1.RADIOGRAGHIC STUDY OF MARGINAL BONE LOSS AROUND OSSEOINTEGRATED IMPLANTS AFTER FUNCTIONAL LOADING.

Regular radiographic examination has been considered as an essential diagnostic method for the osseointegrated dental implantation. This study investigated the marginal bone loss through the measurement on the periapical radiograph around 46 endosseous dental implants. Marginal bone loss have been observed for 5 years after delivery of prostheses. The results were as follows : 1. According to the experimental periods, average marginal bone loss in total implant was 1.821mm in the first year, 1.833mm in the second, 1.892mm at third, 1.910mm at forth and 1.957mm at fifth after delivery of prostheses(P<0.05). 2. The average bone loss was 1.832mm in the maxillae and 1.819mm in the mandible in three years but there was no significant difference between the upper and lower jaw(P>0.05). 3. The average bone loss was 1.824mm in males and 1.818mm in females in five years but there was no significant difference according to the sex(P>0.05). These results indicate that the most of the alveolar bone loss occur within the first year after delivery of dental prostheses resulting in the exposure of polished neck positions, and the bone level stabilizes at first thread portion of the implant fixtures

4.Lymphatic, Vessel, and Neural Invasions in Gastric Cancer and Its Significance in Prognosis.

PURPOSE: This study was undertaken to investigate the prognostic significance of lymphatic invasion (LI), vessel invasion (VI), and neural invasion (NI) in gastric cancer and their correlations with other standard prognostic variables, such as the TNM stage. METHODS: The case histories of 100 gastric adenocarcinoma patients who had undergone a curative resection from 1992 to 1996 at Taegu Hyosung- Catholic University Hospital, Taegu, Korea were analyzed retrospectively. The Chi-Square test was used to determine the statistical significance of differences, and the Kaplan-Meier method was used to calculate survival rates. Significant differences in the survival rate were assessed using the log-rank test. The cox regression method was used for evaluation of independent prognostic significance. RESULTS: The 2-year survival rate was significantly lower (p<0.05) in LI (+) and NI (+) cases. The correlation between the T stage and LI, VI, and NI was statistically significant. Also, the correlation between the N stage and LI, VI, and NI was statistically significant. In the lymph node (+) group (n=43), the 2-year cumulative survival rate was higher in LI (-), VI (-), and NI (-) cases, but this result was not statistically significant. In the lymph node (-) group (n=57), the 2-year cumulative survival rate was higher in LI (-), VI (-), and NI (-) cases, but only the result for the NI (-) group was significant. Multivariate analysis demonstrated that LI, VI, and NI were not significant factors influencing the prognosis, although they were significant in the univariate analysis. CONCLUSION: This study identifies LI, VI, and NI as possible prognostic factor in gastric adenocarcinoma cases, but more careful research is needed.

5.Serum C-Reative Protein As a Predictor for Cardiac Sequelae in Kawasaki Disease.

PURPOSE: The aim of this study was to identify the predictive factors in the early laboratory findings for cardiac sequelae in Kawasaki disease(KD). METHODES: A retrospective review of the records was conducted of all children with KD who were admitted to the Ulsan Dongkang General Hospital, Masan Samsung Hospital, and Gyeongsang National University Hospital between January 1995 and December 1999. We analyzed and compared the early laboratory findings between the patients with and without coronary artery dilatation. RESULTS: A total of 981 patients were divided into two groups : 826 patients(84.3%) with normal coronary artery and 155 patients(15.7%) with coronary artery dilatation. Age and sex were not significantly different between the two groups. The mean serum C-reactive protein(CRP) in the coronary artery dilatation group and in the normal coronary artery group were 5.0 mg/dl(+/-5.3) and 4.1 mg/dl(+/-5.0), respectively, with a significant difference(P<0.05), whereas the other early laboratory findings had no difference between the groups. CONCLUSION: This study shows that the early serum CRP was higher in patients with KD who had coronary artery dilatation than in those with normal coronary artery. There may be a strong possibility of cardiac sequelae at a high level of serum CRP. However, the cut-off value of serum CRP could not be determined for the prediction of cardiac sequelae in patients with KD.

6.p21Cip/WAF1 activation is an important factor for the ERK pathway dependent anti-proliferation of colorectal cancer cells.

p21Cip/WAF1, an important regulator of cell proliferation, is induced by both p53- and extracellular signal regulated kinase (ERK) pathways. The induction of p21Cip/WAF1 occurs by prolonged activation of the ERKs caused by extracellular stimuli, such as zinc. However, not all the cells appeared to respond to ERK pathway dependent p21Cip/WAF1 induction. Here we investigated the cause of such difference using colorectal cancer cells. p21Cip/WAF1 induction and concomitant reduction of bromodeoxyuridine (BrdU) incorporation were observed by zinc treatment within HT-29 and DLD-1. However, HCT-116 cells with high endogenous p21Cip/WAF1 levels did not show any additional increment of p21Cip/WAF1 levels by zinc treatment and did maintain high BrdU incorporation level. The p21Cip/WAF1 induction by zinc depended upon prolonged activation of extracellular signal regulated kinase (ERK) was not observed in HCT-116 cells. The percentage of BrdU positive cells was 50% higher in p21Cip/WAF1 -/- HCT-116 cells compared to p21Cip/WAF1 +/+ HCT- 116 cells, and no cells induced p21Cip/WAF1 incorporated BrdU in its nucleus, yet confirming the importance of p21Cip/WAF1 induction in anti- proliferation. These results again support that p21Cip/WAF1 induction is a determinant in the regulation of colonic proliferation by the ERK pathway.

