1.Cutaneous Manifestations Associated with Liver Diseases.

It has been well recognized that the various cutaneous manifestations associated with liver diseases. A clinical study was made of 255 patients (AVH84, LC70, HC41, CAH26, CPH23, AH11) with the liver diseases at Yeungnam University Hospital during the periods from May to November, 1985. The authors classified the cutaneous manifestations into 7 groups according to pathogenesis, and compared them with other reports. The results were as follows: 1. In 255 patients with various liver diseases, 161 patients (63%) showed the various cutaneous manifestations. 2. The various cataneous manifestations were jaundice and/or pruritus (43.1%), vascular changes (39.6%), allergic manifestations (10.6%), nail changes (5.1%), hormone-induced changes (4.3%), pigmentary changes (3.5%) and others (2.4%) in that order. 3. Cutaneous manifestations were associated most frequently with liver cirrhosis (1.6 groups) and the least with chronic active hepatitis (0.7 group). 4. Allergic manifestations were seen mainly in patients with acute viral hepatitis. Three patients showed the serum sickness-like prodrome. 5. The other cutaneous manifestations were seen mainly in patients with chronic liver diseases.

2.An Anterosuperior Deltoid Splitting Approach for Plate Fixation of Proximal Humeral Fractures.

BACKGROUND: The purpose of this study was to evaluate the usefulness and safety of the anterosuperior deltoid splitting approach for fixation of displaced proximal humeral fractures by analyzing the surgical outcomes. METHODS: Twenty-three patients who could be followed-up for at least 8 months after the treatment of displaced proximal humeral fractures through the anterosuperior deltoid splitting approach were enrolled. We evaluated the reduction of the fractures and surgery-related complications at the last follow-up using X-ray results and clinical outcomes comprising the University of California at Los Angeles (UCLA) scoring system and the Korean Shoulder Society (KSS) score. RESULTS: At the last follow-up of patients treated using the anterosuperior deltoid splitting approach for internal fixation of proximal humeral fractures, we found 22 cases (95.6%) of bone union, a mean UCLA score of 28.3 (range, 15 to 34) and a mean KSS score of 82.1 (range, 67 to 95). Various surgery-related complications were noted; a case of varus malunion after fracture displacement, a case of nonunion, a case of delayed union, two cases of impingement, and a case of partial axillary nerve injury, which recovered completely through the follow-up. CONCLUSIONS: Plate fixation using the anterosuperior deltoid splitting approach could be another reliable option for treating displaced proximal humeral fractures.

3.A Case of Cerebral Venous Thromobosis Associated with Postsplenectomy Thrombocytosis.

Cerebral venous thrombosis is come from thromboplebitis of infectious origin and bland occlusion of cerebral vein by various causes. Although the main cause of cerebral venous thrombosis is thrombophlebitis, the bland occlusion causes it infrequently as a form of an infarct state. We report a case of cerebral venous infarction caused by splenectomy induced thrombocytosis.

4.MRI findings of castleman disease (Giant lymph node hyperplasia): case report.

Castleman disease is a relatively rare disease of differential diagnostic interest in patients with lymphadenophathy. The etiology and pathogenesis of the Castleman disease are still not elucidated and the MRI findings of disease has not yet been reported. Two patients with Castleman disease studied by MRI are presented: one case presented with a localized anterior mediastinal mass and the other case, with a neck mass. The lesions were characterized by relatively high signal intensities on both T1 and T2 weighted images in both cases, and significant degree of enhancement was seen in the cervical Castleman disease.

5.Gallbladder carcinoma: Regional lymph nodes metastases on CT scan.

CT scans of 23 patients with gallbladder carcinoma were retropectively reviewed to determine the incidence of lymphatic spread and to access the pattern of regional lymph node metastases. The lymphatic spread were noted in 74%(17/23). The involved patterns of regional lymph node metastases were nodes of porta hepatis 65%, portocaval nodes 41%, superior pancreaticoduodenal nodes 47%, posterior pancreaticoduodenal nodes 59%, nodes around the celiac trunk 35%, nodes around the superior mesenteric artery 29% and paraaortic nodes 35%. The findings suggested that the pattern of regional lumph node metastases in gallbladder carcinoma occurred along the normal lymphatic pathway of the gallbladder.

7.Intrapancreatic lipoma: a case report.

An echogenic lobulated round mass in the head of pancreas was found during abdominal ultrasonography for routine checkup on a 34 year-old man. CT and MRI showed characteristic findings of lipoma. Clinical followup confirmed intrapnacreatic lipoma, which had been reported only once proviously. This lesion should be added to the list of benign connective tissue tumor of the pancreas.

9.Pathophysiology of Upper Airway Collapse

Previous pathophysiologic studies have revealed that the upper airway dimension reaches a minimal value at the end of expiration, and pharyngeal negative pressure is not an essential prerequisite for upper airway collapse. Furthermore, pharyngeal collapse occurs in spite of increased activity of upper airway dilator muscles in patients with obstructive sleep apnea (OSA) compared with normal subjects. Pharyngeal sensory threshold is elevated in patients with OSA, which contributes to inadequate activity of upper airway dilator muscles. Pharyngeal narrowing results in several types of airflow limitation, of whom the mechanisms are mainly explained using Starling resistor model and negative effort dependence. Several studies have been conducted over the past years to validate polysomnographic surrogate markers which indicate specific pathophysiologic phenotypes, and such attempts may be able to lead us to personalized treatment for OSA.
10.Fibrolipomatous hamartoma of the median nerve: A case report

A case of fibrolipomatous hamartoma of the median nerve in the palm and digits of the hand in a 14 year old man developed in the right hand is reported. This is rare disease characterized by fatty and fibrous tissue infiltration in themedian nerve of the palm and digits. This case was treated by limited debulking of the redundant tumor tissue and excision of the transverse carpal ligament. This case was performed with relief of the symptom.
