5.A Experimental Study of PTEN (Phosphatase and Tensin) Role in Mesothelioma.

BACKGROUND: Conventional treatment for mesothelioma is largely ineffective. We evaluated the novel approach of adenoviral gene transfection of PTEN gene in mesothelioma cancer cell lines, inflammatory and epithelial subtype, which are sensitive to adenoviral p53. MATERIAL AND METHOD: Binary adenoviral PTEN and LacZ (Ad/GT-LacZ and Ad/GV16) vectors were used for transduction of the mesothelioma cell lines, REN (p53 sensitive). Protein levels were determined by Western blotting assay. Apoptosis was assessed by fluorescence-activated cell sorter analysis of subdiploid populations. Cell viability was determined with the XTT assay. Statistical analysis was performed with analysis of variance and the Student t test. RESULT: 72 hours after the treatment of adenoviral PTEN gene, cell killing were 32.9% for REN compared to control cell (2.5%) at MOI of 20. Also we observed the over-expression of proapoptotic protein, bax and decreased expression of bcl-2 protein in REN cells. But the expression of BCL-xl, Bak, Bad proteins were not altered. CONCLUSION: Adenovirus Pten-mediated overexpression of the Bax gene induces apoptosis and decreased cellular viability in p53-sensitive mesothelioma cells. These data suggest that the transfection of PTEN gene may represent a alternative gene therapy strategy to treat mesothelioma.

7.Mild Impairments in Cognitive Function in the Elderly with Restless Legs Syndrome.

OBJECTIVES: Cognitive impairment in restless legs syndrome (RLS) patients can be affected by sleep deprivation, anxiety and depression, which are common in RLS. The objective of this study is to investigate relationship between cognitive impairment and RLS in the non-medicated Korean elderly with controlling for psychiatric conditions. METHOD: The study sample for this study comprised 25 non-medicated Korean elderly RLS patients and 50 age-, sex-, and education-matched controls. All subjects were evaluated with comprehensive cognitive function assessment tools-including the Korean version of Consortium to Establish a Registry for Alzheimer's Disease Assessment Packet (CERAD-K), severe cognitive impairment rating scale (SCIRS), frontal assessment battery (FAB), and clock drawing test (CLOX). Sleep quality and depression were also assessed with Pittsburgh sleep quality index (PSQI) and geriatric depression scale (GDS). RESULTS: PSQI and GDS score showed no difference between RLS and control group. There was no significant difference between two groups in nearly all the cognitive function except in constructional recognition test, in which subjects with RLS showed lower performance than control group (t=-2.384, p=0.02). Subjects with depression (GDS> or =10) showed significant cognitive impairment compared to control in verbal fluency, Korean version of Mini Mental Status Examination in the CERAD-K (MMSE-KC), word list memory, trail making test, and frontal assessment battery (FAB). In contrast, no difference was observed between subjects who have low sleep quality (PSQI>5) and control group. CONCLUSIONS: At the exclusion of the impact of insomnia and depression, cognitive function was found to be relatively preserved in RLS patients compared to control. Impairment of visual recognition in RLS patients can be explained in terms of dopaminergic dysfunction in RLS.

8.Corneal Tobramycin Levels by Topical Administration of Fortified Tobramycin Solutions in Rabbits.

