3.Operative Treatment of Winged Scapula: A Report of 2 Cases.

Winging of the scapula is characterized clinically by a prominence of the vertebral border and inferior angle of the scapula. Trauma, neuritis, neural injury during surgical procedure and many other causes may develop winging of scapula. When pain and functional impairment persist, surgical intervention may become appropriate. We report two cases of winged scapula. One case was serratus anterior palsy due to blunt trauma, which was treated by transfer of teres major and pectoralis minor elongated with a tensor fascia strip. Another case was trapezius muscle palsy due to iatrogenic spinal accessary nerve injury, which was treated by transfer of the levator scapulae, rhomboid major and rhomboid minor muscle. In the former case, there was no weakness nor deformity of affected shoulder. In the later case, there was some residual pain and limitation of motion.

5.The Effects of the Flexible and Rigid External Fixation on the Fracture Healing: An Experimental Study in Rabbits

In order to compare the effects of the flexible external fixations with the rigid one on fracture healing, rabbits tibiae were fractured surgically and each fracture were immobilized with external devices using graphites (the group I), plastics (the group II) and stainless steel (the group III). At six weeks after fracture, clinical and radiological union rates in group I, II and III were 91% 73% and 60% respectively. The rabbits tibiae were load in 4-point bending test to determine the mechanical properties of the fracture healing. The means and standard errors of maximum bend moment of the fractured tibiae were 11. 30±2.33kg.f. cm., 8.55±2.02kg.f. cm. and 9.79±2.09kg.f. cm. in group I, II and III respectively. These data may suggest that a certain amount of the flexibility (ex, graphite) or motion is better than the rigidity (ex, stainless steel) in fracture healing by the method of the external immobilization).

6.A Clinical Study on the Injuries of the Meniscus

Fifty-two patients of meniscal injury were studied in the Department of Orthopedic Surgery, Seoul National University Hospital for 5 years and 6 months from January, 1973 to June, 1978. The results were as follows: 1. Male was more affected than female (M:F = 1.4:1). And the injury of meniscus was most frequently occurred in the age group between 11 and 30 years old (65.2%). 2. Thirteen patients denied the history of trauma (25.2%). 3. Over-all positive rate of Memurray sign was 85% and arthrography was 74%. 4. Lateral meniscus was more frequently affected than medial meniscus. The most frequent type of lesion was peripheral longitudinal tear (11 cases). Discoid meniscus was found in 14 patients. 5. In follow-up study, satisfactory results were obtained in 84.3%.

7.A Case of Diencephalic Syndrome.

Diencephalic syndrome of infancy which is characterized by progressive and profound emaciation often with normal or accelerated skeletal growth, hyperkinesias, hypotension, and hypoglycemia. The profound emaciation so characteristic this syndrome has yet to be explained. The diencephalic syndrome is caused by low grade astrocytoma, ependymoma in the third ventricle including the hypothalamus. Authers report a case of diencephalic syndrome which was confirmed in this patient with ependymoma in the region of anterior hypothalamus by transcallosal approach.

8.Two Cases of Infantile Hydranencephaly.

The term hydranencephaly designates the condition in which large protions of the cerebral hemispheres have been reduced to membranous sacs within a cranium of relatively normal size. Only basal ganglia and remnants of the mesencephalon are found at the base of the skull. We have experienced the 2 cases of infantile hydranencephaly. In surveying the pathogenesis of the hydranencephaly, it becomes apparent that the distribution of most of the lesions falls into certain distinct patterns, which is characterized by almost completely absent of both cerebral hemispheres and by relative gross preservation of the basal ganglia, a portion of lower temporal and occipital and contents of the posterior fossa in our experienced 2 cases. In our cases, the condition may present a defect in vascular ontogenesis or the outcome of vascular occlusion of both internal carotid arteries and their main branches on carotid angiogram and CT scan. This paper presents the essential features for the clinical diagnosis, suggesting pathogenesis in 2 cases of infantile hydranencephaly.

9.Pleomorphic Xanthoastrocytoma: A Case Report.

Pleomorphic xanthoastrocytoma is histologically characterized by marked cellular pleomorphism of lipid-laden neoplastic astrocytes and bizarre giant cells showing mitotic figures and high cellularity. Inspite of its ominous-looking microscopic features, howerver, the prognosis is usually favorable. This tumor develops mainly in the supratentorial area of young people and frequently involves the leptomeninges. We experienced a case of pleomorphic xanthoastrocytoma in 18 year-old-male. In addition to the cellular pleomophism, the prominent reticulin fibers surround the individual tumor cells or the tumor cells nests. Immunohistochemical staining and electron microscopy revealed glial fibrillary acidic protein(GFAP) expression and pericytoplasmic basal lamina in the tumor cells.
10.Influence of Type of Screw on the Bacterial Adherence and Infection.

The purpose of the study was to evaluate the effect of commonly used orthopaedic material, stainless steel (SS) screw (2.7xlOmm, Synthes) and titanium (Ti) screw (2.7x10mm, Synthes), on bacterial adherence and infection. The bacteria used for this study was Staphylococcus epidermidis for its ability of adherence. For in vitro assay, screws of each material were placed into test tube containing 107 colony forming unit (CFU) of S. epidermidis in trypticase soy broth for 18 hours. Following incubation, each screws were treated either with trypsin or with an ultrasonic cleaner. For in vivo assay, we inoculated 5x107 CFU of S. epidermidis into the proximal tibial metaphysis of the rabbits, and then inserted Ti screw on the right side and SS screw on the left side. The amount of in vitro adherence of S. epidermidis was greater for SS screw than for Ti screw. Especially following trypsin treatment, SS groups yielded significantly higher bacterial counts than Ti group, respectively (1.4x105 CFU vs 2.8x104 CFU, p<0.05). We found a significantly higher bacterial count after use of SS screw than after Ti screw in rabbits in vivo assay also. Trypsin treatment for identifying biofilm embedded bacteria from implanted metal may be useful in the diagnosis of infection. Ti screw appears to have biocompatibility to inhibit bacterial adherence compared with SS screw in the rabbit tibial model.
