2.The Significance of Sample Preparation in Measurement of Ionized Calcium.

BACKGROUND: Calcium status is more accurately determined by measuring free calcium, the tightly regulated biologically active form. The concentration of ionized calcium is strongly dependent on different preanalytic factors. In this study the influence of several methodological factors on the concentration of ionized calcium in blood is investigated. METHODS: Authors selected 127 persons of health care management center & comparatively healthy-look, out-patients of our hospital. When serum was needed, blood was anaerobically withdrawn in vacutainers, the serum was separated after standing at room temperature. For the plasma sample blood was anaerobically drawn into the tube with dry sodium heparin 143 IU/10ml blood in the same patient. And then, to avoid CO2 loss, the samples were left unopened and centrifuged anaerobically at 900g for 15 min; the serum and plasma were then pipetted as quickly as possible into 2ml plastic eppendorf-tube, which were completely filled and sealed off immediately and keeping it in refrigeration before testing. For the studies of calcium binding effect by different volume of sodium heparin. blood was collected into two type of tube, each containing 30IU heparin/whole blood ml or 125 IU/ml. Ioniged calcium were measured by ion-selective electrodes. RESULTS: 1. The reference value of ionized calcium in serum and plasma was 4.9+/- 0.19, 4.9+/-0.17 mg/ml(serum versus plasma, p>0.05) respectively. 2. The concentration change of ionized calcium according to heparin volume shows no significant difference until heparin 14.3 IU/blood 1 ml compared with serum. 3. The concentration of ionized calcium of serum and plasma was stable until 4 hours and 4 days after serum and plasma separation. CONCLUSIONS: Above shows that the concentration of ionized calcium have the same reference range on both serum and plasma. But each laboratory should have their own reference range according to heparin volume, ionized calcium in serum and plasma samples kept at -4degrees C remains stable within few days, provided the proposed conditions for storage.

3.Clinical Application of Polymerase Chain Reaction for the Diagnosis of Extrapulmonary Tuberculosis.

BACKGROUND: There are many reports showing the efficacy of polymerase chain reaction(PCR) for the diagnosis of Mycobacterium tuberculosis in sputum. but only few reports in extrapulmonary specimens. Because of the difficulty in establishing a diagnosis of tuberculosis in the extrapulmonary specimens there have been considerable interest in the development of a rapid sensitive diagnostic test that might be useful. Therefore we used PCR for detection of M. tuberculosis DNA in extrapulmonary specimens and compared the results of conventional acid-fast stain, culture methods and PCR assay. METHODS: Total of 63 clinical samples(10 cerebrospinal fluids, 12 pleural fluids, 1 pericardial fluid, 3 bone marrow aspirates, 1 ascitic fluid, 25 fine needle aspirates of lymph nodes, 7 urine, 1 stool and 3 tissue biopsies) in Ewha Womans University Tongdaemun hospital were analysed by the PCR. We performed the PCR using a species-specific M. tuberculosis DNA fragment(mtp 40 gene) as primers that was cloned and sequenced at recent and a 396-bp fragment was specifically amplified. We analyzed sensitivity and specificity of AFB culture and PCR for the diagnosis of extrapulomonary tuberculosis. RESULTS: The positivity of AFB smear, culture and PCR were 2(10%), 4(20%), 13(65%) out of total 20 cases diagnosed as clinically active extrapulmonary tuberculosis. respectively. All of 2 smear-positive samples and 2 of 4 culture-positive and smear-negative samples were PCR-positive. And 9 of 14 smear and culture negative specimens also gave detectable DNA products in PCR The specificity of PCR(95.4%) is compared with those of smear and culture(100.0%). CONCLUSIONS: This results suggest that the PCR assay is a sensitive and rapid diagnostic alternative to classical procedures for the diagnosis of extrapulmonary tuberculosis.

4.MR measurement of normal pituitary gland height on midsagittal section: age and sex differentiation.

Measurement of the normal pituitary gland height was performed on magnetic resonance (MR) images of 144 subjects with no known or suspected pituitary or hypothalamic diseases. Midsagittal T1 weighted images (T1WI) were used for measurement, and mean vertical height according to age and sex group was obtained. In all age groups, the pituitary height was greater in females than in meles, The group of 0-9 years in both genders showed the minimum mean pituitary height, The maximum mean height was observed in the 10-19 years age group in both genders. The height gradually decreased with increasing age after age 20 years. There was no subject with a height more than 9.0 mm in females or 8.0 mm in males. In conclusion the measurement of the normal pituitary pland height using mid sagittal MR imaging can be used for the evaluation of the pituitary gland lesions.

5.Study on Effect of Conjugated Equine Estrogen and Progestogen on Serum Lipid Profiles and Bone Mineral Density in Postmenopausal Women.

