1.Antibiotic Sensitivity of Pathogenic Bacteria of Osteomyelitis: Trend of recent 5 years, especially on Staphylococcus aureus

168 patients with osteomyelitis who were admitted and operated in Department of Orthopedic Surgery, Seoul National University Hospital between January in 1972 and December in 1976 ware reviewed and analized. The following results were obtained. 1. 138 strains of bacteria were isolated from 167 pus culture in osteomyelitis, about 82.6% 2. Of these, 112 strains are staphillococcus aureus. (81.2%). 3. Degrees of Sensitivity to antibiotics were as follows: in sensitive order, cephalothin was 99.0%, gentamycin 98.0%, cloxacilnli 94.5%, lincocin 92.0%, kanamycin 86.3%, streptomycin 81.2%, and chloramphenicol 80.6% was resulted, 4. Resistant rate to antibiotics were as follows: in resistant order, penicillin 88.8%, oxytetracycline 79.6%, tetracycline 76.2%, and ampicillin 72.2% was resulted.

3.Cystic Lesions in Os Calcis

The authors experienced six patients who were admitted in Department of Orthopedic Surgery at Seoul National University Hospital during the period of March 1966 to February 1976 with cystic lesions in calcaneus by radiographic findings. The following results were obtained. Among the six patients, four were osteomyelitis, one was eosinophilic granuloma and the other was fibromyxoid sarcoma. They were all similar in radiographic findings, but were different each other in pathologic findings. 2. Diagnosis is very difficult only with the patients history, hematologic and radiographic findings and the pathologic findings is the most important in the final confirmative diagnosis from benign cystic lesions to malignant one. 3. We describe about the kinds and points of the differential diagnosis. of the other discases from the reported six cases.

4.Marfan Syndrome: A Case Report

Marfan Syndrome is a rare congenital disease characterized by the abnormalities of Sheletal system, eye and cardio-vascular system which was initially desiribed by Marfan in 1896. One case of marfan syndrome was treated at the department of Orthopedic Surgery, College of Medicine, Seoul National University for multiple skeletal deformities. Moderate degree of scoliosis was treated with Milwaukee brace and finger deformities with Z-plasty and full thickness skin graft.

5.Surgical treatment of the Spine Tuberculosis

Two hundreds cases of spine tuberculosis were treated surgically for the period of 7 years between 1970 and 1976 at Seoul National University Hospital and following results were obtained. 1. Many patients failed to be treated with conservative method. Spontaneous fusion was found in 10% of 200 cases with long duration of disease. Anterior fusion was performed in 166 cases and posterior fusion was carried out in 34 cases. In those 34 cases, anterior currettage was combined in 7 cases, anterior fusion combined in 19 cases in which 7 cases were performed anterior and posterior fusion at one stage. Posterior fusion alone was carried out only in 8 cases. 2. Anterior fusion is indicated for: a. decompression in cord involvement b. confirmation of diagnosis c. involvement of one joint space in children d. mild to moderate kyphosis in adult 3. Anterior and posterior fusion is indicated in: a. severe kyphosis for solid funsion b. involvement of more than two joint spaces in children to prevent increasing kyphosis c. one stage operation was technically possible and has many advantages 4. Posterior fusion is indicated in: a. poor general condition b. technically difficult cases.

6.Surgical Treatment of Tennis Elbow

Five cases of tennis elbow seen during the time period from Jaunary 1983 to January 1985 had operative treatment. Of the five clinical cases, four elbows in three patients were treated with Nirschl and Pettrone operation. This operative technique included exposure of the extensor carpi radialis brevis, excision of the identified lesion, and repair. The following results were obtained. 1. Correct analysis of the patient's emotional stability was essential prior to undertaking surgical procedure for tennis elbow. 2. The results of Nirschl and Pettrone operation were graded as excellent in three elbows and fair in one.

7.Radiological Study of Spondylolysis in Korean Laborer

The spondylolysis is a defective ossification in the pars interarticularis of the neural arches without displacement of the vertebral body. The authors analysed 810 lumbar spines of adult Korean laborers on the basis examined to determine the anatomic characteristics of spondylolysis. The following results were obtained. 1. The overall incidence in laborer was 6.8%. 2. The most frequent site of the involvement was the fifth lumbar vertebra. of radiologic study and 3. Spina bifida in spodylolysis was approximately 1.7 times as frequent as is normally expected. 4. Lumbarization in spondylolysis was approximately 5.8 times as frequent as is normally expected. 5. Sacralization was not found in 55 cases of spondylolysis.