7.Effects of S-nitroso-N-acetylpenicillamine on the Neuronal Excitability of the Medial Vestibular Nuclear Neuron.

BACKGROUND: The medial vestibular nucleus is the largest one among the vestibular nuclei and known to play important roles not only in normal vestibular information processing but also in vestibular compensation. Glutamate is known to have a key role in vestibular compensation via long term potentiation and depression. But the action of nitric oxide related with glutamate is poorly studied. This experiment was designed to explore the effects of nitric oxide on the neuronal activity of a rat medial vestibular nuclear neuron using a nitric oxide enhancing drug, S-nitroso-N-acetylpenicillamine (SNAP). METHODS: Experiments were carried out on Sprague-Dawley rats aged 14 to 17 days. Neurons of MVN were obtained via enzymatic dissociation of a microtomized rat brainstem. Whole-cell membrane potentials were recorded at room temperature by using standard patch-clamp techniques. Action potentials were obtained after administration of SNAP. Changes of potassium currents were recorded using SNAP and ODQ (1H-[1, 2, 4] oxadiazolo [4, 3-a] quinozalin-1-one), an inhibitor of guanylyl cyclase. RESULTS: The mean spike frequency of action potentials was increased by adding SNAP. The mean amplitude of afterhyperpolarization was decreased by adding SNAP. The mean potassium current of medial vestibular nuclear neurons was decreased by SNAP. ODQ inhibited the SNAP-induced potassium currents. CONCLUSIONS: These results suggest that nitric oxide increases the neuronal activity of rat medial vestibular nuclear neurons by inhibiting potassium currents via a cGMP dependent mechanism.

8.Results of Phacoemulsificatin with PCL Implantation in Post-trabeculectomy Eyes.

We evaluated the clinical results of phacoemulsification and posterior chamber lens implantation in posttrabeculectomy eyes. 26 eyes operated during Jan. 1990 to Dec.1995 were evaluated retrospectively with preoperative and postoperative visual acuity, intraocular pressure(IOP) and antiglaucomatous agent usage.The cataract operations were done by clear corneal incisions avoiding the filtering bleb sites. The average interval between trabeculectomy and cataract surgery was 25 months. From a total of 26 eyes 11 had the diagnosis of primary open angle glaucoma(POAG) and 12 eyes chronic angle closure glaucoma(CACG). The preoperative visual acuity was 0.3 or less in 53.8%, and the postoperative visual acuity was 0.4 or better in 57.7%. The preoperative and postoperative IOPs were 14.4+/-4.3mmHg and 15.3+/-4.6mmHg respectively, showing a slight increase. Filtering bleb failure was observed in one eye in which trabeculectomy was formed twice. Antiglaucomatous agent usage decreased to 8 cases from an initial 10 cases. The preoperative IOP of the 8 cases was 17.3+/-5.1mmHg, while it was 13.1+/-3.3mmHg in the 18 cases in which the IOP was controlled without treatment. There was a significant difference between the two(P=0.03). There was a significant relationship between preoperative and postoperative antiglaucomatous agent usage(P=0.01). Taken individually, POAG showed a greater incidence of postoperative antiglaucomatous agent usage than CACG, but was insignificant(P=0.56).

10.Malignant Ectomesenchymoma in a Two-Month-Old Boy.

Malignant ectomesenchymoma is a rare tumor originating from remnants of migratory neural crest(ectomesenchyme) and composed of neuroectodermal as well as mesenchymal components. Neuroblasts and ganglion cells constitute the neuroectodermal components and rhabdomyosarcoma is the most frequently encountered mesenchymal components. We report a case of malignant ectomesenchymoma in a two-month-old boy who was presented with abdominal pain and urinary difficulty. The tumor appeared to arise from the prostatic region and enlarged to compress the bladder leading to bilateral hydronephrosis. Immunohistochemical studies for the resected tumor confirmed the presence of mixed ganglioneuroma and rhabdomyosarcoma establishing the diagnosis of malignant ectomesenchymoma. However, in the initial biopsy specimen of tumor, poorly differentiated round to oval cells positive for both desmin and vimentin staining were uniformly noted and the incorrect diagnosis of rhabdomyosarcoma was made. Intensive multi-agents chemotherapy, surgery and radiotherapy had failed in preventing the development of local recurrences. Subsequently, invasion of pubic bone and lung metastases ensued. This report enlightens the need for immunohistochemistry to seek possible neuroectodermal components in a tumor specimen suggestive of rhabdomyosarcoma.
