1.Genistein-Induced Apoptosis of p815 Mastocytoma Cell.

BACKGROUND: Numerous studies demonstrated that genistein induced the decrease of proliferation and apoptosis in a variety of cells. However, there is no report about the effect of genistein on proliferation and demise of mast cells. OBJECTIVE: This study was conducted to investigate genistein-induced aoptosis of mast cells as it pertains to both its basic drug mechanism and the potential therapeutics of the pathologic conditions accompanying mast cell proliferation. METHODS: p815 murine mastcytoma cell line was used to assess the effects of genistein treatment including viability and proliferation, morphlolgic study, DNA electrophoresis, the effect of caspase inhibitor, western blotting, and mitochondrial event. RESULTS: Genistein indeced many apoptotic manifestations as evidenced by changes in cell morphology, generation of DNA fragmentation, activation of caspase 3, and DNA hypoploidy. The reduction of mitochondrial membrae potential and the release of cytochrome c to cytosol were also demonstrated. However, reduction of mitochondrial membrane potential and cytochrome c release were not prevented by caspase inhibitors zVAD-fmk and BocD.fmk, or PTP(permeability transition pore) blockers such as bongkrekic acid and cyclosporin A. CONCLUSIONS: This in vitro study suggests that pathologic increases in mast cell number possibly be regulated in vivo by therapeutic strategy enhancing apoptosis by treatment of genistein.

2.Clinical and Mycologic Studies of Tinea Cruris.

BACKGROUND: The incidence and causative dermatophytes of tinea cruris has been changed with time. Although clinical and mycologic studies of whole dermatophytosis have been reported many times, there have been only a few studies about tinea cruris reported. OBJECTIVE: The purpose of this study was to investigate clinical and mycological features of tinea cruuis. METHODS: We evaluated the clinical and mycologic aspects of tinea cruris in 598 patients with tinea cruris from January 1985 to December 1997 in Yeungnam University Hospital. RESULTS: The incidence of tinea cruris was 20.8% of 2,878 total dermatophytoses. The ratio of male to female patients was 23.9:1. And most of them were in their twenties and thirties. Most(82.1%) of them involved both sites of the groin. Concurrent dermatophytic infections of tinea cruris were noted in 29.4% of the cases and the most common one was tinea pedis(22.4%). Three hundred and thirty eight dermatophytes were isolated from 598 patients with tinea cruris. They were Trichophyton(T.) rubrum(95.3%), T. mentagrophytes(4.4%), Epidermophyton floccosum(1.2%), Microsporum canis(0.9%). CONCLUSION: Tinea cruris is a male predominant dermatophytosis, the most common causative organism is T. rubrum, and tinea pedis seems to be an important reservoir.

3.Ichthyosis Linearis Circumflexa.

The authors present a 3-year-old boy showing generalized migratory erythema with doubleedged scaling broders since 3 month after birth, Histologic examination revealed an acanthotic epidermis with hyperkeratosis, focal parakeratosis and papillomatosis. The PAS-positive, diastase-resistant amorphous materials were found in th space of parakeratoric scales and cytoplasms of spinous cells just below the parakeraroric lesion. On clectron microscopy, we could observe the round dense bodise in the upper spinous cells and a number of lipoid granules in the horny layer. Our patient showed characteristic clinical, clinical, microscopic and ultrastructural gearures of ichthyosis linearis citcumflexa(ILC). However, he showed no hair shaft abnormalities, which is the common finding of Metherthon/sysdrome(NS) along with ILC and atopic deathesis.

7.Methidathion Poisoning.

Although methidathion is an organophosphate insecticide, it is different from the other organophosphates in terms of toxicity. Because of its relatively high fat solubility, the apparent volume of methidathion distribution throughout the body is very high, indicating that hemoperfusion is not effective in removing this organophosphate from the body. Redistribution of methidathion from fat to blood can also occur when plasma levels diminish. Additionally, acetylcholinesterase aging, which is the loss of an alkyl side chain that prevents reactivation by oximes, is very rapid so that the effective reactivation by oximes is thwarted. Thus, methidathion's effect on acetylcholinesterase inhibition is long lasting, particularly with a high dose. In addition to its parasympatholytic effect and ability to induce muscle paralysis, methidathion poisoning is associated with a profound and long-lasting circulatory collapse due to sympathetic ganglion blockade. This report presents the case of a 55-year-old man who accidentally ingested a high dose of methidathion. He later developed enteroinvasive aspergillosis infection-induced multiple bowel perforations on two separate occasions while on mechanical ventilator support, resulting in a fatal outcome. The renin-angiotensin axis activated by sympathetic ganglion blockade may have reduced the patient's splanchnic blood flow, contributing to translocation of endotoxin. Also, the effect of excessive acetylcholine on non-neuronal acetylcholine receptors may have contributed to the development of fatal enteroinvasive aspergillosis in this patient.

9.The Shape of Küntscher nail for Treatment of Tibial Shaft Fractures

We have experienced 40 cases of the tibial shaft fractures treated with Küntscher nail from 1979 to 1986. Authors analysed these cases and our own experimental study concerned with the shape of Küntscher nail. The shape of Küntscher nail for the tibial shaft fractures should be designed according to the type and location of the fracture. The proximally bent and distally straight nail is used for the extension fracture, proximally and distally bent nail or entirely bent nail is inserted for the flexion fracture of the tibia. The large nail(over 13 mm in diameter) may produce injury to the patella because it has minimal flexibility. In order to permit easy driven down of nail and prevent this injury, the nail should b. bent into three to four segments and the length of the longest segment should not exceed the permissible length of straight nail (Permissible length is distance from entrance of nail to posterior cortex of the upper fragment, where tip of the (nail impinged-about 12cm) The midpoint of the middle segment of dual dent nail is placed at the fracture site. The middle segment of the nail may bent anteriorly for flexion fracture and posteriorly for extension fracture, securing the dynamic fixation of the fracture.
