1.Cysticercosis of Breast: A Case Report.

Authors reviewed mammographic ultrasonographic and MRI findings a patient with breast cysticerocosis, which was operated and pathologically proved. Mammography showed a 1 cm sized round radioopaque lesion with curvilinear calification, which was located near the pectoralis major muscle. Ultrasonograiphic findings showed heterogenous hypoechoic cystic lesion with internal hyperechoic nodule and posterior acoustic shadowing. T2Wl and proton density MR image showed low signal intensity with cresentic high signal intensity portion. 2D-FLASH dynamic MRI showed intermediate signal intensity and peripheral signal void area, which was not enhanced with Gd-DPTA. The possibility of cysticercosis can be considered be considered when a cytic lesion is discovered near the pectoralis muscle in a patient living in an endemic area.

3.Functional Recovery after Operative Treatment of Hip Fractures in the Elderly.

The goal of fracture treatment is to restore the patient to pre-injury function level. But the outcome assessment after orthopedic interventions has usually focused on physician-defined parameters of technical success, such as fracture union, alignment and range of motion rather than patient function and quality of life. However, the correlation among improvements in these parameters and functional status, psychosocial well being is inconsistent and weak in the elderly. Therefore authors assessed the functional status after surgical intervention of hip fractures in the elderly with functional recovery score(FRS). Independence in basic activities of daily living, in instrumental activities of daily living, in mobility, freedom from pain and intact memory were assessed in 133 hip fractures in the elderly. The hip fractures in the elderly resulted in 24.8% loss of function after the first year. The older the age, the more the loss of function after surgery of hip fractures. The more functional loss was observed in female patients and in patients with poor postoperative radiological results, but statistically insignificant (P>0.05). The more functional loss was observed in patients with underlying medical disease preoperatively and it was statistically significant (P<0.05). We recommend the evaluation of functional status in the elderly after hip fractures.

4.Chest x-ray findings of opportunistic infections

The chest X-ray findings of 20 cases of pulmonary opportunistic infections were analyzed according tocausative agents. The results we as follows; 1. Final diagnoses of 20 cases of opportunistic infections weretuberculosis in 6 cases, pneumocystis carinii pneumonia in 5 cases, bacterial infection in 7 cases, and fungalinfection in 2 cases. 2. The underlying diseases were leukemia in 6 cases, kidney transplantation in 6 cases,lymphoma in 3 cases, nephrotic syndrome in 1 case, nasopharyngeal cancer in 1 case, multiple myeloma in 1 case,agranulocytosis in 1 case, and hypogammaglobulinemia in 1 case. 3. In tuberculosis, all the 6 cases showed severemanifestations such as miliary tuberculosis, tuberculous pneumonia, moderately advanced tuberculosis andtuberculous pericarditis. 4. In pneumocystis carinii pneumonia, the most frequent findings were bilateral alveolardensities and peripheral field of the lung was saved in most cases. 5. In 2 cases of fungal infections bilatarealmultiple cavitary nodules were noted. 6. In cases of bacterial infection there were more cases of gram negativeinfection than gram positive and 2 cases of pseudomonas revealed bilateral multiple cavitary nodules.

Recently, the efficacy of immuno-oncologic agents for advanced hepatocellular carcinoma (HCC) has been proven in several trials. In particular, atezolizumab with bevacizumab (AteBeva), as a first-line therapy for advanced HCC, has shown tremendous advances in the IMBrave150 study. However, second or third-line therapy after treatment failure with AteBeva has not been firmly established. Moreover, clinicians have continued their attempts at multidisciplinary treatment that includes other systemic therapy and radiotherapy (RT). Here, we report a case that showed a near complete response (CR) of lung metastasis to nivolumab with ipilimumab therapy after achieving a near CR of intrahepatic tumor using sorafenib and RT in a patient with advanced HCC who had experienced treatment failure of AteBeva.

10.Clinical Results from Subsidence and Loss of Lordosis after Anterior Cervical Discectomy and Fusion.

OBJECT: The aim of this study is to analyze the statistical significance between the clinical result and subsidence and lordosis loss after anterior cervical discectomy and interbody fusion operation for degenerative cervical disc diseases. METHODS: From January 2004 to December 2006, retrospective study of 82 patients with symptomatic degenerative cervical disc disease who underwent anterior cervical discectomy and interbody fusion using the PEEK SolisTM cage and the carbon composite Osta-PekTM cage with autologous bone graft was done. Mean follow-up period was 13.2 months. Clinical results were assessed by bone fusion, subsidence, segmental lordosis loss and Odom's criteria. RESULTS: Of the 82 patients, single level fusion was accomplished in 67 patients and two-level fusion in 15 patients. Total accomplished fusion level was 97: C3/4 in 9 levels, C4/5 in 29 levels, C5/6 in 37 levels, C6/7 in 21 levels, C7/T1 in 1 level. 9 patients fit into complete resolved, 59 improved and 14 not changed, thus the success rate in clinical outcome was 82.9%. Bone fusion was successfully confirmed in the total of 82 patients. The height of disc space was decreased to 2.13+/-2.16mm: 2.64+/-1.81mm in the Osta-PekTM cage, 2.44+/-1.36mm in the PEEK SolisTM cage. The Osta-PekTM cage provided higher subsidence tendency than the PEEK SolisTM cage in our study. However, there was no statistical significance between the two cage groups. The subsidence and lordosis loss showed poor clinical outcome, but there was no statistical significance. CONCLUSIONS: There were no significant differences between the Osta-PekTM cage and PEEK SolisTM cage on clinical outco mes. Both cages showed low subsidences and lordosis loss with good fusion rate and clinical outcome.
