1.A Study of the Prevalence of Contact Sensitization to Rhus and Ginkgo Antigens.

Recently, people living in a rapidly industrializing country such as Korea seems to have less opportunity to be exposed to Rhus and Ginkgo trees than before, which may cause a lower prevalence of sensitivity to such allergens than those reported in the past. To prove the prevalence of sensitivity in Koreans, we have tested 0. 002g and 0.0l%, Urushiols and 10% Ginkgo leaves produced by Torii company to 72 atients with or suggesting contact dermatitis. The results were as follows: 1) Twelve of 72 patients(13, 9%) showed positive reactions and two patients(2. 8 %) showed active sensitization to both 0. 002% and 0. 0l% Urushiols. Only one patient(]. 4%) reacted to 10g Ginkgo leaves, and she was also reacted to the Urushiols. 2) Age and sex distribution of the positive reactors were none under thirty, 2 in thirties, 3 in forties, 4 in fifties, one over sixty, and seven female patients outnumber three male patients. 3) Out of 5 patients having past history cf contact dermatitis to Rhus tree, only patients showed positive reactions. Out of 67 patients without past history of allergy to Rhus, 8(ll, 9%) showed positive reactions and 59(88. 1%) showed negative reactions.

2.A Comparative Study of Commercial Antigens and Fresh Antigens in Atopic Dermatitis and Chronic Urticaria Patients with Fish and Shellfish Antigens.

This study was undertaken to observe the difference of patch test and prick test positive reactions in three groups, 20 atopic dermatitis patients, 20 chronic urticaria patients, and 20 normal controls, Twenty commercial antigens (ToriiCo.) and twenty three frozen and boiled antigens of fishes and shellfishes were used as test materials. The results were as follows: 1. No positive reaction was observed in all sixty subjects in patch test. 2 In prick test, difference in reactivity was observed among atopic dermatitis (14. 7%), chronic urticaria (8.7%), and normal control(1. 5%) 3. There was significant difference in reactivity between Torii and frozen antigens in atopic dermatitis group, but no significance in chronic urticaria and control groups. 4. Boiling altered the reactivity in tested materials in all three groups of subjects singificantly. 5. Patients with past or family history of allergic diseases showed double fold higher reactivity tban patients without any history.

4.Free Second Toe-to-Index Transplantation for Index Reconstruction

The development of a safe method of transfer of the greater toe for thumb reconstruction, using dorsalis pedis artery and the great saphenous vein, suggested the possibility of one stage microvascular transfer of the second toe for reconstruction of surgery. Based upon our part experience with microsurgical experiments, limb transplantation and toe-to-thumb transplantation since 1975, we accomplished the second toe-to-index transplantation done in Korea on October 24, 1979, and succeeded in one stage second toe-to-index transplantation in two cases. Following results were obtained. I. Free second toe-to index transplantation on making a index is the most excellent method of index reconstruction with results in minimum functional and cosmetic impairment. 2. Technically, skin incision, isolation of blood vessels and flexor tendon in preparation for transplantation of second toe is more difficult than that of greater toe. 3. The specialized microsurgical training with skillful technique is essential for success.

5.Effects of c -Fos Immunoreactivity on Central Nervous System after Administration of Aspirin.

Aspirin is one of the popular non -steroid anti -inflammatory drugs used in the management of pain. This study was performed to investigate the effects of aspirin on c -Fos expression in rat CNS after inducing somatic pain with formalin. Male S.D. rats were injected subcutaneously with 0.1 ml of 5% formalin in the plantar surface of right hindpaw. For experimental group, aspirin was administered orally before injection of formalin. Asprin -untreated group was utilized as the control group. Rats were sacrificed at 0.5, 1, 2, 6 and 24 hours after formalin injection. Rat brains were removed and sliced in rat brain matrix. Brain slices were coronally sectioned at interaural 5.70 ~6.70 mm. Serial sections were immunohisto-chemically reacted with polyclonal c -Fos antibody. The numbers of c -Fos protein immunoreactive neurons in the cingulate cortex, primary somatosensory area, and hippocampus were counted and analyzed statistically with Mann - Whitney U test. Results were as follows: 1. Higher numbers of c -Fos immunoreactive neurons were found in the cingulate cortex, primary somatosensory area and hippocampus. 2. Both aspirin -treated and -untreated groups, numbers of c -Fos immunoreactive neurons were significantly higher all time points than formalin -untreated group, which peacked at 2 hours. 3. The numbers of c -Fos immunoreactive neuron of the aspirin -treated group were less compared to the aspirin - untreated group at each time point. In conclusion, these results provide some basic knowledge in understanding the mechanism and control of formalin - induced somatic pain.

6.Clinical Analysis of Cranial Nerve Injuries in Craniocerebral Trauma.

The clinical analysis of cranial nerve injuries was performed on 435 cases with cranoicrerbral trauma. This prospective study included the correlation between cranial nerve injuries and risk factors such as intracranial hematoma, initial Glasgow Coma Scale(GCS) score, pneumocephalus, and other combined injuries. The results were revealed as follows : 1) 133 cranial nerve injuries(on 97 patients) were noted among 435 craniocerebral trauma victims(97/435=22.2%). 2) The order of frequent cranial nerve injuries was facial nerve(7.3%), olfactory nerve(6.9%), oculomotor nerve(4.4%), abducens nerve(3.9%), optic nerve(3.2%), etc. 3) Bilateral involvment of cranial nerve injuries was noted in 16.5%(22/133). 4) The incidence of immediate onset of cranial nerve injuries was 66.9%(89/133). 5) The incidence of cranial nerve injuries was significantly high in patients with pneumocephalus and low initial GCS score. 6) The functional recovery of injured cranial nerve within 3 months was noted in 30.1%(40/133).

