1.Expression of matrix metalloproteinase-1 and -2 mRNA in retrodiscal tissue of the temporomandibular joint.

Matrix metalloproteinases (MMPs) play an important role in the normal morphogenesis, maintenance, and repair of matrix and also have important functions in pathologic conditions characterized by excessive degradation of extracellular matrix, such as rheumatoid arthritis, osteoarthritis, periodontitis and in tumor invasion and metastasis. In this study, expression of MMP-1 and -2 mRNA in retrodiscal tissue of the temporomandibular joint (TMJ) was examined and compared with magnetic resonance imaging (MRI) and surgical findings. MMP mRNAs in the retrodiscal tissue samples were detected by reverse transcription - polymerase chain reaction. TMJ internal derangement (ID) was categorized as normal disc position, disc displacement with reduction, early stage of disc displacement without reduction (DDsR) and late stage of DDsR. TMJ osteoarthrosis (OA) was classified with normal, mild and advanced OA. The amount of synovial fluid collection was divided into not detected, small, large and extremely large amount on MR T2-weighted imaging. Perforation and adhesion were examined during open surgery of the TMJ. Six out of 37 samples were excluded because of little amount of extracted total mRNA. MMP-2 mRNA was detected whole joints, and so the MMP-2 mRNA seems to be expressed normally in retrodiscal tissue. However, MMP-1 mRNA was expressed in 8 of 31 joints. Frequencies of MMP-1 mRNA expression according to the TMJ IDs, amount of synovial fluid and surgical findings made no significant difference. MMP-1 mRNA was detected more frequently in OA groups (7/16 joints, 43.8%) than in normal bony structure group (1/15,joints, 6.7%). Expression of MMP-1 mRNA in retrodiscal tissue might be related with OA of the TMJ.

3.The effects of induced nasal obstruction on the craniofacial skeleton of the growing rats.

The purpose of this study was to investigate the effects of induced nasal obstruction on the craniofacial skeleton of the growing rats. Ninety five Sprague-Dawley rats of four weeks of age were divided into three groups of which the first group got the one naris occluded, the second group was on both nares occluded, and the third served the control. The experimental animals were sacrified 2, 5, 8, 12 weeks after occlusion of nasal airway by means of electrocauterization followed by preparation of dry skulls, measurements, and statistical analyses. The findings were as follows: 1. The body weight increments in two nasal obstruction groups were significantly lower than the control group. 2. The bilaterally occluded group showed the smallest size in the magnitide of craniofacial skeleton, then unilateral occluded group and control group in order. 3. The dimensional difference in craniofacial skeleton between experimental and control groups were the most outstanding at 5 weeks of experiment in length, and 8 weeks of experiment in width. 4. In spite of various significant dimensional differences among the groups, no distinct description upon the interrelation between the oral respiration and its impact on the craniofacial growth could be drawn in the very experimental design.

4.A Case of Subdural Hematoma due to Brandt-Daroff Habituation Excercise.

Subdural hematoma is a common disease of intracranial hemorrhage that accounts for 11-21% of all head trauma patients. It is defined as a hematoma that develops between dura mater and subarachnoid space. In elder patients, mild trauma may not always be accurately recognized and symptoms may not develop slowly due to the large area of subdural space. Headache is the most common symptom of subdural hematoma and may be associated with hemiplegia, dysarthria, sensation disorder, conscious disorder, and etc. We present, with a literature review, a case of a patient who developed subacute subdural hematoma after performing Brandt-Daroff habituation exercise to treat benign paroxysmal positional vertigo.

5.A case of Behcets Esophageal Ulcer.

A 38-year-old man, with recurrent oral ulcers for 10 years, was admitted because of recent aggravation of odynophagia and sore throat. About 4 years earlier, he had been performed abdominal surgery for intestinal perforation. Gastrofiberscopic examination showed small round ulcers at hypiopharynx and 6cm sized longitudinal linear ulcer at mid esophagus. Biopsy specimens at mid-esophagus showed chronic inflammation. Besides oral ulcer, he had perianal ulcers and skin rashes. He was managed with steroid, colchicine and sulfasalazine under the diagnosis of esophageal involvement in Behcet's disease. After 3 months from discharge, esophagogram and gastrofiberscopic examination showed some improved appearance, but symptoms recurred for steroid tapering. He has been followed in much improved status for 8 months after discharge.

