2.Experimental autoimmune encephalomyelitis in cynomolgus monkeys.

Experimental autoimmune encephalomyelitis was induced in macaques. T cell clones infiltrated into the brain lesion area were compared with those in blood. Intradermal immunization of macaques with brain white matter derived from healthy macaque in combination with pertussis toxin, induced neurological symptoms in two macaques. One died on day 25 after immunization, whereas the other survived. Gross examination of the brain from the dead macaque, showed clear hemorrhagic lesions in the white matter. Hematological analysis showed that drastic T cell response was induced in macaques immunized with white matter, but not in control macaques. Flow cytometric analysis of blood cells from the affected macaques demonstrated an increase of CD4 and CD8 T cell populations expressing the CD69 early activation marker. Single strand conformation polymorphism (SSCP) analysis of T cell receptor beta chain showed T cell clones infiltrated into the brain lesion, which were different from those found in the peripheral blood of the same monkey. The present paper shows that SSCP analysis of TCR is useful in studying clonality of T cells infiltrating into the brain tissue of macaque with EAE.

3.Influence of Ginseng Powder and Ginseng Saponin on the Growth of Candida albicans in vitro.

Panax ginseng C.A. Meyer (Araliacea) has been known for thousands of years in the Far East as a remedy effective against a multitude of ailments. Pharmacological investigations have shown that the basic effect in ginseng action is its capacity to increase nonspecific resistance of the organism to various untoward influences. Ginseng saponin has been considered the active principle for ginsengs most therapeutic properties. Recently, the use of cream and soap with ginseng extract has been gradually increasing. The purpose of this study is to evaluate the efficacy of Panax ginseng to Candida albicans in vitro. During this experiment ginseng saponin and powder of Panax ginseng root were diluted serially in Sabouraud's dextrose agar, and candidial suspensions of isolated strains from candidial intertrigo and female genitalia were inoculated into each medium containing different concentrations of test materials. Then their growth was observed for weeks at 25'C. After that we measured the size of colonies grown in various concentrations and eompared them with those of the Sabourauds dextrose media. For the complementary study Candida albicans strain isolated from vagina were plated on both Sabouraud's dextrose media and 80 mg/cc ginseng root powder. Microscopic examination, germ tube test and sugar fermentation test were then performed to compare the production of budding cells, the growth of pseudo-hyphae and the characteristics of the yeast on each media. Results from this study show that ginseng saponin and roots of Panax ginseng have no significant effect on growth and multiplication and the characteristics of Candida albicans in vitro.

4.A Case of Verrucous Carcinoma Developed on the Retroauricular Area.

Verrucous carcinoma, a distinct variant of well differentiated squamous cell carcinoma, was originally described as an entity by Ackerman. This histologically benign lesion features relentless local growth and little tendency to metastasize. The commonest location of verrucous carcinoma is the oral cavity. Less frequently, the tumor originates in the squarnous mucosa, skin of the genitalia(glans penis, vulava, vagina, scroturn and perineum), nasal fossa, the larynx or the sole. Now we report a case of verrucous carcinoma developed on the retroauricular area in a 49-year-old man. He was treated by surgical excision followed by skin graft. This appears to be the first reported case arising on the retroauricular area.

6.Comparison of Automated Lamellar Keratoplasty with Laser Assisted In Situ Keratomileusis in Correction of High Myopia Over -11.00D.

In order to compare the clinical results of Automated Lamellar Keratoplasty with Laser Assisted In Situ Keratomileusis for corection of high myopia, the effects of the procedure on 9 patients(10eyes) following ALK and 14 patients(15 eyes) following LASIK who were followed up at least 6 months were analyzed retrospectively. Mean preoperative spherical equivalent(S.E) was -17.32D(-11.50D~-22.50D) in ALK group and -16.05D(-12.00D~-24.00D) in LASIK group, while mean postoperative S.E was -3.19D at 1 week, -3.50D at 1 month, -4.12D at 3 months, -3.83D at 6 months in ALK group and +2.10D at 1 week, 0.70D at 1 months, -0.10D at 3 months, -0.58D at 6 months in LASIK group. Mean preoperative uncorrected visual acuity was 0.03 in ALK group and 0.04 in LASIK group. At postoperative 6 months, mean uncorrected visual acuity was improved to 0.18 in ALK group and 0.52 in LASIK group. The percentages of patients achieving correction within +/-1.00D at 6 months were 20% in ALK group and 47% in LASIK group. Postoperative cylindrical change was not significant in each group(p>0.05) and also best corrected visual acuity did not show statiscally significant changes in each group(p>0.05). The complications included folds of corneal flap, increased IOP, night halo, and corneal haze. In conclusion, ALK, when performed for high myopia with Ruiz normogram, resulted in undercorrection. LASIK seems to be much more predictable and accurate than ALK in high myopia.

