2.The Geographic Patterns of The Adult Male Bladder Cancer Patients in Korea: Clue as to the Occupational Exposure.

We calculated the age specific Proportionate Morbidity Ratio(PMR)' for different areas as the outcome index by using the national cancer registry data(1988 1991) to assess the patterns of stomach cancer and bladder cancer. Also, we calculated 'Manufacturing workers Concentration Index(MCI)' for different areas as the exposure index by using the census data reported in 1980 to assess the relationship between cancer site specific PMRs and MCIs. As a result, we found the significant correlation(r=0.61, p=0.002) between the bladder cancer in the age of 40s and the industrial exposure indicator. Future analytic studies should be fruitful in identifying more occupational risk factors for bladder cancer. Investigation of cancer incidence including geographic variations and difference by age may identify patterns suggesting occupational exposures.

3.Modulation of lipid metabolism by mixtures of protamine and chitooligosaccharide through pancreatic lipase inhibitory activity in a rat model.

Overweight and obesity are usually related with high fat and calorie intake, and seriously causative of lifestyle-related diseases such as cardiovascular disorders, arteriosclerosis, and colon cancer. In this study, we propose a novel dietary therapy against overweight and obesity using mixtures of protamine and chitooligosaccharide (COS), which are known to interrupt the lipid metabolism in the body. Protamine is a dietary protein originated from salmon reproductive organ, and COS is an oligosaccharide made from chitin or chitosan by chemical or enzymatic hydrolysis. In the enzyme activity analysis in vitro, protamine and COS strongly suppressed the activity of pancreatic lipase, which is the primary enzyme for the digestion and absorption of lipids in the intestine. In in vivo animal test, the mixtures of protamine and COS significantly reduced the serum levels of triglyceride (TG), total cholesterol (T-CHO), and low density lipoprotein-cholesterol (LDLC) and inhibited the accumulation of lipids in liver tissue of Sprague Dawley (SD) rats fed high fat diets. On the other hand, they increased fecal TG and T-CHO contents. From these alterations in lipid metabolism, we verified that protamine and COS mixtures could effectively interrupt the digestion and absorption of dietary lipids in the body by inhibiting pancreatic lipase activity. In addition, protamine and COS mixtures increased the serum level of high density lipoprotein-cholesterol (HDLC), responsible for removing cholesterol from cells and protecting atherosclerosis, and therefore decreased the potential risks of cardiovascular diseases by lowering values of the atherogenic index (AI) and cardiac risk factor (CRF). Taken together, we suggest protamine and COS mixtures as a prominent dietary therapy for the prevention of overweight, obesity, and further cardiovascular diseases related with hyperlipidemia.

4.Modified Brostrom Operation for Revision Lateral Ankle Ligament Reconstruction.

PURPOSE: We assessed the clinical results of modified Brostrom procedure as a revision method after failure of a primary reconstruction. MATERIALS AND METHODS: This is a retrospective study of seven patients treated with Modified Brostrom procedure after failed lateral ankle ligament reconstruction between 1996 and 2002. Instability symptom developed average 4.7 month after the initial reconstruction surgery at other clinics. All patients had significant functional impairment before surgery and not responded to conservative protocols. Modified Brostrom procedure was applied to all patients. RESULTS: The average follow up was 51 months (18 to 84). Seven of eight patients had clinical stability following revision reconstruction, six patients (75%) returned to their previous functional level. American Orthopaedic Foot and Ankle Society ankle-hindfoot scores averaged 87.5. There is no difference in active or passive range of motion of plantar flexion or dorsiflexion when compared to the contralateral ankle. However, three patients were noted to have lost some degree of inversion when compated to contralateral ankle. Two patients had osteochondral lesion and multiple spurs and had pain around the ankle that prevented their full recovery. One patient complained of persistent pain which was considered complex regional pain syndrome. CONCLUSION: Though the outcome of the Modified Brostrom procedure as a method of revision surgery was less satisfactory compared to the results of primary ankle reconstruction, it would be an appropriate option when concomitant abnormalities were not accompanying.

5.Mechanical properties of laser welded Co-Cr alloy.

STATEMENT OF PROBLEM: The joints of removable partial denture alloys have failed frequently after routine usage. PURPOSE: The purpose of this study was to evaluate the mechanical properties of the laser welded Co-Cr alloys. MATERIAL AND METHOD: For this study, 20 Co-Cr specimens were casted and 10 of them were seperated on the middle area and laser welded with Alpha laser welding machine(Siro Lasertec, Pforzheim, Germany). Rest of them which were as cast, were used as a control group. For the section of the experimental specimens, wire cutting machine was used to make a even gap of the all specimens. Laser welding was done with manufacturer's instrunction and tested each specimen by Instron Machine. Tensile strength, 0.2% yield strength and % elongations were recorded. Fractured surfaces were investigated with SEM. RESULTS: The results were as follows : 1. The tensile strength of the laser welded group(617.7MPa) was about 75% of the as cast group(820.4MPa). It had stastically singnificant diffrences(p<0.05). 2. The % elongation of the experimental group was 6.6 which was lower than the control group(14.3). 3. Fracture of the experimental group occured in the welded surface and showed many voids. In contrast, the fracture surface of the control group was showed rough surfaces without any voids. CONCLUSION: The tensile strengths of the as-cast joints were higher than those for the laserwelded joints, and the % elongation of the experimental group was lower than the control group. Porosity was found in laser-welded joints.
6.Shiftwork and Sickness Absence in Korean Manufacturing Industries.

