normal group(910 cases), borderline hypertention group(78 cases), define hypertension group(182 cases) and hypertensive heart disease group(55 cases) in Heart Center, Paik Hospital, Inje Medical College, Pusan, Korea between September 1985, were as followings; 1) LVDeD, 5.94+/-1.07cm in hypertensive heart disease group was significantly increased compared with 4.93+/-0.64cm in normal group, 4.92+/-0.73cm in borderline hypertension group and 4.90+/-0.64cm in define hypertension group. LVSeD, 4.86+/-0.98cm in hypertensive heart disease group was significantly increased compared with 3.24+/-0.58cm in normal group, 3.21+/-0.52cm in definite hypertension group. 2) LVDeV/s, 180.26+/-84.76ml/m2 in hypertensive heart disease group was significantly increased compared with 86.13+/-37.34ml/m2 in normal group, 86.26+/-41.02ml/m2 in borderline hypertension group and 83.78+/-30.86ml/m2 in definite hypertension group, and LVSeV/S, 97.51+/-55.67ml/m2 in hypertensive heart disease group was significantly increased compared with 25.28+/-13.25ml/m2 in normal group, 23.68+/-14.62ml/m2 in borderline hypertension group and 24.02+/-14.36ml/m2 in definite hypertension group. 3) There was no significant differance of SV/S, between 61.50+/-26.02ml/m2 in normal, 63.98+/-30.86ml/m2 in borderline, 63.06+/-22.96ml/m2 in definite hypertension group and 61.82+/-30.67ml/m2 in hypertesive heart disease group. E.F. 44.06+/-9.94% in hypertensive heart disease group was significantly reduced comparing with 71.08+/-8.89% in normal group, while 74.35+/-8.26% in borderline and 73.28+/-8.49% in definite hypertensive was significantly increased. F.S., 17.02+/-5.18% in hypertensive heart disease group was significantly reduced comparing with 34.44+/-6.81% in normal, while 35.64+/-7.62% in borderline and 36.87+/-7.26% in definite hypertension group was significantly increased. 4) VSTh, 1.24+/-0.32cm in hypertensive heart disease group, 1.17+/-0.12cm in definite hypertension group and 1.16+/-0.41cm in borderline hypertension group are significantly increased comparing with 0.99+/-0.21cm in normal group. LVPWTh, 1.01+/-0.27cm in hypertensive heart disease group and 0.90+/-0.21cm in definite hypertension group was significantly increased comparing with 0.79+/-0.16cm in normal group. 5) VSTh was increased in 13.3% of borderline hypertension group, 32.8% of definite hypertension group and 31.6% of hypertensive heart disease group. LVPWTh was increased in 4.4% of borderline hypertensive group, 26.9% of definite hypertension group and 30.2% of hypertensive heart disease group. Symmetrical septal hypertrophy was noted in 5.1% of borderline hypertension group, 20.1% of definite hypertension group and 32.1% of hypertensive heart disease group. Total septal or left ventricular posteior wall hypertrophy was noted in 17(21.8%) of 78 borderline hypertension group, 154(89.8%) of definite hypertension group and 51(93.9%) of 55 hypertensive heart disease group.