8.Immediate Internal Fixation of Open Fractures of the Long Bones

Between May, 1984 and October, 1986, twenty patients were treated at Han-Ra Hospital of JejuCity and Chung-Ang University Hospital of Seoul with immediate internal fixation with or without bone graft of open fractures of the long bones within three days. Fractured bones were ten tibiae, six femurs, three humeri and one radius and ulna. The injuries were classified on the basis of extent of soft tissue injury as defined by Gustilo and Anderson and consisted of four TypeI injuries, five Type II injuries and eleven Type III injuries. All wounds were closed primarily and radiological follow up was qhecked every month till union occurred. The complications included two deep infections, one non-union and one amputation. This study demonstrates that immediate internal fixation with bone graft when necessary is a beneficial method of treatment of open fractures of the long bones if copious irrigation, adequate debridement and skillful internal fixation technique are given golden period after injury. The results were related to the severity of the initial soft tissue injury and surgical technique and the period of union was not related to the individual bone. Autogenous bone grafting at the time of immediate internal fixation in comminuted or segmental fractures in which interfragmental compression cannot be obtained is recommended.

9.Bone Density and Mechanical Property after Locked Intramedullary Nailing of the Long Bone Fractures: Preliminary Study of QCT & Lunar of 15 Long Bone Fractures.

The decrease of bone density after fracture is a well-known phenomenon. Several studies of the changes of bone density after fracture demonstrated bone density decreased not only at the fracture site, b ut also along the whole fractured bone. Being treated with locked intramedullary (IM) nailing, 15 patients with the long bone fractures of lower limbs were examined by Quantitative Computed Tomography (QCT) or Dual-Energy X-ray Bone Densitometer (Lunar DPX-L) a few days after nail removal. In the cases of QCT (5 patients of the tibia shaft fracture), cortical bone density, cortical thickness and geometrical shape (i.e. external antero-posterior and medio-lateral diameters) of the fracture site were compared to those of the contralateral side. On the other hand, in the case of Lunar' (5 patients of the tibia shaft fracture & 5 patients of the femur shaft fracture), only cortical bone density was compared to that of the contralateral side. The results of this study were as follows: 1. When treated by the locked intramedullary nailing of the long bone fractures, the bone density of the tibia & femur shaft fractures measured from the fracture site, proximal and distal sites to mid-fracture decreased. 2. There was a difference in the bone density of the uninjured long bone diaphysis of the tibia & femur shaft fractures. That is, in the case of the tibia shaft fracture, the bone density of the ipsilateral femur diaphysis measured by QCT increased 2 %, and increased 7 % when measured by Lunar. But, in the case of the femur shaft fracture, the bone density of the ipsilateral tibia diaphysis measured by Lunar decreased 4%. 3. In the case of the tibia shaft fracture, the cortical thickness in the fracture site measured by QCT increased 72 %, and external antero-posterior and medio-lateral diameters increased 7 % and 63 %. It seemed to be the formation of peripheral callus due to medullary reaming preceding intramedullary nail insertion. Therefore, this clinical study demonstrated that the mechanical property of the long bone fractures was restored and restricted weight bearing hardly needs to recommended when the nail was removed after the fracture union. 4. The bone densities of the tibia shaft fratures measured by QCT were not different with those measured by Lunar.

10.Influence of Type of Screw on the Bacterial Adherence and Infection.

The purpose of the study was to evaluate the effect of commonly used orthopaedic material, stainless steel (SS) screw (2.7xlOmm, Synthes) and titanium (Ti) screw (2.7x10mm, Synthes), on bacterial adherence and infection. The bacteria used for this study was Staphylococcus epidermidis for its ability of adherence. For in vitro assay, screws of each material were placed into test tube containing 107 colony forming unit (CFU) of S. epidermidis in trypticase soy broth for 18 hours. Following incubation, each screws were treated either with trypsin or with an ultrasonic cleaner. For in vivo assay, we inoculated 5x107 CFU of S. epidermidis into the proximal tibial metaphysis of the rabbits, and then inserted Ti screw on the right side and SS screw on the left side. The amount of in vitro adherence of S. epidermidis was greater for SS screw than for Ti screw. Especially following trypsin treatment, SS groups yielded significantly higher bacterial counts than Ti group, respectively (1.4x105 CFU vs 2.8x104 CFU, p<0.05). We found a significantly higher bacterial count after use of SS screw than after Ti screw in rabbits in vivo assay also. Trypsin treatment for identifying biofilm embedded bacteria from implanted metal may be useful in the diagnosis of infection. Ti screw appears to have biocompatibility to inhibit bacterial adherence compared with SS screw in the rabbit tibial model.
