1.Focal Nodular Hyperplasis in Liver.

Focal nodular hyperplasia is a benign hepatic tumor mainly composed of nodules of hepatocytes and Kupffer cells separated by fibrous septa. In general, it is difficult to differentiate focal nodular hyperplasia and hepatocellular carcinoma on ultrasonography, conventional CT(computerized tomography), and angiography. But IV bolus CT is of particular value in the diagnosis of focal nodular hyperplasia because it can divide enhanced CT into early and late phase and can characterize tumor vascularity and analyze any intratumoral elements. In our case, it was seen as a hypoechoic mass lesion on ultrasonograpl'hy and hyperdense mass lesion on early-phase IV bolus CF and isodense mass, lesion on late-phase IV bolus CT. On angiography, hypertrophy of the feeding artery and tumor staining were well visualized. The patient underwent operation and the mass was pathologically confirmed to a focal nodular hyperplasia. We report the first case of focal nodular hyperplasia on IV bolus CT in Korea.

2.A Case of Nodular Amyloidosis.

A 38-year-old male patient visited our clinic complaining of three skin lesions on the scalp. There were yellowish to brownish, waxy, non-tender, walnut-sized nodules. Hematoxylin and eosin staining revealed amorphous pinkish material deposits in the dermis. The Congo red stain and Dylon stain under polarizing microscopy showed yellow-green birefringence and the immunoglobulin-lambda light chain stain showed a positive reaction. An electron microscopic examination revealed filaments with uniform diameter(6 to 10nm) that were straight and neither branched nor anastomosed. Based on the clinical, histopathological, immunohistochemical and electron microscopical findings, the skin lesions were diagnosed as nodular amyloidosis.

3.A Clinical Observation on the Antihypertensive Effects of Trimazosin Hydrochloride in patients with Mild to Moderate Hypertension.

Trimazosin Hydrochloride is a new orally active antihypertensive agent derived from the Quinazoline family, related chemically and pharamacologically to parazosin, but unlike its precedecessor, no first-dose syncope has been note, and no tolerance in the management of long term hypertensive patients. The antihypertensive effect of Trimazosin was observed from Oct. 1984 to Mar, 1985 at hospital of Chung-Ang Univesity as outpatient base involving a total 25 patients(Male;16, Female;9)with mild to moderate Hypertension(Diastolic B, P> or =90mmHG but< or =110mmHg). And the average age was 51.5 year(39-66yrs.) Our data demonstrate that 1) Trimazosin is an effective antihypertensive agent for controlling both systolic(166mmHg to 135.7mmHg)and diastolic(102.8mmHg to 85.3mmHg)pressure, and the mean arterial pressure was decreased from 123.8mmHg to 102.1mmHg(P%0.001). 2) We achieved significant sustained reduction in blood pressure with Trimazosin alone in 10 patients, Trimazosin plus Thoazide diuretics in 4 patients, Trimazosin plus-Beta-blocker in 5 patients, and with above triple regimensin 5 patients. 3) Side effects observed during Trimazosin theraphy(mean 3.8 months)appear to be minor and are reflection of its hemodynamic effects, (fatigability 2, dizziness 3, nausea 3 and headache 3) except one had to stop due to severe pounding headache.

4.A Clinical Study on Factors affecting Spontaneous Resolution of Vesicoureteral Reflux in Children.

We report a case of 53-year-old man with plasmacytoid transitional cell carcinoma of the urinary bladder, which may be confused with plasmacytoma. The patient initially presented with gross hematuria and dysuria for two months. Cystoscopy and radiologic studies revealed multiple intraluminal protruding masses on the urinary bladder invading perivesical fat tissue. After urinary cytologic examination and cystoscopic biopsy, radical cystectomy and pelvic lymph node dissections were done. Urine cytology showed single cells and poorly cohesive cells with round eccentric nuclei, bi-or multi-nucleation, indistinct nucleoli, coarse chromatin, and abundant basophilic cytoplasm within relatively clear background. The cytologic findings of tumor cells were similar to the plasma cells seen in plasmacytoma. The tumor of the bladder was composed of discohesive, individual cancer cells with diffuse pattern that simulated lymphoma or plasmacytoma. Immunohistochemical and electron microscopic studies clearly established the epithelial nature of the neoplasm. Recognition of this plasmacytoid type of transitional cell carcinoma of the urinary bladder can avoid the misdiagnosis.

5.Cutaneous Manifestations in Sepsis Caused by Methicillin-Resistant Staphylococcus aureus.