6.Misdiagnosis of Spontaneous Intracranial Hypotension Presenting as Acute Mental Deterioration Caused by Unilateral Acute Subdural Hematoma: Case Report

Spontaneous intracranial hypotension (SIH) is usually a benign disease which mostly present as orthostatic headache and resolves by conservative treatment or epidural blood patch. However, in severe cases large subdural hematoma or brain caudal herniation can progress to brain herniation and neurologic complications. We introduce a rare case of SIH which presented as acute mental deterioration with unilateral acute subdural hematoma. A 60 years old female visited to emergency room for stuporous mental change and unilateral acute subdural hematoma. Decompressive craniectomy and hematoma removal was performed to release brain herniation and increased intracranial pressure. There was temporary improvement of consciousness, but sustained leakage of cerebrospinal fluid (CSF) and caudal brain herniation worsened patient's condition. After recognizing that CSF leakage and hypovolemia was the underlying disease, emergent epidural blood patch and early cranioplasty was performed. After treatment CSF volume was normalized and patient recovered completely without neurologic deficits. Acute mental deterioration with unilateral subdural hematoma is a rare presentation for SIH. Treatment strategy for subdural hematoma with concomitant SIH patients, should be planned carefully with concerns to CSF hypovolemia and intracranial pressure.
7.The effect of washing phosphoric acid etchant on shear bond strength of an orthodontic adhesive.

The aim of present study in vitro was to evaluate and compare the effects of different washing times of enamels etched with low phosphoric acid solution which makes unsoluble salts and etched but contaminated with saliva on shear bond strength of an orthodontic adhesive to enamel, and to observe the washing effect on the etched enamel surface by scanning electron microscope. All brackets were bonded with Mono-Lok2(TM)) on the labial surface of extracted human bicuspids after etching with 20w/w% and 37w/w% phosphoric acid solution for 60 seconds and then washing for 0, 5,10 and 20 seconds respectedly, After etching with 37w/w% phosphoric acid solution and contaminating with saliva for 30 seconds and then washing for 0, 5, 20 and 30 seconds and re-etching for 10 seconds. After 24 hours passed in the 37C water bath, the shear bond strengths were measured on Universal Test Machine. The data were evaluated and tested by ANOV A and Duncan's multiple range test, and those results were as follows. 1. There was no significant differences between (P>0.05) shear bond strength of bonded bracke ts with 5, 10, 20 seconds washing etched enamel using 37%w/w% phosphoric acid solution. 2. The shear bond strength of bonded brackets with 20w/w% phosphoric acid and then washing for 5 seconds showed bonded strength durable to occlusal force but its coefficiency score was high and etched surface was not cleaned completely and therefore it was assumed that its clinical application is not applicable. 3. There was no significant differences between (P>0.05) shear bond strengths of bonded brckets with washing for 5 seconds etched enamel using 37w/w% phosphoric acid solution and 10, 20 seconds washing etched enamel using 20w/w% phosphoric acid solution. 4. The shear bond strength of washing for 5 seconds etched enamel which was contaminated with saliva showed sufficient bonded strength durable to occlusal force but its coefficiency score was high and therefore its clinical application was not applicable. 5. After etching, the sample contaminated with saliva showed the sufficient shear bond strength even washing 20 seconds without re-etching.

Primary lymphoma of the kidney is rare, and in most cases is attributable to lymphomatous renal infilitration of systemic non-Hodgkin's lymphoma or an extension from an adjacent site of the disease. Since the renal parenchyma is not a lymphoid organ, the mechanism by which renal lymphoma occur remains poorly understood. We report here a case of primary bilateral B-cell renal lymphoma in 26-year-old man who was treated successfully with combination chemotherapy.