7.Clinical Analysis of Interhemispheric Subdural Hemorrhage and Tentorial Hemorrhage.

A propecive analysis of 50 patients with acute interhemisphric subdural hemorrhage and tentorial hemorrhage, an unusual pattern of acute subdural hematoma, who were managed in a uniform way was analyzed to related outcome to hemorrhagic site, initial Glasgow Coma Scale(GCS) and combined injuries. The incidence of acute interhemisphric subdural hemorrhage(ISH) and tentoria hemorrhage(TH) after head trauma was 3.83%, 50 cases among 1303 head injured cases. And 80% of the above hemorrhage disapperaed within two weeks after trauma. There was no significant relationship between feature of hemorrhage and intial GCS(P>0.05), but there was highly significant relationship between initial GCS and Glasgow Outcome Scale(GOS) (p<0.001). There noted significant relationship between initial combined injury and GOS(p<0.01), and also brainstem injury and GOS(p<0.001).

8.Direct detection of hemophilia B F9 gene mutation using multiplex PCR and conformation sensitive gel electrophoresis.

PURPOSE: The F9 gene is known to be the causative gene for hemophilia B, but unfortunately the detection rate for restriction fragment length polymorphism-based linkage analysis is only 55.6%. Direct DNA sequencing can detect 98% of mutations, but this alternative procedure is very costly. Here, we conducted multiplex polymerase chain reactions (PCRs) and conformation sensitive gel electrophoresis (CSGE) to perform a screened DNA sequencing for the F9 gene, and we compared the results with direct sequencing in terms of accuracy, cost, simplicity, and time consumption. METHODS: A total of 27 unrelated hemophilia B patients were enrolled. Direct DNA sequencing was performed for 27 patients by a separate institute, and multiplex PCR-CSGE screened sequencing was done in our laboratory. Results of the direct DNA sequencing were used as a reference, to which the results of the multiplex PCR-CSGE screened sequencing were compared. For the patients whose mutation was not detected by the 2 methods, multiplex ligation-dependent probe amplification (MLPA) was conducted. RESULTS: With direct sequencing, the mutations could be identified from 26 patients (96.3%), whereas for multiplex PCR-CSGE screened sequencing, the mutations could be detected in 23 (85.2%). One patient's mutation was identified by MLPA. A total of 21 different mutations were found among the 27 patients. CONCLUSION: Multiplex PCR-CSGE screened DNA sequencing detected 88.9% of mutations and reduced costs by 55.7% compared with direct DNA sequencing. However, it was more labor-intensive and time-consuming.

9.Histology of Nevus Flammeus Following Argon Laser Treatment.

We observed the histopathological findings in 16 patients with nevus flammeus according to the time sequence after argon laser therapy. The results were as follows : 1. Immediately after treatment There was nonspecific damage to the epidermis, upper part of the dermis and upper portion of the pilosebaceous unit to a depth of 0.4~0.5mm depth. The specific damage to the vessels, which showed thrombi formation and partial disruption of the vessel walls was seen to a depth of 0.8~0.9mm. 2. One week after treatment Crust was formed and the epidermis regenerated almost completely. The thrombi still persisted and there was partial disappearance of the vessel walls. 3. Four to 10 months after treatment The epidermis appeared normal except the rete ridge flattening. The upper dermis was diffusely fibrotic, with relatively few residual vessels, many of them with slit-like contour.

10.Immunocytochemical Study on Distribution and Morphology of Dendritic Cells in the Tongue during Development and Growth of the Rat.

Activation of T cells for an immune response requires the participation of antigen presenting cells (APC) that express class II major histocompatibility complex gene products on their surface. Until recently, the macrophages have been considered to be the prime candidates for this role, but it is now recognized that other cells, including dendritic cells, B cells, activated T cells and endothelial cells, can present antigen effectively. Particularly, among them, dendritic cells (DC) are considered to be very efficient APC for various T -cell dependent immune responses in comparison with other types of APC. Nonlymphoid dendritic cells including Langerhans cells and interstitial dendritic cells strongly express the MHC class II products and have characteristic dendritic morphology. As far as we know, there is no study on the ontogeny of MHC class II -immunoreactive dendritic cells in the rat tongue. The aim of the present study is to investigate the ontogeny and morphological characterization of dendritic cells in the tongue of growing and developing rats. The distribution and morphology of the dendritic cells in the rat tongue were studied from the fetal 15 -day until 180 days after birth by means of immunocytochemical methods using anti -rat MHC class II monoclonal antibodies. The results were as follows: 1. MHC class II -immunoreactive dendritic cells were first found in the muscle layer of 17 -day fetus, and in the epithelium and lamina propria of the tongue at birth. 2. The number of MHC class II -immunoreactive dendritic cells was gradually increased with age, particularly, in the epithelium at 14th day, in the lamina propria at 14th day, and in the muscle layer at 21st day after birth. 3. Numbers of MHC class II -immunoreactive dendritic cells were higher in the dorsal part than in ventral part of the tongue at all developmental stages. Especially, dendritic cells were twice higher numbers in the epithelium, a little higher numbers in the lamina propria and almost same numbers in the muscle layer. 4. With age, shapes of MHC class II -immunoreactive dendritic cells were changed from round to dendritic and aggregated together. In conclusion, the above results suggest that the increases of the number and the changes of the morphology in MHC class II -immunoreactive dendritic cells, with age may influence on effects of cell -mediated immune responses.
