1.The Incidence of Allergic Disease among the Pediatric Outpatient.

The incidence of allergic disease in pediatric outpatient department of a hospital, where primary care is undertaking, was studied from Apr. 1, 1989 to Mar. 32, 1990. The results were as follows, 1) The incidence of respiratory tract disease was the highest with the value of 57.2% of the total patients, followed by gastrointestinal tract disease(13.9%), infectious disease (6.7%), visit for preventive medicine (6.7%), allergic disease (3.0%), neurologic disease (2.9%), cardiovascular disease (2.8%) etc. 2) The allergic patients were 3.0% of total patient and sex ratio was M:F=1.4:1. 3) The allergic patients were most prevalent in1~3 years of age group (46.6%)followed by under 1 year old group (22.4%), and 4~6 years of group (21.3%). 4) The allergic disease was most prevalent in fall (42.8%), followed by spring (20.4%), and winter(19.0%). 5) Among the allergic disease, asthma was most frequent (67.4%), followed by allergic rhinitis(11.8%), atopic dermatitis (10.6%), urticaria (9.3%). 6) Bronchial asthma was most prevalent among 1~3 years of age group (51.9%) and in fall (50.0%) 7) Allergic rhinitis was most frequent among 1~3 years of age group (40.9%) and in fall (34.8%). 8) Atopic dermatitis was most frequent among under 1 year of age group(64.4%)and in spring and winter(28.8%). 9) Urticaria was most frequent among 1~3 years of age group (36.5%) and in summer (32.7%). 10) The percentage of each allergic disease to the total outpatient was as follows; asthma (2.0%), allergic rhinitis(0.40%), atopic dermatitis(0.3%), urticaria (0.3%). 11) Sex ratio of each allergic disease was as follows; asthma (1.5:1), allergic rhinitis(1.6:1), atopic dermatitis (1.1:1), urticaria (1.4:1). With these findings, we ovserved that allergic disease is the fifth most common disease group among the pediatric out patients, covering 3.0% of total patients. Recently, we have had the idea that the prevalence of allergic disease is increasing remarkably. The findings of this study can be used as one of basic data for the study of prvalence and increase of allergic disease with further similar trials afterward.

2.Immediate Internal Fixation of Open Fractures of the Long Bones

Between May, 1984 and October, 1986, twenty patients were treated at Han-Ra Hospital of JejuCity and Chung-Ang University Hospital of Seoul with immediate internal fixation with or without bone graft of open fractures of the long bones within three days. Fractured bones were ten tibiae, six femurs, three humeri and one radius and ulna. The injuries were classified on the basis of extent of soft tissue injury as defined by Gustilo and Anderson and consisted of four TypeI injuries, five Type II injuries and eleven Type III injuries. All wounds were closed primarily and radiological follow up was qhecked every month till union occurred. The complications included two deep infections, one non-union and one amputation. This study demonstrates that immediate internal fixation with bone graft when necessary is a beneficial method of treatment of open fractures of the long bones if copious irrigation, adequate debridement and skillful internal fixation technique are given golden period after injury. The results were related to the severity of the initial soft tissue injury and surgical technique and the period of union was not related to the individual bone. Autogenous bone grafting at the time of immediate internal fixation in comminuted or segmental fractures in which interfragmental compression cannot be obtained is recommended.

3.Study on Effect of Conjugated Equine Estrogen and Progestogen on Serum Lipid Profiles and Bone Mineral Density in Postmenopausal Women.

Hormone replacement therapy combined with progestogens induces changes in effect of estrogen on serum lipid levels and it has been known that the changes depend on a type and dosage of progestogen. It is also known that progestational agent induces positive ch-ange in bone mineral density. To study the effects of progestogen on lipoprotein and bone metabolism, we administ- ered conjugated equine estrogen 0.625 mg alone to 50 postmenopausal women, in combinat- ion with medroxy- progesterone acetate 5 mg to 40 postmenopausal women. The data demonstrated a beneficial effect in lipoprotein profiles in both groups. Total cholesterol in two groups decreased from the baseline values, LDL-cholesterol decreased significantly by 4.8 % in group I and 16.2 % in group II(p < 0.05), HDL-cholesterol increa- sed significantly by 11.3 % in group I and 14.7 % in group II(p < 0.05), triglyceride incre- ased slightly in both groups. Bone mineral density of femur was maintained and BMD of vertebrae increased by 1.1 % in group I and 2.0 % in group II, but it is not statistically significant. The differences of changes between two groups were not statistically significa- nt. Our results suggest that medroxyprogesterone acetate have no adverse effect on HDL -cholesterol and have no additive effect on bone mineral density in hormone replacement therapy.