Sepsis refers to the systemic response to serious infection. Patients with sepsis usually manifest fever, tachycardia, tachypnea, leukocytosis, and a localized site of infection. Methicillin-resistant Staphylococcus aureus(MRSA) is a gram-positive, nonmotile, aerobic, catalase- positive coccus, which is resistant to all the B -lactam antibiotics. Cutaneous manifestations in sepsis are maculopapules, nodules, petechiae, ecchymoses, purpurae, pustules, vesiculobullae, hemorrhagic bullae and ulcers. When MRSA is identified in blood cultures and skin tissue cultures, the skin lesions can be considered as cutaneous manifestations in sepsis caused by MRSA. We report two cases with erythematous pustules, petechiae, hemorrhagic bullae and maculopapules caused by MRSA sepsis. MRSA grew in blood cultures and skin tissue cultures.

7.Fibrolipomatous hamartoma of the median nerve: A case report

A case of fibrolipomatous hamartoma of the median nerve in the palm and digits of the hand in a 14 year old man developed in the right hand is reported. This is rare disease characterized by fatty and fibrous tissue infiltration in themedian nerve of the palm and digits. This case was treated by limited debulking of the redundant tumor tissue and excision of the transverse carpal ligament. This case was performed with relief of the symptom.

9.Heart Rate Adjustment of ST Segment Depression as a Myocardial Ischemia Index of Coronary Artery Disease.

BACKGROUND: Exercise testing is an importnat diagnostic and prognostic procedure in the assessment of patients with ischemic heart disease. But standard ST-segment depression criteria was not high enough to estimate coronary srtery disease. Recently, the heart rate adjustment of ST segment depression, ST segment/heart rate slope and index, have been proposed as a more accurate criteria for diagnosing significant coronary artery disease. The objective of this study was to compare the discriminating power of proposed ST segment/heart rate slope and index with that of a standard method of assessing exercise-induced ST segment depression for estimating coronary artery disease. METHODS: Sixty nine patients with ischemic heart disease were studied with exercise treadmill testing and coronary angiography. Computer-measured ST-segment amplitudes were obtained and analysis of the heart rate-adjusted ST segment depression(ST/HR slope and big up tri, Delta ST/HR index) was done. The sensitivity, specificity, and extent of coronary artery disease on each criteria were compared. RESULTS: 1) The sensitivity of big up tri, Delta ST/HR index partition of 1.6uV/beats/min was slightly higher(83%) and the specificity of ST/HR slope partition of 2.4uV/beats/min was higher(87%) than the standard exercise electrocardiographic criteria. 2) Early onset of ischemic ST-segment depression, profound ST-segment depression(> or =2mm), and downsloping ST-segment were associated with more extensive coronary artery disease. 3) On ST/HR slope, no CAD was 1.7+/-0.26uV/beats/min, one vessel disease was 2.6+/-0.34 uV/beats/min, two vessel disease was 2.7+/-1.36uV/beats/min, one vessel disease was 2.8+/-0.35uV/beats/min, and on big up tri, Delta ST/HR index, no CAD was 1.8+/-0.38uV/beats/min, one vessel disease was 2.8+/-1.36uV/beats/min, two vessel disease 3.4+/-1.44uV/beats/min, and three vessel disease was 3.7+/-2.95uV/beats/min. The increment of ST/HR slope and big up tri, Delta ST/HR index were associated with the coronary artery disease and its severity, but the correlations were not high enough. CONCLUSION: The heart rate adjustment of ST segment depression was not high enough for improved detection of coronary artery disease, compared with standard ST-segment depression criteria. But these indexes can be improved the clinical usefulness of the treadmill exercise test for coronary aretry disease.

10.Evaluation of intracranial hemorrhage in neonates and infants using real time sonography

The real time high resolution mechanical neurosonographic sector scanner is a convenient and useful instrumentfor detection of intracranial lesions in premature infants as well as low birth weight. From this experience,authors report with 104 neonates and infants who had been suspected intracranial hemorrhage and had been detectedand graded by sonography. The results were as followed: 1. The incidence of intracranial hemorrhage is 20 of 46(43.5%) neonates who had been studied before 3 days of age. But 7 of 16(43.7%) neonates showed ICH between the 4-7days of age. 2. There was no difference of prevalence in sex ratio in neonates with ICH. 3. There was closerelationship between ICH and gestational weeks. 15 of 27 (55.6%) neonates who were born under 32 gestational weeksshowed ICH was graded as 3 or 4 in 7 of 10. 4. 10 of 16(62.5%) neonate who were born under 1,500 gram body weighthad ICH within first 4 days. The severity of ICH was graded as 3 or 4 in 7 of 10. 5. The mode of deliveryinfluenced neonatal ICH incase of difficulty delivery such as forcep(4/4, 100%), vacuum (7/10, 70%), breech (3/3,100%), and C/S(13/33, 39.4%). 6. Incidence of ICH in neonates with pathologica lung conditions (29/40, 72.5%) washigher than ICH with normal chest findins(15/64, 23.4%). 7. For frequent follow up study of brain, neurosonographywas very helpul and economic in case of ICH, and could find congenital glioblastoma multiformed(1), and low gradeastrocytoma(1) incidentally.
