4.An Evalution of Cell Mediated Immunity in Leprosy Patients and a Study of Treatment of Passive Transfer Immunity by CMI Conversion with Immune Cell Transfer.

Leprosy is an infectious dis as in which the immune machanism, in addition to the bacillus, plays an important role in Pathogenesis. As leprosy has two polar types, one of which is characterized by well preserved cellular immunity with a good prognosis and the other which shows no cellular immunity and a poor prognosis, it has been considercd the best human model for immunologic research. By studying the differences between these two typs, insights into immune deficiecies might well lead to the improved treatment of leproatous leproy(i.e.no cellular immunity) patients. Therefore, some immuno-theraputic trials for leprosy patients have already been reported by somc authors. In an attempt to survey spcific as well as nonspecific cellular immunity in leprosy, the authors prformed the lepromin and tuberculin skin tests with DNCB active cutanous sensitization on 203 leprosy patients in residence at the national leprosarium of Korea. All groups of Ieprosy patients showed decreased skin reactivity to the three tests, compared with normal, healthy control groups(p<0.01). This phenomenon is probably due to disorganization of lymphnode architecture and antigenic competition. Although a, statiscally significant difference could not be found in the tubercuIin and DNCB sensitization test results among all groups, the tuberculoid leprosy group and the arrested leprosy group showed stronger reactivity than the non-tuberculoid leprosy group. Patients positive for DNCB sensitization showed more positivity to the tuberculin test (66%)than to the lepromin test (30%) (p<0.01), thus suggesting that nonsp cific CMI and specific CMI exerted diffenent effects. Patients positive for DNCB sensitization also showed a greater probability (77%) to be responsive to either of the other two delayed cutanous hyperssensitivity tests than not. Leprosy patients in the third or fourth decade of life showed 63-66% positivity to DNCB active sensitization, while those in the six or seventh decad showed a positivity of 18-28%. These points toward significant correlation batween age and immun status. (p<0.01). The spontansous flare up during DNCB sensitization most frequently occurred on the 10th to 12th day of senitization: positive reaction after chalenge was most frequent in 72 hrs. after the test but some ractions were seen as late as five days after th. test. Two patiants of whole blood and its equivalent of leukocyte-rich plasme were irfused into 10 patients who had shown negative reaction to al! three tests. Conversion of delayed cutaneous hyprs:nsitivity among then was as follows; Four in tuberculin test (3 cases of which were strengthenings of preexisting ractivity). 2 in lepromin test, and 4 m DNCB sensitization. Of th 6 patients who showed a conversion in any one of the delayed cutaneous hypcrsensitivity evaluation tests, 4 showed conversion in more than two tests Of the 10 patients, who receivcd the transfusions (whole blood or lecocyte-rich plasma), 5 cases showed a high fever for 2 days afterrvards. The authors regard histocytotoxicity, incompatible histocompatibility, or perhaps simply a transfusion reaction as the underlying cause of this febrile reaction. One patient developed probably a phobia type psychosis following the blood transfusion and was subseuetly dropped from this study. Two months of observation following the transfusion revealed no definite clinical improvement.

5.Complications in the Treatment of Comminuted Fracture and Nonunion by Ilizarov Procedure

We reviewed the records of thirty-four patients who had been managed with the Ilizarov technique for the comminuted intra- and extra-articular fractures and nonunion of the long bones with or with-out bone defect. The goal of this study is to analysis the clinical complications of the Ilizarov technique, and to find out the methods solving them. 1. This study comprises 34 patients who were treated by Ilizarov technique at the Chonbuk National University Hospital from March, 1992 to June, 1993. 2. Among 34 cases, 22 were comminuted intra- and extra-articular fractures, and 12 were nonunion of the long bones. 3. The complications were divided intd problem, obstacle and complication; problem in 16 cases, obstacle in 11 cases and complication in 6 cases. 4. We conclude that the Ilizarov technique is a useful method in management of the severe commi- nuted fractures and the nonunion of the long bones, but we have to consider the complication sincerely and endeavor to reduce them.

7.Metamerism in composite resins under five standard illuminants - D65, A, C, FCW and TL84.

This study was done to present a criterion in selection of the most proper light sources and materials by measuring metamerism index(MI) of the light curing composite resins with spectrocolorimeter. Metamerism is defined when two objects appear to be the same color in one illuminant but different in another. This is due to the fact that they have different spectral curves that fail to match under the second illuminant. In this study, A1 & A3 shade of five light curing composite resins (Esthet-X, Filteck Z250, Filteck A110, Charisma, Vitalescence) were chosen based on Vita shade. Five samples were made for shade of each product with Teflon mold (diameter: 15mm, thickness: 2mm). Metamerism index of each samples on a Barium sulfate plate (L*=96.54, a*=0.19, b*=0.01) prepared for sample fixation were measured with spectrocolorimeter(Miniscan XE plus, Model 4000s, Hunter Lab, USA) by applying standard light source D65, C, Fcw, TL84 and A. Standardization was done with reference standard (X=80.8, Y=85.7, Z=90.8) and light trap. The results were as follows. 1. Different resins with same Vita shade showed recognizable color difference(DeltaE*>2). 2. All composites had MI below accepted value 0.5 between standard illuminant(D65, C, & A) and below 1.5 under fluorescent condition (Fcw & TL84). 3. MI value between D65 and A showed higher value than MI value between other source of light(p<0.01). 4. All resins except Z250 showed MI value that A3 is higher than A1 between D65 and A(p<0.05).

9.Actinomycosis on left submandibular area: a case report.

Actinomycosis is a chronic suppurative and granulomatous bacterial infection characterized by contiguous spread, abscess formation and sinus tract formation. There are four clinical forms according to the lesional site, as 1) cervicofacial, 2) thoracic, 3) abdominal, and 4) disseminated form. Recently, we experienced a case of 54 year-old patient with left mandibular actinomycosis. The pathognomonic findings of actinomycosis is sulfur granule with multiple filaments in Gram-stain and the treatment of actinomycosis is surgical excision of mass or sinus tract with massive antibiotics (esp. Penicillin) therapy for 6 to 12 months.

10.Usefulness & Pitfalls in CT Arterioportography.

PURPOSE: To evaluate the usefulness, patterns and appearances of false positive lesions, and technical problems of CTAP(CT Arterioportography). MATERIALS AND METHODS: CTAP was done in 45 patients in whom hepatic lesions were suggested on other radiologic studies. CTAP findings were compared with the informations obtained by operations, biopsies, and follow-up imagings. Additional findings, which were not detected in other radiologic studies, false positive manifestations, and technical errors were analyzed. RESULTS: CTAP detected 51 additional hepatic lesions which are not detected in other imaging studies. Of the 51 lesions, ten were true positive hepatic tumors, 2 were inflammatory lesions and 39 were false positive .lesions. False positive lesions included perfusion defects of periportal and perifissural areas, subcapsular perfusion defects and nonlobar, nonsegmental geographic perfusion defects. In 2 cases, CTAP aided in deciding the surgical strategy. On the other hand, there were 4 cases causing interpretational confusion and 6 cases of improper imaging due to technical errors. CONCLUSION: CTAP was valuable in detection of additional hepatic cancer lesion as well as in making the treatment plan. However, since many false positive lesions and improper imaging due to technical error occur during the examination, the understanding of such problems is essential for correct interpretation of CTAP.
