2.Antenatal Screening for Gestational Diabetes by 50-g, 1-hour Glucose Screening Test.

OBJECTIVE: Gestational diabetes mellitus is defined as carbohydrate intolerance of variable severity first diagnosed during pregnancy. It is associated with adverse outcomes of pregnancy including obstetrical complications such as increased rate of cesarean sections, preeclampsia, and birth trauma, and perinatal morbidities, such as macrosomia, hypoglycemia, hypocalcemia, and hyperbilirubinemia. Therefore, screening for gestational diabetes mellitus and early diagnosis of this condition allows intervention to be carried out, thereby, the reduction of the untoward effects mentioned above can be minimized. METHODS: Screening for abnormal glucose metabolism was carried out in 489 pregnant women. A 50-g oral glucose load without regard to time of day or last meal, and a 1-hour plasma glucose determination with a threshold of 140mg/dl were used as a glucose screening test(GST). Patients with an abnormal GST underwent an oral glucose tolerance test(GTT). RESULTS: The overall incidence of gestational diabetes was 2.7%. The occurrence of this disorder was significantly related to the age of pregnant women, parity, or the presence of risk factors for gestational diabetes and obesity(Body Mass Index> or =26kg/m2). CONCLUSION: This study suggests that Korean pregnant women should be screened for gestational diabetes.

4.Serum Anti-Mullerian Hormone and Inhibin B Levels at Ovulation Triggering Day Can Predict the Number of Immature Oocytes Retrieved in In Vitro Fertilization Cycles.

The aim of this study was to investigate whether serum levels of anti-Mullerian hormone (AMH) and inhibin B at ovulation triggering day correlate with the number of immature oocytes obtained from stimulated in vitro fertilization (IVF) cycles. Fiftynine consecutive cycles of ovarian hyperstimulation and IVF were selected from 45 women who had tubal (n=18) or unexplained infertility (n=27) and obtained at least one oocyte. Serum levels of AMH and inhibin B at ovulation triggering day were measured by enzyme-linked immunosorbent assay (ELISA). Univariate analysis and multiple regressions revealed that serum AMH or inhibin B levels were significantly correlated with immature oocyte count and the correlation coefficients were higher compared to the mature oocyte count. Serum AMH and inhibin B levels on triggering day seems to be more closely related with the immature oocyte count and thus could be good predictors to determine the immature oocyte count in IVF cycle.

5.The Clinical Usefulness of Cervicovaginal Fetal Fibronectin as a Prediction of the Time of Induction.

OBJECTIVES: Fibronectin is a high-molecular-weight glycoprotein found in the plasma and extracellular matrix. The presence of fetal fibronectin in cervicovaginal secretion of pregnant women during the second and the third trimester has been suggested as a means of identifying women in preterm labor who be delivered prematurely. Therefore, our purpose was to determine the clinical efficacy of the presence of fetal fibronectin in cervicovaginal secretion of patient in full term as biochemical predictor of Time of Induction. METHODS: Fetal fibronectin was obtained at external os and posterior vaginal fornix in 30 women in full term who had no uterine contraction with intact membrane. The cervix was assessed by digital vaginal examination and scored with a modified Bishop score. RESULTS: There was a good correlation in delivery time after induction between the fetal fibronectin and the modified Bishop score (r=0.695,P<0.005) CONCLUSION: The presence of fetal fibronectin is better than Bishop score that is dependent on clinical experience as biochemical predictor of Time of Induction.

6.Apolipoprotein epsilon4 and Cognitive Function, Daily Living Ability and Behavioral Symptoms of Dementia.

OBJECTIVES: We investigated the relationship between apolipoprotein epsilon4 and clinical features of dementia. METHODS: Samples were obtained from 33 dementia patients in Korea, Buyeo Geriatric Hospital and apolipoprotein E polymorphisms were analysed using polymerase chain reaction and restriction fragment length polymorphism. The multidimensional evaluating methods (MMSE-K, B-ADL, BEHAVE-AD, GDS, GMHR) for dementia were performed to analyze relationship with Apolipoprotein E genotypes. RESULTS: The distributions of the apolipoprotein E genotypes of the dementia patients were presented by epsilon3/epsilon3 (58%), epsilon3/epsilon4 (22%), epsilon2/epsilon3 (15%), and epsilon4/epsilon4 (0%). There is no significant relationship between apolipoprotein epsilon4 and the findings of multidimensional evaluating measures. CONCLUSION: We could not find the association between apolipoprotein epsilon4 and the scores of the multidimensional evaluating measures of dementia patients. The results suggested that apolipoprotein epsilon4 might not play a main role in cognitive function, daily living ability and behavioral symptoms of dementia.

A case of arthrogryposis multiplex congenita is an extremely rare and complex syndrome characterized by contractures of multiple joints in different parts of the body. Arthrogryposis multiplex congenita usually occurs alone but may be associated with other malformations. 8.Alexithymia in Patients with Alopecia Areata.

OBJECTIVES: Alexithymia has been regarded as the general personality of psychosomatic disease, but it's controversial. The object of the study is to find out the relationship between alexithymia and alopecia areata. METHODS: Thirty one alopecia areata patients were compared to 31 normal healthy persons in alexithymic tendency using TAS-20K. Also MMPI and SCL-90-R were checked in both groups. Psychiatric diseases were ruled out. RESULTS: The scores of F, K, Hs, D, Hy, Pd, Pa, Pt and Sc of MMPI in alopecia areata patients were different from those in normal healthy persons. The scores of SOM, O-C, I-S, DEP, ANX, HOT, PHOB, PAR, PSY, GSI, PSDI, and PST of SCL-90-R in alopecia areata patients were significantly higher than those in normal healthy persons. In TAS-20K, the scores of Factor 1 and Total in alopecia areata patients were higher than those in normal healthy persons. CONCLUSION: Our results suggest that alopecia areata patients are more alexithymic than normal healthy persons.
