1.Detection of Human Papillomavirus 16/18, 31/33/35 in Verrucous Carcinoma of the Larynx by In Situ Hybridization with Human Papillomavirus DNA Probes.

Verrucous carcinoma of the larynx is a distinct and uncommon variant of well-differentiated squamous cell-carcinoma. The authors hybridized in situ a case of laryngeal verrucous carcinoma with DNA prebes to humman papillomavirus (HPV) 6, 11, 16, 18, 31, 33 and 35. The DNAs from the verrucous carcinoma of larynx hybridized with mixed DNA probes HPV-16/18 and 31/33/35. In addition, there was an evidence of HPV infection based on hybridization with mixed DNA probe HPV-6/11 in the adjacent papilloma tissue. By in situ DNA hybridization techniques, we clearly demonstrated human papillomavirus (HPV-16/18, and 31/33/35) related sequences in this neoplasm. These findings suggest the role of HPV-6/11 in the development of laryngeal papilloma ad HPV-16/18 and 31/33/35 probably on the progression to verrucous carcinoma.

4.Gastric pseudolymphoma: report of 3 cases

The pseudolymphoma of the stomach is known to be a benign porliferation of lymphoid tissue, which can bemistaken hostoloigcally for malignant lymphoma. The etiology of pseudolymphoma is controversial, but it B believedto be a manifestation of chronic inflammatory process. Authors present 3 cases of gastric lymphoma. Impresssionsof upper gastrointestual series were as follows; lymphoma or chronic gastritis in one case, ulcerative carcinomain another case and early gastric carcinoma in the other case. Initial endoscopic findings suggested infiltratingcarcinoma, ulcerative carcinoma and two benign gastric ulcers, respectively. One case was associated wtih earlygastric carcinoma.

5.Ureteroscopic Removal of Multiple Renal Pelvis and Lower Calyceal Stones .

The goal of surgical stone management is to achieve maximal stone clearance with minimal morbidity to the patient. Because of its efficacy, efficiency and low morbidity, extracorporeal shock wave lithotripsy(ESWL) is the first-line treatment for almost all urinary calculi. However, stone clearance after ESWL has been shown to be affected by the stone burden, location, chemical composition and kidney anatomy. Especially, the success rate for ESWL is reduced in lower pole stone(LPS). Percutaneous nephrolithotomy(PNL) and retrograde ureteroscopic intrarenal surgery(RIRS) are alternative treatment options for stones refractory to ESWL such as LPS. Although effective, PNLsubjects the patient to increased morbidity compared to ESWL and RIRS. Because of its low morbidity and relatively high success rate, RIRS for LPS is an attractive treatment modality in selective patients. We report a case of multiple renal pelvis and lower calyceal stones treated with ureteroscopy.

6.Comparison of Cost-Effectiveness between Before and After Risperidone in Schizophrenic Patients.

OBJECTIVES: This study was intended to compare cost-effectiveness between before and after risperidone in schizophrenic patients. METHODS: Twenty-four patients who have been prescribed traditional antipsychotics, and elapsed at least one year after changing into risperidone, were singled out retrospectively and evaluated in terms of treatment effects and quality of life by Brief Psychiatric Rating Scale (BPRS) and Quality of Life Scale (QLS), respectively. And we also reviewed hospitalization days, number of hospitalization, and number of visit to outpatient clinic while they took traditional antipsychotics and risperidone during one year, respectively. RESULTS: As for patients prescribed risperidone, there was a significantly decreasing trend in BPRS by 13.46 on the average, as QLS increased significantly by 18.96, compared to before risperidone medication. In addition, hospitalization days and number of hospitalization decreased by 18.96 days and 0.63 times per capita, respectively after risperidone treatment (p<0.05, p<0.05). The direct costs were estimated about 102, 000, 000 won for traditional antipsychotics and 100, 000, 000 won for risperidone during 1 year, showing no significant difference between two groups. The proportion in inpatient units out of total cost occupied 77.2% for traditional antipsychotics in comparison to 33.7% for risperidone. CONCLUSION: Risperidone may have better treatment effects and favorable cost-benefit effect than traditional antipsychotics despite of several limitation.

7.A Case of Takayasu's Arteritis Associated with Aortic Regurgitation and Coronary Artery Involvement.

Takayasu's arteritis is an arteritis of undetermined etiology, which affects the aorta and the proximal portions of its major branches. But aortic regurgitation and coronary artery involvement were unusual manifestations in this disease. We experienced a patient of Takayasu's arteritis who represented these unusual manifestations. Aortogram revealed grade IV aortic regurgitation and proximal segment narrowing of left main coronary artery. A case of Takayasu's arteritis associated with aortic regurgitation and left main coronary artery involvement is reported with a review of literatures.

Cryptococcal meningoencephalitis is one of the most cornInon fungal infections of the central nervous system, which mainly affects immunocompromised hosts, especially AIDS patients and patients with long-term steroid therapy. In advanced cases, it causes various complications such as encephailitis, hydrocephalus, and even death if not treated or inadequately treated. Authors have treated a case of cryptococcal meningoencephalitis with focal cerebellar lesion, whose primary infection sources were thought to be right frontal sinus and the ethmoidal sinus, with combination chemotherapy of amphotericine B and flucytosine.

A case of traumatic thoracolumbar epidural hemorrhage is presented in a 37-year-old man with ankylosing spondylitis. The patient complained neck and back pain right after the accident but paralegia and difficult urination followed 2 days later. Spine CT showed thoracolumbar epidural mass, which was confirmed to be hemorrhage associated with spine fracture and was solely responsible for paraplegia.
