1.Paget's Disease of the Breast: Significance of Mammographic Findings.

PURPOSE: To evaluate the significance of mammographic findings in patients with paget's disease of the breast. MATERIALS AND METHODS: The clinical and mammographic records of ten patients with surgically-proved paget disease of the breast were retrospectively reviewed. RESULTS: Of these ten patients, the mammograms of two(20%) were normal, while those of six(60%) showed nipple or subareolar abnormalities, and those of six(60%) showed parenchymal abnormalities. Of six patients who had nipple change only, the mammograms of four showed nipple orsubareolar abnormalities and those of three showed parenchymal abnormalities. Of two patients who had nipple change and palpable mass, the mammograms of both showed nipple or subareolar and parenchymal abnormalities. Of two patients who had palpable mass only, the mammograms of both failed to show nipple or subareolar abnormalities and those of one showed parenchymal abnormalities. CONCLUSION: In Paget's disease of the breast, mammographic findings are nonspecific and are limited to an evaluation of the extension of the primary carcinoma. Therefore clinical findings should be correlated with mammographic findings.

2.Two Cases of Atypical Optic Neuritis.

Optic neuritis refers to any inflammatory disorder of optic nerves, but it usually denotes an acute or subacute disease of optic nerves attributed to inflammation associated with demyelination. The diagnosis of optic neuritis is usually made on a clinical ground. The condition usually presents as a painful subacute unilateral loss of vision, which progresses over a few days to 2 weeks. The pain varies in severity, although it typically does not interfere with sleep. We report 2 cases of atypical optic neuritis rapidly recovered by the administration of high doses of steroid that presented with severe eyeball pain accompanied by vague visual loss and painlessly insidious onset of visual loss respectively, with normal findings in fundoscopic examinations, visual evoked potentials and orbital MRIs.

3.Long term effects on oral progestogen (medroxyprogesterone acetate) on the bone mineral densities and the level of serum lipid metabolism during estrogen replacement therapy in postmenopausal women.

BACKGROUND: In postmenopausal women, progestogen should be added to protect the endometrium from hyperplasia or carcinoma induced by the unopposed estrogen. However, the effects of progestogen on bone mineral densities and serum lipodproteins have not been precisely evaluated in Korean postmenopausal women. METHODS: To evaluate the effects of progestogen on bane mineral densities and serum lipoprotein in estrogen rephcement therapy, we canducted a 2-year trial of long conjugated equine estrogen(conjugated estrogen 0.625mg/day) with or without cyclic progestogen(MPA 5mg/day for 12 days) in 120 postmenopausal women. In all subjects, bone mineral density was measured in lumbar vertebra(L2-L4) and femur neck using dual energy X-ray absorptiometry(DEXA) and serum lipoprotein was measured from the beginning of the treatment, 12 manths, and 24 manths later, respectively. RESULTS: BMD of femur neck in both groups increased but not significantly compared to basal level at 12 months and/or 24 months of treatment. As for BMD of lumbar spine, it increased significantly in both groups. Both groups showed a significant decrease in the levels LDL cholesterol, but there was no statistical significance in serum triglycerids. Conjugated estrogen plus MPA group in constrast to conjugated estrogen only group showed a significant decrease in total cholesterol levels. CONCLUSIONS: These results suggest that the addition of MPA of the daily of 5mg for 12 days cyclically in estrogen replacement treatment appear to be effective in postmenopausal women with protection on bone mineral density and maintenance of long-term favorable effects on serum lipoprotein.

We report a case of a cardiac lipoma arising from the free wall of right atrium, which was diagnosed as the presumed source of angina by compressing the right coronary artery in a 54-year-old woman. Surgical excision of the tumor was performed well. She continues to do well for six months without angina.

5.Sonographic Features of Phyllodes Tumors of the Breast: A Pictorial Review.

Phyllodes tumors of the breast are rare, accounting for 0.3-0.5% of breast neoplasms. The tumors are divided into benign, borderline and malignant lesions according to the histological features. Phyllodes tumors are commonly recurrent, so wide local excision is considered as the only curable treatment. Therefore, an accurate pre-operative diagnosis can reduce recurrence after treatment. On ultrasonography, benign phyllodes tumors are generally seen as well circumscribed, oval shaped, hypoechoic or isoechoic masses, and occasionally, internal clefts or cystic portions are visible. Borderline or malignant phyllodes tumors tend to be larger and more highly categorized than benign tumors. Most phyllodes tumors present as a palpable mass, which usually require sonographic evaluation with a core biopsy; however, the reported diagnostic accuracy is approximately 60 percent, due to limitations of the histological features. Thus, a follow-up ultrasonographic evaluation is essential after a core biopsy and a phyllodes tumor should be considered in cases of growth occurring after a benign biopsy.

