4.Chronic Diarrhea in Infancy.

Diarrhea is an extremely common cause of morbidity in infancy. Occasionally it becomes protracted, leading to a vicious cycle of malabsorption, malnutrition and failure to thrive. Thirty nine infants of chronic diarrhea who had been admitted to the Department of pediatrics, Yonsei University College of Medicine for 3 years between September, 1989 and December 1992, were clinically studied and analized, the results were as follows: 1) The mean age of the patiens was 7.5+/-4.9 months. Symptoms started at 4.0+/-3.4 months of age and lasted for 3.6+/-4.1 months. 2) Among 39 cases, there were 16 cases of secondary lactase deficiency (41%), 9 cases of milk allergy(23%) and 3 cases of rotaviral enteritis(8%). Three patients (8%). had hepatitis and a patient had sepsis. 3) Incidence is much more common in formula-fed infants than brast-fed infants. 4) The growth status of the patients were poor; 54% of the patients by body weight and 39% by height fell into lower 10 percentile of normal distribution. Malnutrition accompanied the majority (82%) of the patients; 46% of mild form, 23% of moderate and 13% of severe form by Gomez classification. 5) Laboratory data on admission showed anemia in 21% of patients, hypoalbuminemia in 13% and hypocholester olemia in 31%. 6) Thirty one percent of the patients were improved with conservative tratment only. 33% of the patients got improved by low lactose milk feeding and milk withdrawl was done in 23% of the patients. Total parenteral nutrition was performed in 13% of the patients. All patients were cured with the treatment. With the above results, the most common cause of chronic diarrhea in infancy is postinfectious disaccharidase deficiency. followed by milk allergy. Low lactose milk feeding and milk withdrawl can improve the infants from chronic diarrhea.

7.Statistical Survey on the Amputees

We have made a statistical survey on 2441 amputated limbs of 2150 patients who were treated at the Depart- ment of Orthopedic Surgery and who were prescribed the prosthetics at the Prosthetic Center of Veterans Hospital from Mar. 1972 to Feb. 1982. 1. The peak incidence was in the age group from 21 to 30 years with 1546 cases (63.3%). Of 2150 patients only 9 patients were female. 2. The main causes of amputation were trauma with 1901 cases (77.9%), vascular insufficiency with 326 cases (13.4%) and chronic osteomyelitis with 157 cases (6.4%). And the most fre#quent cause of traumatic amputation was explosive injury with 1080 cases (56.8%). 3. Of the 2441 amputated limbs, 1756 cases (71.91°) were at the lower extremity and the most frequent site was below the knee with 1000 cases (57%). 4. Minor limb amputation was performed on 326 cases (13.4%). 5. Multiple limb amputation was performed on 255 patients (11.9%) and of these 12 patients were amputated on 4 extremities. 6. Reoperation was performed on 420 cases (17.2%). It was most prevalent in the lower extremity with 365 cases (86.9%) and their main cause was neuroma with 171 cases (40.7%). The most frequent site was below the knee with 240 cases (57.2%).

10.A Study on the Usefulness of Birth Registration Data in Rural Korea.

The onprovement of civil registration reguires continuous study rather than periodic efforts. More and better statistics, however, are urgently required to formulate development programs and planning, Data obtainable from the civil registration are usually marred by errors of omission which are difficult to correct. This study aimed at finding out the problems occurring when a set of crude birth registration data in a rural area is used. Data Sources of this study are : 1) For birth registration : government birth registration records obtained from myun office and other government offices. 2) for the actual number of births : birth and child records from the Kang Wha Community Health Project. The study area is Sunwon Myun and Naega Myun in Kang Wha Gun, Gyunggido. The reference period for the accumulated data is one full year : Jan. 1st 1975 - Dec. 31st 1975. Major findings are as follows : If the number of registered birth is compared with the actual number of births which occurred in the target area, the former os far greater than the latter. The general assumption usually is , that the actual number of exceeds the registered number of birth in Korea. The observation from this specific study in this specific target area, shows the opposite trend. The number of births which actually occurred during the year of 1975 in the study area namely 256. The difference comes mainly from the fact that many cases of births from other areas were registered in the target area. In other words birth is not registered where it occurred but where the permanent residence address is. Among 550 births registered in the target area 66% did not occur in the target area. Only one third of all registered births were registered within the legal period for birth registration which is 2 weeks. 34% of the registered births actually occurred in 1974, but were registered in 1975. In 55% of the cases a difference was observed between the actual data of birth and the registered data of birth. From the 256 births which occurred in the target area, only 153 births (59%) were registered at the myun office and the remaining 130 births (41%) were not resistered there in the year of study. 6% of the 550 cases listed as registered have no separate registration sheets. Nevertheless, they definitely have been registered in the birth list at the myun office. 3% of the 550 cases are not recorded in this list but have a separate registration sheet at the myun offices. In conclusion, birth registration data have many errors and problems. Their usefulness as a source for vital and other statistics should be reconsidered. A series of sound methological studies will be necessary to establish their actual usefulness. A continuous and permanent compulsory system of birth recording is needed.