7.Nutritional Status of the Elderly Living in Cheongju - II. Anthropometric, Biochdemical and Clinical Assessment.

To assess the nutritional and health status of the elderly, anthropometric measurements and blood test for analyzing biochemical indices were carried out from August to September in 1996. Data on the incidence of a specific disease and clinical symptoms were also collected by interviews with a questionnaire. Among the total of 169 free-living elderly interviewed(91 men, 78 women), 86 elderly(57 men, 29 women) participated in the anthropometric measurements, and 73 elderly(52 men, 21 women) in blood tests. Average heights and weights were much lower than the standards established in the Korean Recommended Dietary Allowances. While average triceps skinfold thickness of men and women were above the 50 percentile of the reference data, mid-arm muscle circumferences were lower than the 50 percentile. Prevalences of anemia, assessed by hemoglobin using WHO definition, were 38.5% for men and 33.3% for women. Blood levels of total protein and albumin were above the normal limit for all participants. Mean blood cholesterol levels of men and women were 163.9mg/dl and 185.8mg/dl, and triglyceride levels were 138.2mg/dl and 161.9mg/dl, respectively. Women older than 75 years generally had high levels of BMI, total cholesterol and triglyceride. Health status assessed by clinical symptoms generally showed no significant difference by age, and their smoking, and drinking habits. Males and the elderly who exercised regularly had better health status. It was suggested that exercise was a good way to improve health status.

9.Diabetes Mellitus in Orthopaedic

Diabetes mellitus is one of troublesome disorder for the orthopaedic patient who should be operated on some other problems. Internist will advice and take care of them as far as diabetic problem is concern. However, orthopaedic surgeon should fully understand and follow every details about the condition of his own patient. Gamerin-Davalos hadclassified the diabetes mellitus into four types of potentiate, chemical and clinical diabetes. Potentiate form is not detectable at present method. The chemical and latent chemical forms are subclinical types and will be detected by the special examinations. In the orthopaedic practice, surgeon should keep in mind and perform proper tests for any suspecious symptomes or histories related to diabetes mellitus. Administration of insulin for the diabetes should be full doses but nerve be over-dosed. Neuropathy, vascular changes and infections are the common complications to diabetic patient and closely influence to the prognosis of major surgery. Ellenbery et al. had emphasized that the neuropathy is not a complication but rather a concomitant feature of diabetes mellitus. It may appear nerve root symptome to confuse with the herniated intervertebral disc. Vascualr change of the thickening of capillary basement membrane will causes gangrene of extremities and unexpected massive bleeding during surgery. Susceptibility of infection on diabetic patient is well known fact and in case of chronic osteomyelitis or tuberculosis of bone persist for long time without specific reason, surgeon should check once for the posibility of diabetes mellitus. Administration of any general anesthetic agents will elevate blood sugar level especially the ether, however, fluothane is a selective one at present. In case of surgery on diabetic patient, aceton uria and CO2 combining power should be checked closely for four to five days post-operatively. Blood sugar level should be kept slightly elevated during and post-operative period for diabetic patient. Surgeon should check his patient for any complications in case of increased dose of insulin is required post-operatively.

10.The Factors Affecting the Constipation of the Elderly at Nursing Homes.

PURPOSE: The purpose of this study is to identify factors affecting the constipation of the elderly at nursing homes. METHODS: A total of 184 elders at 7 nursing homes in P City participated. Information about constipation status (Constipation Assessment Scale), functional status (Katz' Activities of Daily Living), nutritional status (Mini-Nutritional Assessment), and depressive symptoms (Geriatric Depression Scale Short Form Korea version) were collected through interviews. Descriptive statistics, Chi-square test, and t-test were used to describe and compare the non constipation group and the constipation group. Multiple logistic regression analysis was used to determine factors affecting constipation status. RESULTS: The prevalence of constipation was 47.5% (Men 37%, women 51%). Elders suffering from a disease (P=.021) and having low sleep satisfaction (P=.000) were more likely to be constipated than those who did not. The factors affecting the constipation of the elderly at nursing homes were gender (OR=3.558), sleep satisfaction (OR=.580), and depressive symptoms (OR=1.132). CONCLUSION: According to the result, to reduce the constipation rate of old women at nursing homes, we have to develop guidelines that can solve problems related to constipation cause by sleep-satisfaction and depressive symptoms.