6.Isolated Pulmonary Arterial Hypertension-Janus' Faces of Hyperthyroidism.

We describe a 54-year-old woman with isolated pulmonary arterial hypertension accompanied by hyperthyroidism due to Graves' disease. Her pulmonary artery hypertension resolved spontaneously after restoration of euthyroidism. This case suggests that hyperthyroidism should be considered a reversible cause of pulmonary arterial hypertension.

7.Deep Learning-based Prediction of Axial Length Using Ultra-widefield Fundus Photography

Purpose:
To develop a deep learning model that can predict the axial lengths of eyes using ultra-widefield (UWF) fundus photography.
Methods:
We retrospectively enrolled patients who visited the ophthalmology clinic at the Seoul National University Hospital between September 2018 and December 2021. Patients with axial length measurements and UWF images taken within 3 months of axial length measurement were included in the study. The dataset was divided into a development set and a test set at an 8:2 ratio while maintaining an equal distribution of axial lengths (stratified splitting with binning). We used transfer learning-based on EfficientNet B3 to develop the model. We evaluated the model’s performance using mean absolute error (MAE), R-squared (R2), and 95% confidence intervals (CIs). We used vanilla gradient saliency maps to illustrate the regions predominantly used by convolutional neural network.
Results:
In total, 8,657 UWF retinal fundus images from 3,829 patients (mean age, 63.98 ±15.25 years) were included in the study. The deep learning model predicted the axial lengths of the test dataset with MAE and R2 values of 0.744 mm (95% CI, 0.709–0.779 mm) and 0.815 (95% CI, 0.785–0.840), respectively. The model’s accuracy was 73.7%, 95.9%, and 99.2% in prediction, with error margins of ±1.0, ±2.0, and ±3.0 mm, respectively.
Conclusions
We developed a deep learning-based model for predicting the axial length from UWF images with good performance.

9.Aortic Dissection and Rupture in a Child.

After developing sudden severe chest pain, an 11-year-old boy presented to the emergency room with chest pain and palpitations and was unable to stand up. The sudden onset of chest pain was first reported while swimming at school about 30 minutes prior to presentation. Arterial blood pressure (BP) was 150/90 mmHg, heart rate was 120/minute, and the chest pain was combined with shortness of breath and diaphoresis. During the evaluation in the emergency room, the chest pain worsened and abdominal pain developed. An aortic dissection was suspected and a chest and abdomen CT was obtained. The diagnosis of aortic dissection type B was established by CT imaging. The patient went to surgery immediately with BP control. He died prior to surgery due to aortic rupture. Here we present this rare case of aortic dissection type B with rupture, reported in an 11-year-old Korean child.

10.Report on the External Quality Assessment Scheme of Hepatitis Viral Markers in Korea, (2016–2017).

As part of the immunoserology program of the Korean Association of External Quality Assessment Service, we organized two trials on the external quality assessment of hepatitis viral markers in 2016 and 2017. The hepatitis viral antigens and antibodies program consisted of 10 test items. We delivered two and three types of pooled sera specimens to 965 and 965 institutions for the first and second trials of external proficiency testing in 2016, respectively. The number of participating laboratories was 915 (94.8%) and 913 (95.0%) in the first and second trials in 2016, respectively. We also delivered three kinds of pooled sera specimens to 936 and 1,015 institutions for the first and second trials of external proficiency testing in 2017, respectively. The number of participating laboratories was 920 (98.3%) and 996 (98.1%) in the first and second trials in 2017, respectively. The most commonly tested items were hepatitis B surface antigen, followed by the antibodies to hepatitis B surface antigen, anti-hepatitis C virus, hepatitis B envelope antigen, antibodies to hepatitis B envelope antigen, anti-hepatitis A virus and antibodies to hepatitis B core antigen. The most frequently used methods for detecting viral markers were the chemiluminescence immunoassay and the electrochemiluminescence immunoassay, but they yielded a few-false positive results due to the matrix effect. The immunochromatographic assay yielded false-negative results for anti-hepatitis A virus due to low sensitivity. Continuous improvement in the quality of viral hepatitis testing through participation in the survey seems necessary.