Bacterial keratitis is a common ophthalmic disease. In certain cases of pseudomonas keratitis, the corneal perforation may occur within 24-48 hours of the onset. Soa suitable, massive antibiotic should be promptly used in these cases. Intensive topical therapy with fortified aminoglycoside antibiotics is a current mainstay in the treatment of bacterial keratitis because insufficient antibiotic concentrations may not inhibit bacterial growth. In 1985, Glasser and associates reported the effect of longer dosing intervals on corneal gentamicin levels with using topical fortified gentamicin solutions in rabbits. We compared peak and trough antibiotic levels achieved in the rabbit cornea by various topical administrations of tobramycin which was prepared in a concentration of 13.6mg/ml by fortifying commercially available ophthalmic tobramycin solution with injectable drug. A loading dose consists of one drop given every minute for five minutes. 1. Eyes receiving one drop every hour(Group 1) and three drops every two hours(Group 5) showed little conjunctival hyperemia. Eyes given one drop every 30 minutes(Group 2) developed minimal inflammatory responses, as did eyes given a single loading dose followed by one drop each hour(Group 4). The moderate inflammatory response occurred in eyes receiving sequential loading doses(Group 3). Abnormalities in the cornea and the iris were not seen in all studied groups. 2. Gentamicin peak level in sequential loading doses group(Group 3) was significantly higher than those achieved by one drop every hour(Group 1) or one drop every 30 minutes(Group 2). At two hours of gentamicin administration, sequential loading doses(Group 3) produced remarkably high concentrations: than those produced in Group 1, Group 2, or three drops every two hours(Group 5). During the first four hours, Group 3 represented high antibiotic levels than those produced in Group 1, Group 2, Group 5, or a single loading dose followed by one drop each hour(Group 4). There were no significant differences between trough levels with on drop every hour(Group 1) and three drops every two hours(Group 5).

9.Seizure Induced by Photosonic Device.

BACKGROUND & SIGNIFICANCE: Photosensitive epilepsy may occur by the diverse new implements. We experienced three patients suffered from the first seizure while using photosonic devices. The device is composed of light stimulating goggles and sound producing headphones. CASE: All the patients were teen-aged school girls and the types of seizure were generalized tonic-clonic seizure, focal motor seizure with secondary generalization, and complex partial seizure. They lost their consciousness while using the devices with the red colored light. Events began without aura in two but one patient felt continuous red flash light until after taking off the device. EEG revealed right frontal dominant spike-wave complexes in one but other patients showed no epileptiform discharges. During a year follow-up the patient who showed EEG abnormalities experienced another seizure that was focal motor seizure with secondary generalization but remained two did not. COMMENTS: Though the video game epilepsy is regarded as a kind of photosensitive epilepsy, the emotional excitement may contribute to the occurrence of seizure. But, this photosonic device is used for relaxation and our three patients showed normal EEG while photic stimulation during the tracing. We suggest that seizure induced by the photosonic lev genuine type of photosensitive epilepsy.

10.The Effects of Fixation Device and Holder on Clinical Result of LASIK.

Many devices using in LASIK and PRK remake to minimize side effect. Fixation devices(EK-Fixation devices:Sejong Medical, Korea)and Holder(Global Fixation ring multiple teeth 13 mm diameter:Katena, USA; below holder)are made to minimize side effect called Ablation Decentration. Our study aims at evaulating the effect of using Fixation devices and Holder after LASIK. Corneal topography was used to anaylze centration in groups of patients:15 had LASIK using Fixation devices, 23 had LASIK using Holder, 20 had LASIK not using Fixation devices and Holder. Mean amount of decentration in topography of group of LASIK using Fixation devices was statistically significantly decreased in pre-operation(0.220+/-0.158)compared with 0.083+/-0.055 in post operation(P=0.017). In LASIK using Holder, mean amonut of decentration in topography in pre-operation(0.214 +/-0.124)was statistically significantly decreased compared with 0.146+/-0.071 in pre-peration, too(P=0.044). But mean amount of decentration in topography in LASIK not using Fixation devices and Holder, 0.156+/-0.100 in pre-operation was not statistically significantly different from 0.200+/-0.178 in post-operation. In LASIK groups using Fixation devices, mean amount of decentration in topography was significantly decreased comparing with other groups(P=0.010). And then, mean amount of decentration in topography from preoperative cornea center to postoperative cornea center using Fixation devices was 0.243+/-0.109, using Holder was 0.248+/-0.154 compared to control group(0.267+/-0.163). Groups using Fixation devices and Holder was significantly decreased comparing with other groups (P=0.010). In conclusion, Fixation devices and Holder are made to minimize side effect called Ablation Decentration in our study.
