3.Two Modified T2 Sympathicotomies in Palmar Hyperhidrosis.

BACKGROUND: Thoracoscopic T2 sympathicotomy had been performed as a simple and effective method in treating palmar hyperhidrosis, however, this method had the complications of compensatory hyperhidrosis and facial anhidrosis. Therefore, a more limited and modified methods for T2 sympathicotomies were introduced and comparative analysis of the modified upper and lower T2 sympathicotomy were made in the treatment of palmar hyperhidrosis. MATERIAL AND METHOD: From January 1997 to December 1998, 41 patients with palmar hyperhidrosis had been treated by modified T2 sympathicotomy at the Kon-Kuk University Seoul Hospital. Twenty-four patients underwent a modified upper T2 sympathicotomy(Group A), and seventeen patients underwent a modified lower sympathicotomy(Group B). A comparison between groups A and B were made according to the medical records and interview results. RESULT: All patients showed symptomatic improvements after the operation. The anisocoria, facial anhidrosis and dissatisfaction for compensatory hyperhidrosis were more common in Group A and the individual satisfaction for the operations were higher in Group B. CONCLUSION: The modified lower T2 sympathicotomy might be a more effective and less complicated method than the modified upper T2 sympathicotomy.

Intrathoracic meningocele is a protrusion of the spinal meninges through an intervertebral foramen into thethoracic cavity. The majority occur in association with neurofibromatosis. The authors experienced a case ofintrathoracic meningocele in 26 years old male patient associated with neurofibromatosis, and present conventional CT and CT metrizamide myelographic findings.

6.Pulmonary leiomyoma: a case report.

A 56 year old female patient was found to have a lung mass in the right upper lobe lobe during evaluation for cough and sputum. The mass had grown slightly over the past 6 months when she was admitted for an operation. Preoperative studies revealed the benign nature of the tumor, and a thoracoscopic right upper lobectomy was performed. Postoperatively, the diagnosis was histologically proven to be pulmonary leiomyoma which is a rare type of benign lung tumor.

7.Mondor's Disease in Antecubital Area.

Mondor's disease is also called thrombophlebitis and it is not a common condition. The characteristics finding of Mondor's disease is a subcutaneous cord that is tender and tense. This disease commonly occurs in the breast and abdomen. The etiology of Mondor's disease is unknown, but the generally agreed on causes are trauma, excessive exercise and breast surgery. On rare occasions, this disease is related to malignancy, pregnancy, filariasis and so on. This disease is self limiting and it is usually treated conservatively and symptomatically. We treated one woman who had Mondor's disease on the anterior side of the left elbow. This is a very rare location for Mondor's disease and so clinicians need to be aware about the possibility of this unusual presentation for this disease.

8.Management patterns of upper gastrointestinal symptoms.

BACKGROUND: Upper gastrointestinal symptoms is the one of the most frequent symptom in primary medicine, those manifestations are various and can not find the cause in many cases. In Korea the management patterns for upper gastrointestinal symptoms are using drug store, oriental medicine, folk methods, diet therapy in addition to medical service. We have investigated the behavioral patterns for the control of upper gastrointestinal symptoms. METHODS: The authors surveyed the patients who had visited Dong guk university Kyung ju hospital with upper gastrointestinal symptoms from April 1 to June 30 in 2000. RESULTS: 269 questionnaires were collected and among them 247 completed ones were analysed. 112 subjects were used medical service only(45.3%), 135 subjects were used alternative methods also(54.7%). Among the 135 subjects who used alternative methods, 60 visited to drug store(44.4%), 36 took oriental medicine(26.7%), 18 used folk herbal remedy(26.7%), 5 used diet therapy(3.7%), 16 used folk manual therapy(11.9%), most commonly. Women more commonly used alternative methods(P<0.01). By occupation, housewives more commonly used alternative methods and specialists less commonly used alternative methods. There were no significant difference between two groups in other general characteristics, most severe symptom, cost, satisfaction. CONCLUSION: We find the behavioral patterns for relieving upper gastrointestinal symptoms are using alternative methods(drug store, oriental medicine, folk herbal remedy, folk manual therapy, diet therapy) in addition to using medical service. Women and housewives more commonly used alternative methods, and specialist less commonly used alternative methods.

9.An Anatomical Study of the Superior and Inferior Gluteal Arteries Supplying Gluteus Maximus for Musculocutaneous Flaps.

The tissues of gluteal region including skin and underlying gluteus maximus muscle are used for reconstructions of head and neck deformities caused by trauma and lumbosacral defects caused by bed sores, and for reconstruction of breast. Moreover, gluteus maximus flaps were used for reconstruction of anal -and vaginal -sphincter dysfunctions after radical resection for treatment of cancer in anorectal or vaginal region. Because the knowledge on the precise course and branching patterns of the arteries supplying the gluteus maximus muscle enables the prediction of the safety of surgery, the perforating branches supplying skin and subcutaneous tissues which exited from the gluteus maximus muscle, the positions of the superior and inferior gluteal arteries exited from pelvic cavity under the gluteus maximus muscle, and the course of these arteries under surface of the muscle were investigated. Total ninety -one dissected gluteus maximus from 47 Korean cadavers (44 bilateral specimens and 3 unilateral specimens) were observed. The perforating branches exited from the gluteus maximus were divided into upper and lower parts. The superior gluteal artery supplied upper two fifth of gluteal region and the inferior gluteal artery supplied the rest of gluteal region. The positions of superior and inferior gluteal arteries exited from pelvic cavity were in 1 cm medial to upper one third point on connecting line from posterior superior iliac spine to greater trochanter of femur, and middle point on connecting line from posterior superior iliac spine to ischial spine, respectively. The courses of the superior and the inferior gluteal arteries were classified into four types by distribution patterns. The most common incidence (46.5%) was observed in the typical type (Type I) that the superior and inferior gluteal artery supplied the upper or lower part of gluteus maximus muscle, respectively. The incidence of type II that some branches of inferior gluteal artery run up to the area supplied by superior gluteal artery was 16.3%. In contrast to type II, the incidence of type III that some branches of superior gluteal artery run down to the area supplied by inferior gluteal artery was 18.6%. The incidence of Type IV that only superior gluteal artery supplied the muscle was 18.6%.

10.A Case of Primary Pulmonary Leiomyosarcoma.

Primary Pulmonary leiomyosarcomas are rare tumors. These tumors may arise at any level of the lung in which smooth muscle fibers are found. The highest incidence of sarcoma is during the fourth decade whereas for bronchogenic carcinoma the maximum incidence is during the sixth and seventh decades. Leiomyosarcomas are aggressive and progressive. Treatment is mainly surgical. The chemotherapy is ineffective and the effectiveness of radiotherapy depends on the total dose of irradiation. Prognosis and significant survival rate are related to the size of the lesion. We report one patient with primary pulmonary leiomyosarcoma involving the right lower lung.
