1.A Case of Adynamia Episodica Hereditaria .

Four members of a family suffering from frequent attacks of flaccid paralysis with asymptomatic myotonia were evaluated. There was an autosomal dominant pattern of inheritance and their earliest symptoms were noticed between 1-3 years of age. The plasma potassium level rised during the attack and an EMG demonstrated myotonic discharge. Treatment with acetazolamide was beneficial. These findings were consistent with adynamia episodica hereditaria. The clinical features and pathogenesis were also described.

5.A Clinical Study of Cryptococcal Infection.

A clinical study was carried out in 15 patients with cryptococcosis who were seen at Severance Hospital from June, 1974 to September, 1983 and the following results were obtained. Age of patients ranged from 13/12 to 72 years (mean 23.9) with the peak incidence in child and adolescence. Common symptoms at admission were headache, fever, chilling etc. Initial impression at admission were Tbc. Meningitis, Aseptic Meningitis, Cryptococcal Meningitis, lymphoma and hepatitis etc. in that order. Involved organs were meninges in 14 patients (93.3%), lymphnodes 4 (26.6%), lung 2, liver 2, skin 1, Brain 1, adrenal gland 1. Variable findings were obtained in initial CSF study, but all cases except two were positive in initial CSF india ink preparation. In our study, the prognosis was better in patients without coexisting disease than with coexisting disease.

Multiple dermatofibromas are rare and some case reports show this disease has been related to generalized defective immune function: autoimmune disease and altered immunity, such as AIDS and systemic lupus erythematosus(SLE), can be treated with immunosuppressive agents. But there are no specific reports of this disease developing in healthy people. We describe a case of multiple dermatofibromas in a 38-year-old healthy man who showed no abnormality of the immune function.

8.Clinical Features of Cutaneous Premalignant Lesions in Busan City and the Eastern Gyeongnam Province, Korea: A Retrospective Review of 1,292 Cases over 19 Years (1995~2013).

BACKGROUND: The global prevalence of premalignant lesions has been continuously increasing in recent years, but there has been little research regarding the distribution and incidence of cutaneous premalignant lesions in Korean populations. OBJECTIVE: We conducted this retrospective study to analyze recent trends in the incidence and clinical patterns of cutaneous premalignant lesions in the Korean population. METHODS: We reviewed 1,292 cases (3,651 lesions) of patients with cutaneous premalignant lesions, including actinic keratosis (AK) and Bowen's disease (BD), from the Department of Dermatology at Dong-A University Hospital (January 1995 to December 2013). RESULTS: The average cutaneous premalignant lesion annual incidence was 1.82%, and the incidence continuously increased from 0.70% to 4.25% over the study period. The most common cutaneous premalignant lesion was AK (75.85%), followed by BD (24.15%). The mean age of onset was 68.76 years (men, 70.89 years; women, 65.56 years), and the male:female ratio of patients was 1:1.52. Major skin cancers, including squamous cell carcinoma (SCC, 8.90%), basal cell carcinoma (BCC, 6.42%), and malignant melanoma (MM, 0.70%), were detected in 15.79% of patients with cutaneous premalignant lesions. Three patients (0.23%) were previously diagnosed with both SCC and BCC. In addition, 59.13% of patients had a single lesion, while 40.87% had multiple lesions. Patient age, history of previous skin cancers, and occupation-related exposure to ultraviolet radiation were more common in patients with multiple lesions. CONCLUSION: Cutaneous premalignant lesion incidence has gradually increased in the Korean population.

9.Prenatal Cytogenetic Analysis of 145 Fetuses Conceived by Intracytoplasmic Sperm Injectiion ( ICSI ).

Prenatal diagnoses were performed in 145 fetuses resulting from 73 singleton and 36 twin pregnancies, all established by intracytoplasmic sperm injection (ICS: amniocentesis in 108 patients and Chorionic villus sampling in one. The prenatal cytogenetic results were obtained from pregnancies after ICSI using ejaculated spermatozoa, epididymal spermatozoa, testicular spermatozoa and after the replacement of frozen-thawed embryos derived from ICSI. The Karyotypes were normal in 138 cases (95.2%) of the prenatal diagnoses and there were 2 cases (1.4%) de novo and 5 cases (3.4%) inherited chromosomal aberrations. The two cases of de novo abnormalities were: 46, XY, t(6;7)(q21;p22) and 47, XY, +21 (trisomy 21).

10.Compliance with Anticonvulsant Therapy in Epileptic Children.

PURPOSE: Good compliance with an anticonvulsant medication regimen is essential for the successful management of epilepsy. However, pediatric compliance with anticonvulsant therapy did not well be studied. The aim of this study was to identify the following key in noncompliance in pediatric out patients receiving anticonvulsant therapy determining factors. METHODS: Eighty-one epileptic outpatients under fifteen years of age with anticonvulsant therapy from the pediatric department of Kang-Dong Sacred Heart Hospital was enrolled for the study. Variables such as age, sex ratio, the reason of poor compliance, medication regimen and seizure frequency were extracted from each patients'medical record. RESULTS: Up to 37% of the patients was labeled as a poor compliant group. Compliance was low at the under 6 year and over 13 year-old patient group, and was also low for those who hadn't had a seizure in a while and for those who had a low seizure rate since last year. The mean plasma level of anticonvulsants was higher in the good compliant group than in the poor compliant group. The most reported obstacle to good compliance was forgetfulness. Side effect and drug rejection were also reported. CONCLUSION: To the high risk group of poor compliance, reminding, decreasing side effects, making simple administration adjustments have been reported to be useful in promoting compliance with medication regimens.
