3.Mycological findings of Trichophyton tonsurans isolated in New Orleans area.

There had been no reports of trichophyton tonsurans infection in Korea yet. We have much chances to import the organism through international travels and exchanges. But we, Korean, have no experiences to observe T. tonsurans. Author tried mycological studies with T. tonsurans insolated in New Orleans, USA. The results are as follows: Gross findings of T. tonsurans showed that fine granular surface with light yellow hue or white color and grooving in the central area on the front side and mahogany brown color on the reverse side. On the urease test, various reddish discoloration was noted. Microscopic findings showed that septated hyphae, macroconidia, chlamydoconidia and microconidia. Macroconidia were 305 septated, smooth-surfaced, and were found more frequently in the whitish colony. Microconidia were characteristic in their arrangement and shape; round or oval shaped microconidia laterally to hyphae, some were match-head like terminal swelling. Hair perforation test showed positive results in 16 strains among 19 tested strains. All these findings are similar to T. rubrum and T. menatgrophytes, the most frequent isolates of dermatophytes in Korea, and we have to pay an attention to differentiate carefully.

The infantile cortical hyperostosis is relatively rare unknown cause disease. After the first report of a case of this was that of caffey in 1945, this disease is increasing in frequency. This disease characterized by subperiosteal new bone formation on various bone, leukocytosis, increased Erythrocyte Seidmentation Rate, fever. We experienced two cases of this and reported them in this paper with a brief review of tbe literature.

5.A Case of Molluscum Contagiosum on the Upper and Lower Eyelids.

Molluscum contagiosum is a contagious disease of the skin charaterized by the appearance of small globular, umblicated epithelial tumors, and caused by a virus of pox virus group. Authors experienced a case of molluscum contagiosum on the right upper and lower eyelids. And there was follicular conjunctivitis, but no corneal complication. The lesion was managed by simple extraction of the lesion under the local infiltration anesthesia.

6.Clinical and Mycologic Studies of Tinea Cruris.

BACKGROUND: The incidence and causative dermatophytes of tinea cruris has been changed with time. Although clinical and mycologic studies of whole dermatophytosis have been reported many times, there have been only a few studies about tinea cruris reported. OBJECTIVE: The purpose of this study was to investigate clinical and mycological features of tinea cruuis. METHODS: We evaluated the clinical and mycologic aspects of tinea cruris in 598 patients with tinea cruris from January 1985 to December 1997 in Yeungnam University Hospital. RESULTS: The incidence of tinea cruris was 20.8% of 2,878 total dermatophytoses. The ratio of male to female patients was 23.9:1. And most of them were in their twenties and thirties. Most(82.1%) of them involved both sites of the groin. Concurrent dermatophytic infections of tinea cruris were noted in 29.4% of the cases and the most common one was tinea pedis(22.4%). Three hundred and thirty eight dermatophytes were isolated from 598 patients with tinea cruris. They were Trichophyton(T.) rubrum(95.3%), T. mentagrophytes(4.4%), Epidermophyton floccosum(1.2%), Microsporum canis(0.9%). CONCLUSION: Tinea cruris is a male predominant dermatophytosis, the most common causative organism is T. rubrum, and tinea pedis seems to be an important reservoir.

7.Mating Type and its Mycological Characteristics of Micrpsporum Gypseum Comples.

Granuloma pyogenicum is a benign vascular tumor, usually about 0.5 to 2cm in diameter. It occurs as a single lesion with few exceptions and consists of pedunulated nodule with a dull red or slightly purplish color. We observed a 29-year old man who had multiple satellite recurrences in the scapular region which developed after excision of a single lesion of granuloma pyogenicum. Histapathologic findings showed many newly formed capillaries that had prominant endothelial cells and showed varying degrees of dilation without appearance of epiderrnal collarette sign. The lesions were much improved by X-ray irradiation.

8.Evaluation of Factors for Accurate Diagnosis of Scabies.

In order to evaluate the accurate diagnosis of scabies through exatr.inations of all requred. conditions, the author conducted an observation on 769 cases of scabies at the Department of Dermatology, Kyungpook University Hospital from January, 1962 to Octover, 1973, In 1962 the frequency of the disease appeared to be low but it dramatically rose to 20 or 30 times between 1963 and 1966, again showing an increasing tendency from 1971. The ratio of frequency between male and female was 4.2: 1. Seasonal occurrence of scahics was found to be highest in winter. Infectious rate in the family was 83.3% and incubation period vas estimated from 15 days to 2 months. Acarus scabiei or its ova were found in 46.5% of all cases examined and in patients over 16 years of age. Acarus scabiei or ova were chiefly found in vesicles of the finger web but the ova were also found in the subungual fold of the finger, which is an important site of examination.

9.Impact of increasing the level of copayments on the number of physician visits.

The level of copayment increased in order to stabilize the financial condition of the health insurance on 1986. An important question regarding the policy was whether the increase in the level of copayments reduced the utilization of medical services in the poor selectively. In spite of the importance of the research question, no study has been reported. This study was designed to find out changes in nuniU rs of physician visits, to explain charac teristics influencing the difference of utilization before and after the program. Finally the interaction effect between the program and the level of income was examined for the abover question. A total of 10,421 persons from eight institutions was selected as the study sample. Research findings are as follows. 1. The number of physician visits decreased by ten percent as a result of increasing the level of copayment. 2. The decrease was remarkable in some groups such as children, rural area and large family. 3. The most important factor which explained the difference was the number of physician visits before the introduction of the new program.. The more numbers of physian visits during the last year were, the more numbers of physian visits decreased after the program. 4. The interaction term between the program and the level of income was statistically significant in the multiple regression model which explained physician visits and its coefficient was negative. It means that an increase in copayment did not reduced the number of physician visits in the poor, selectively. 5. It can be concluded that imposing adequate copayment reduces the use of medical services as well as medical costs without serious damage in access especially for the poor pule.