Hormone replacement therapy combined with progestogens induces changes in effect of estrogen on serum lipid levels and it has been known that the changes depend on a type and dosage of progestogen. It is also known that progestational agent induces positive ch-ange in bone mineral density. To study the effects of progestogen on lipoprotein and bone metabolism, we administ- ered conjugated equine estrogen 0.625 mg alone to 50 postmenopausal women, in combinat- ion with medroxy- progesterone acetate 5 mg to 40 postmenopausal women. The data demonstrated a beneficial effect in lipoprotein profiles in both groups. Total cholesterol in two groups decreased from the baseline values, LDL-cholesterol decreased significantly by 4.8 % in group I and 16.2 % in group II(p < 0.05), HDL-cholesterol increa- sed significantly by 11.3 % in group I and 14.7 % in group II(p < 0.05), triglyceride incre- ased slightly in both groups. Bone mineral density of femur was maintained and BMD of vertebrae increased by 1.1 % in group I and 2.0 % in group II, but it is not statistically significant. The differences of changes between two groups were not statistically significa- nt. Our results suggest that medroxyprogesterone acetate have no adverse effect on HDL -cholesterol and have no additive effect on bone mineral density in hormone replacement therapy.

6.Nutritional Status of Mentally Retarded Children by Residence and by Degree of Handicap.

We studied the nutritional status of on mentally handicapped children living at home or in institutions since early teenage years are nutritionally important. The subjects of 7 to 12 year old mentally retarded children attending a special education school in Seoul were surveyed with questionnaires as well as 2-day dietary recall records, with the help of persons of their care-giver when needed. Among the 64 children, 54.7% are living in institutions and the rest of them are living at home. They were ranged from the trainable (64.1%), the educable (26.6%), and the non-trainable (9.4%). Their average daily intake of energy intakes (%RDA) was 2,070.1 kcal (94.1%), Ca 603.9 mg (75.5%), Fe 11.1 mg (92.5%), Vt.A 507.5 RE (84.6%), Vt. B2 1.1 g (88.2%), niacin 14.1 g (93.6%) and Vt. C 58.2 g (83.1%). Their average intakes of these nutrients were significantly higher in subjects of institutions than at home. The nutrients consumed at a much higher level than the RDA of the normal children were Vt. B1 (1.6 g, 146.8%) and protein (75.3g, 136.9%). The higher percentage of children at home were under consumed of several nutrients (< 75% RDA) than ones in institutions. When comparing the degree of handicap, energy and nutrient intakes except Vt. C were highest in educable children than trainable ones or Dawn's children. MAR of the diets of the subjects was 0.84. Children at home showed lower MAR as well as NAR of each nutrients, whereas children belonged to INQ < 1 were less at home. Handicapped children at home were snacking higher amount relative to their calorie intake and too frequently, that may lead to their poor nutrition. There was positive correlations between factors of nutrition and physical and dietary behaviors, but there were no correlations between factors of nutrition and health-related habits. Nutritional caring mentally handicapped children in institutions seemed to be more effectively managed.

BACKGROUND: Hepatitis B vaccination schedule commonly used in Korea is divided largely into 0, 1, 2 month scheduled vaccination group(0, 1, 2 group) and 0, 1, 6 month scheduled vaccination gorup(0, 1, 6 group). The only difference bet.ween two groups is the interval from 2nd dose to 3rd dose. This st,udy had been carried out, to find whether t.he difference of vaccination interval influence the vaccination complet,ion rate or not. METHODS: Study objects are 135 persons over 20 years old who had heptitis B vaccination more than once in injection room of Dongkuk University Kyong-ju Hospital from Jan. 1st in 1996 to Dec. 31th in 1996. Data about vaccination completion were gathered from record book of injection room and telephone interview. RESULTS: Hepatitis B vaccination completion rate is 73.8% in 0, 1, 2 group and 72.5% in 0, 1, 6 group. The reasons for incomplete vaccination are forgetting vaccinat.ion date(36.4% in 0, 1, 2 group and 50% in 0, 1, 6 group), having no time to spare for vaccination(54.5% in 0, 1, 2 group and 43% in 0, 1, 6 group) and knowing positive HBsAb before completion of scheduled vaccination(9.1% in 0, 1, 2 group and 7% in 0, 1, 6 group). CONCLUSIONS: There is no difference in Hepatitis B vaccination complet,ion rate bet.ween 0, 1, 2 group and 0, 1, 6 group.

10.A Basic Study on Improvement and Computerization of Nursing Record.

This study was designed to develop a basic plan for computerization of nursing records. The subjects were 7 nursing record forms, 58 charts, 23 nurses, 2 nurses managers, a nurse and computer specialist, 16 master course students and 3 professors. Data collection was conducted through questionnaire, observation and interview. The collected data were analyzed for problems, plan of improvement and needs for computerization. Based upon these results, it is recommended that nursing record computerization was needed a basic plan to integrate needs of nursing record computerization. The basic plan as follows: 1. To illustrate a data flow path of nursing record and data dictionary that show nurse's work and record process. 2. To establish a system in order to use multi-tasking and graphic user interface. 3. To establish hardware and software in order to embody integrated management of computer based system through structured walk through. 4. To choose effective database management system and to achieve Log as record unit.
