1.Nonalcoholic Fatty Liver Disease: Lifestyle Modification.

Non-alcoholic fatty liver disease (NAFLD) is the most common form of chronic liver disease. The prevalence of NAFLD is growing gradually worldwide with increases in obesity, sedentary lifestyles, and an unbalanced diet. NAFLD ranges from simple steatosis without inflammation to steatohepatitis that can progress to cirrhosis. There is no single effective treatment that has widespread effects in NAFLD. The cornerstone of treatment is lifestyle modification, including weight reduction, diet, and physical activity. An approximately 7-10% weight reduction via diet or physical activity can improve the liver histopathology. Risk factors for NAFLD include a high-calorie diet, high-lipid diet, high-carbohydrate diet, saturated fatty acids, trans fatty acids, cholesterol, high fructose intake, and low-choline diet. Factors that protect against NAFLD include a low-calorie diet, low-carbohydrate diet, low-lipid diet, monounsaturated fatty acids (MUFA), polyunsaturated fatty acids (PUFA), dietary fiber, coffee, green tea, and light alcohol consumption. Physical activity also helps to manage NAFLD with or without weight reduction. Recent reports found that resistance training is as effective as aerobic training. Lifestyle modification has very low compliance. To maintain a treatment program, a multidisciplinary team approach is required that includes physicians, dietitians, physical trainers, and psychologists.

3.A Biomechanical Study on the Fixational Strength of the Trans-pedicular Screw: In vitro measurement

With porcine vertebrae, the static and dynamic holding power of the pedicle screws under various conditions were measured to understand the biomechanical nature of the transpedicular screw fixation in spine. The objectives of the present study were; (1) to find the correlation between the insertion depth of the screw and the resulting holding power, (2) to determine the effect of bone cement augmentation on the screw fixation in a loosened hole, and (3) to assess the load-sharing mechanism between the cortical and the cancellous one surrounding the screw in this fixational system. The geomorphological characteristics of each porcine vertebra was measured directly with a micro-caliper. The bone mineral density of the specimens was also measured. Material with screw was holded in the fixed cross head of material testing system(Autograph E-10T). Testing force was applied and graph was obtained in the chart record. The results of the static pull-out tests in this study showed that there was a statistically-significant positive correlation between the screw diameter and the pull-out resistance(p < 0.05). The strength of the fixation did not actually increase as much as the insertion depth of the screw increased in these tests(r=0.457). In low-cycle fatigue tests, the increased number of cycles was required to clinical failure in the deeper-inserted crews. Considering the mechanical failure, a statistically-significant positive correlation between the failure cycle and the insertion depth was observed in both deep and shallow insertion groups(p < 0.05). Nonpressurized PMMA augmentation appeared to restore the ability of the screws to withstand pullout loading of the original value. On the effect of the cortical and cancellous bone, the most important factor was the cortical bone of the entrance near the screw.

Rectus sheath hematoma is an uncommon cause of acute abdomen, caused by a tear in deep epigastric vessels or its branches. It occurs spontaneously, after trauma or surgery. Rectus abdominus myonecrosis is a life threatening complication of rectus sheath hematoma. We report a case with fatal rectus sheath hematoma complicated by rectus abdominus myonecrosis caused by ischial fracture in a chronic active rheumatoid arthritis patient.

5.Postoperative Nausea and Vomiting after Strabismus Surgery in Pediatrics: A Comparison between VIMA with Sevoflurane and TIVA with Propofol .

BACKGROUND: Postoperative nausea and vomiting (PONV) is a common problem after general anesthesia. Compared with inhalational anesthetics, propofol has an antiemetic effect. We investigated the incidence of PONV after propofol and sevoflurane anesthesia for strabismus surgery in pediatric patients, and evaluated the effectiveness of ondansetron prophylaxis after sevoflurane anesthesia. METHODS: A healthy pediatric patients (n = 150) were divided into three groups. Group P (n = 50) received intravenous anesthesia with propofol; Group S (n = 50), inhalational anesthesia with sevoflurane; and Group S-O (n = 50), inhalational anesthesia with sevoflurane and 0.06 mg/kg (up to 4 mg) of ondansetron. The occurrence and intensity of PONV were recorded. RESULTS: The incidence of PONV after sevoflurane-ondansetron anesthesia did not differ from that after propofol anesthesia, but was significantly attenuated. The recurrence of PONV after sevoflurane-ondansetron was lower than after propofol and sevoflurane anesthesia. CONCLUSIONS: Sevoflurane anesthesia with ondansetron prophylaxis may be a good alternative to propofol in terms of avoiding PONV.