5.A Study on Quality of Life of Patients with Acne.

BACKGROUND: Acne is a common chronic disease which most frequently affects the face. This visibility is a major cause for concern in affected individuals. OBJECTIVE: The purpose of this study was to investigate the quality of life of patients with acne. METHODS: We interviewed 130 acne patients, and determined the impact of acne on the quality of life with a questionnare. RESULTS: Among 130 cases of acne, the numbers of male and female patients were 52(40.0%) and 78(60.0%) respectively, and tbe mean age was 21 years. There was no difference between the sexes in the quality of life criteria used. The scores of social criteria of patients older than 20 years were significantly higher than thme of patients younger than 20 years. The severity of facial acne was correlated with the severity of chest acne but not with that of back acne. The severity of chest acne was correlated with that of back acne. The severity of facial acne was correlated with the scores of the physical, social and psychological criteria, and the severity of chest acne was correlated with that of physical and social criteria. However, the severity of back acne was not correlated with those of any of the criteria. The psychological well-being of the patients was moderately or highly affected by the acne more often than the other criteria. CONCLUSION: Acne has a substantial impact on the quality of life. The practical use of these indices are: (1) reveal areas of particulur concern in patients with acne; (2) to identify individuals who may require more intensive treaiment; and (3) to give a patient oriented inforrnation on the quality of life expected.

7.A Case of Allergic Contact Dermatitis due to DermatopR Ointment and PlancolR Lotion.

The anti-inflammatory activity of a corticosteroid may mask the contact allergic reaction it is causing. This may be an important reason why contact allergy to corticosteroids has often been missed. A simple persistent aggravation of a dermatitis in a patient treated with one or more topical corticosteroids is sufficient to raise a suspicion of corticosteroid hypersensitivity. A 23-year-old male visited our department for the evaluation of his aggravated pruritic oozing erythematous papules, scales and edema of the face. Previously, he was treated with Dermatop ointment and Plancollotion. A Patch test showed positive reactions to prednicarbate 10%, 1%, hydrocortisone-17-butyrate 10%, and triamcinolone 10%. We conclude prednicarbate in Dermatop ointment, hydrocortisone-17-butyrate in Plancol lotion to be the etiological agents.

8.A case of hyperimmunoglobuline E syndrome.

The hyperimmunoglobulin E syndrome is a primary immunodificiency disorder characterized by recurrent staphylococcal infections and markedly elevated serum IgE level. Clinical features are coarse face and severe infections of the skin-furunculosis or chronically pruritic dermatitis and sinopulmonary tract infection from infancy by coagulase positive Staphylococcus aureus, or Candida albicans etc. The patients's serum IgE level is elevated but the basic immunologic pathogenesis not fully understood. We have experienced a case of hyperimmunoglobulin E syndrome in a 26/12-year-old who had suffered from recurrent staphylococcal pneumonias and abscesses and chronically pruritic dermatitis from 1 month of age with elevated serum IgE level. A brief review of the related literature is presented.

9.Effect of Spiral Elastic Band on Gait Function in Patients with Chronic Stroke

PURPOSE: This study examined the effects of a spiral elastic band for the walking function on patients with chronic stroke. METHODS: Twenty one chronic stroke patients were recruited and divided randomly into the experimental group and control group. Both groups performed mat exercises and gait training three times a week for four weeks, and gait training was applied to the experimental group with additional spinal elastic bands. RESULTS: The results of this research were as follows. The 10MWT measurements showed that the velocities within and between the groups decreased significantly, and the results of TUG showed significant decreases in velocities after the interventions in both the control group and experimental group. On the other hand, there were no significant differences between the control and experimental group. The FRT measurements showed significantly increased stride lengths within and between the groups. The measurements of the stride length, stride velocity, cadence, and step length showed significant improvement within the groups, but there was no significant difference between the groups. The measurement of stance showed that the non-paralytic patients had a significant increase in the rates within the groups and a significant difference was observed between the groups. CONCLUSION: Spiral elastic bands are an effective intervention method for rehabilitation programs to enhance the walking function in the clinical field. A treatment needs to be developed for patients with walking problems due to various disorders by investigating the action mechanism of spiral elastic bands.