In order to provide necessary information for better health of workers through understanding the actual status of the industries adopting shift systems. The data were gathered from a stratified random sample of 347(0.5%) firms selected out of about 70,000 manufacturing industries throughout the nation in 1993 stratification during sampling was by industrial group and number of workers. The major findings obtained from 288 firms surveyed completely were as follows: 1. About 20.2% of the 288 firms were adopting shift systems and shiftworkers accounted for about 25.1% of the total work force in 288 firms. 2. The bigger number of workers was, the higher the adopting rate of shift system was. 3. The rate of having welfare facilities such as dining room, commuting bus, washing facilities, and health care room etc. was higher in the industries adopting shift systems. 4. The major industrial groups adopting shift systems were the rubber & plastic goods manufacturing industry (54.1 per 100 firms) and the textile manufacturing industry (44.8 per 100 firms). However the proportion of shiftworkers was higher in the textile manufacturing industry (70.5 per 100 firms) and the electronics industry (57.9 per 100 firms). 5. The most predominant work schedule was the weekly rotating, semi-continuos 2-crew 2-shift system (47.5%). 6. In the industries adopting shift systems, shiftworkefs had an adjusted average of 0.29 spells per 100 workers (O.14-0.45 in 95% C.I) compared to 0.23 spells per 100 nonshift dayworkers (0.15-0.31 in 95% C.I) for 1 month. 7. Also in the industries adopting shift systems, the adjusted average annual turn-over rate of shiftworkers was 13.07 per 100 workers (12.03-14.12 in 95% C.I) compared to 10.18 per 100 nonshift dayworkers(9.53-10.82 in 95% C.I).

7.Evaluation of the Arachnoid Cyst Treatment.

Intracranial arachnoid cyst are becoming a more frequent finding as a result of the increased use of cranial computed tomography. But the best treatment of arachnoid cysts remains the subject of controversy. We report 46 cases of patients with arachnoid cyst. Thirty nine cases(84.7%) had supratentorial locations. The cases of 22 patients with mild symptoms and small cysts were treated with conservative management only. Surgical treatment was performed on 24 patients with severe or uncontrolled symptoms and large sized cysts. Surgery consisted of craniotomy and cyst excision with communication to basal cistern. Surgical results and long-term outcomes were good. However there was a one operative death due to acute subdural hematoma. It is, therefore conclued that surgical excision of arachnoid cyst with communication to basal cistern is one of the more effective methods of alternative treatment. And further studies of the natural history of conservative cases should be undertaken for a futher understanding.

The 4th and 5th metatarsocuboid joint arthritis causes moderate functional deficit during walking. It is difficult to obtain good results with conservative treatment. The arthrodesis of the affected joints can be a possible surgical option, but with a limited success. We report two cases of the 4th and 5th metatarsocuboid joint arthritis managed with anchovy tendon interposition arthroplaty.

9.Clinical Analysis and Surgical Treatment of Vertebral Aneurysm.

Aneurysms of the vertebral artery are relatively uncommon. Diagnois and treatment of such condition aneurysm has a potentially higher degree of technical difficulty. We retrospectively analyzed the clinical features and therapentic outcomes of 12 consecutive patients with vertebral aneurysms that we have encountered during the last 11 years. Most of these aneurysms occured in females (9 of 12). Eight patients presented as classic subarachnoid hemorrhage, three were diagnosed incidentally, and the other one patient had fusiform aneurysm presented with mass effect. Among the 12, there were 7 saccular aneurysms, 2 fusiform aneurysms, and 3 dissecting aneurysms. Eight of these were located at the origin of the posterior inferior cerebellar artery(PICA), two were at the junction of the vertebral and basilar arteries, and two were at the distal portion of PICA. Direct neck clipping was performed in 8 patients and proximal ligation of the vertebral artery was done in 4 patients due to the impossibility of direct neck clipping. Eleven of them experienced no major morbidity. However, one patient with fusiform aneurysm treated by vertebral artery clipping developed right cerebellar hemispheric infarction. Unfortunately this patient died 10 months after surgery due to lung abscess related to Behcet's disease.

10.Metaplastic Carcinoma of the Breast with Chondroid Calcification: A Case Report.

Metaplastic carcinoma is a rare form of breast carcinoma in which a variety of metaplastic changes occur. These commonly involve squamous or spindle cells, but pure chondroid metaplasia is relatively uncommon. We report a case of metaplastic carcinoma of the breast which mainly involved chondroid metaplasia and in which chondroid calcifications were seen on mammograms.