6.Parenchymal Neurocutaneous Melanosis in Association with Intraventricular Dermoid and Dandy-Walker Variant: A Case Report.

Neurocutaneous melanosis (NCM) is a rare congenital disease that is characterized by the presence of large or multiple congenital melanocytic nevi and melanotic lesions of the central nervous system. We report here on the CT and MR imaging findings of an unusual case of NCM that was associated with intraventricular dermoid and Dandy-Walker malformation.

7.Real-Time Measurement of 3-Dimensional Eye Movements by Videooculography.

BACKGROUND AND OBJECTIVES:In order to analyze and understand the oculomotor system completely, it is essential to measure horizontal, vertical, and torsional eye movements. We developed a new system of 3-dimensional videooculography that can accurately estimate the pupil center and measure torsional eye movement in real time while minimizing the errors caused by upper eyelid droop, eyelashes, corneal reflection, and eye blinking. MATERIALS AND METHOD:For accurate estimation of the center and diameter of the pupil, the edges of the pupil were searched by using a sector-form window and circle fitting was performed using the least square regression. Torsional eye movement was measured using polar cross-correlation technique with modification to lessen the errors produced by the change in pupil size. Calibrations were performed and to verify the proposed system, the linearity between the measured and actual eye movements was measured. Also, the results of the proposed system were compared with those of another commercialized system. RESULTS: The linearity between the measured and actual eye movements showed errors of less than 1%. The spatial resolutions of the system for horizontal, vertical, and torsional eye movements were 0.3, 0.4, and 0.2, respectively whereas the temporal resolution was 30 frames/sec. In comparison to the commercialized system, our system showed less artifact by eye blinking when measuring vertical eye movement. CONCLUSION:We proposed an algorithm and a system for measuring horizontal, vertical, and torsional eye movements which minimized the influence of partial eyelid closure, eyelashes, corneal reflections, and change in pupil size.

8.A Case of Blastic Plasmacytoid Dendritic Cell Neoplasm with Mutations in DNMT3A, TET2, SRSF2, and ATRX Genes

Blastic plasmacytoid dendritic cell neoplasm (BPDCN) is a rare and aggressive lymphoma with an overall incidence of 0.04 cases per 100,000 people. BPDCN is a hematopoietic clonal neoplasm that originates from plasmacytoid dendritic cell precursors. A 63-year-old man presented with multiple erythematous nodules over his whole body, including his face, trunk, and both upper and lower extremities that appeared 1 month ago. Skin biopsy showed diffuse dermal infiltration by monomorphic atypical lymphocytes with large, irregular nuclei and scant cytoplasms. Immunohistochemical staining was positive for CD4, CD56, and CD123. The karyotype test showed abnormalities in male chromosomes 47, XY, +8 [2]/46, and XY [25], and mutations in DNMT3A, TET2, SRSF2, and ATRX genes were identified in a next-generation sequencing (NGS)-based acute myeloid leukemia gene panel test. The patient was diagnosed with BPDCN and treated with a KALLA 1406 regimen; however, he died on the 17th day of treatment.
9.Laryngotracheal Separation for Intractable Aspiration in Severe Brain Damage

Laryngotracheal separation (LTS) is one of the treatment options for intractable aspiration. Here, we present two cases of LTS with successful prevention of intractable aspiration.The first case was a 60-year-old man with severe hypoxic brain damage caused by hydrogen sulfide (H2S) intoxication; he was admitted to our department for comprehensive rehabilitation. Frequent tracheal suction was required due to the excess secretion from saliva aspiration. Matters were complicated when he suffered frequent regurgitation and vomiting, which was followed by tracheal aspiration. In the second case, a 78-year-old man presented with a severe traumatic brain injury. He had experienced recurrent aspiration pneumonia, which was unresponsive to conventional therapy. In both cases, LTS was performed to prevent intractable aspiration pneumonia. Subsequent to the LTS procedure, videofluoroscopic swallowing study showed complete prevention of the aspiration in both patients. Moreover, tracheal secretion was reduced, and tracheal suction was no longer required. Post-surgery, there was no development of aspiration pneumonia during their hospital stays.
