1.Radiological evaluation of cortical rim sign of the kidney

The rim sign in renovascular compromise may be defined as a thin nephrographic rim outlinning a kidney with another wise faint nephrogram. Renal infarction with gelform and blood clots was induced experimentally in 5 dogsand high dose IVP and CT were performed to evaluate renal cortical rim sign after renal infarction. One expireddue to anesthetic technical failure, and one was embolized partially. So we could be obtain the result on 3 dogs.The reults were as follows; 1. The high dose IVP films show cortical rim sign on 24 hrs and 72 hrs films in twodogs. The CT films show rim sign in 3 dogs. 2. The rim sign may be visualized, if the renal artery is occuludedenough to drop perfusion presssure below the authoregulatory range. 3. The high dose IVP and early nephrogramphase are necessary for the rim sign.

2.Repair of Neglected Rupture of the Achilles Tendon using V-Y Tendinous Flap

The 6 patients with neglected rupture of the Achilles tendon were treated by end to end anastomosis using the V-Y tendinous flap originated by Abraham in 1975. A tendinous flap was developed over the tendon by making an inverted V incision, which was then repaired in a Y fashion. The postoperative results were satisfactory with no functional disability of the ankle and with recovery of muscle strength for periods of 8 months to 15 months, but slight limited dorsiflexion of the ankle in two cases were noticed. The advantages of this method are: 1. Optimum functional recovery. 2. Recovery of muscle strength and tendon contour. 3. Prevention of late stretching or possible rupture of the healed tendon at the rupture site.

3.A Survey for Methicillin

Nine strains (6%) were confirmed as resistant (MIC=20ug/ml) to methicillin (MT) among 150 isolates of staphylococcus aureus. These MT-resistant staphylococcus aureus (MRSA) strains were also turned out to be more less resistant to other antibiotics tested, such as penicillin, chloramphenicol, tetracycline, and gentamicin except for cephaloridine. It was noteworthy, however, that strain MRSA 5 showed a relatively susceptible attitude toward most of the antibiotics tested, while being extremely resistant to MT only. Salt (NaC1) containing (5-3%) media did not have any significant effect on the degree of MT-resistance of MRSA strains. There was, however, such a tendency as that by the addition of salt the degree of MT-resistance was lowered in the highly MT-resistant strains while enhanced in MT-susceptible ones. It was confirmed that penicillinase did not have any effect on the MT-resistance while most of the penicillin- resistant strains produced the enzyme.

5.Airway Obstrauction during Endotracheal Anestheaia - Case report.

Acute airway obstruction during endotracheal anesthesia is embarrassing and critical situation which requires early diagnosis and immediate management. The authors experienced extremely high inspiratory resistance and declared expiration time during endotracheal anesthesia for right pneumonectemy of lung cancer in the left decubitus position. A foreign material was suspected to be dislodged in the trachea distal to the endotrac-heal tube. But the surgical procedure did not allow to change the tube for examination. Ventilation was managed by rotating and moving the endotracheal tube frequently. After extubation, the patient expectorated a polypoid mass and the cause of the obstru-ction was proved to be a result of the detached and dislodged tumor mass of the lung, in diameter 3X1.5 cm.

8.Status Epilepticus in Adult Hospitalizde Patients: Cause and Clinical Outcome.

Background AND PURPOSE: Status epilepticus (SE) is one of the major neurological emergency that requires immediate treatment to avoid significant morbidity and mortality. Thus, understanding the cause, features and prognosis of SE is important for the evaluation and treatment of this condition. We retrospectively reviewed the possible cause and clinical outcome of adult patients treated for SE at the Hallym Univrsity Hospital from 1994 to 1998. METHODS: For the identification of patients, we searched the data bank for patients meetinf criteria of SE who were 18 year or older, and their medical records were reviewed. We also investigated the relationships between cause, response to anticonvulsant therapy and short-term clinical outcome. RESULTS: The selected 127 patients wer 84 males and 43 females, aged 18 to 85 yeats (meen age: 49.5 years). The possible etiologies of SE were withdrawal of AED (n=27, 21.3%), anoxia (n=22, 17.3%), CNS infection (n=20, 15.7%), stroke (n=16, 12.6%), alcohol-related (n=15, 11.8%), metabolic (n=8, 6.3%), unknown (n=7, 5.5%), drug inroxicatio (n=5, 3.9%), trauma (n=4, 3.2%) and cerebral tumor (n=3, 2.4%). in 77 patients (n=77, 60.6%), SE was successfully aborted with first-line therapy, which usually included diazepam with or without phenytoin. The food responders to AEDs occurred in patients with AED withdrawal, alcohol-related, stroke, unknown, and trauma, The poor response related to anoxia, drug intoxication and CNS infection. Seventy nine patients (62.2%) had food outcome, but nineteen patients (14.9) were died. Anoxia, drug intoxication, CNS infection and metabolic abnormalities were associated with particularly poor outcome compared with other etiologies. CONCLUSION: This study indicates that the etiology of SE may help predict both the intial response to drug therapy and short-term clinical outcome.

9.Molecular Evolution and Identification of Yersinia Species by 16S rDNA Analysis.

Sequence information from the ribosomal RNA molecule can be used as a molecular clock. Therefore, 16S rRNA sequences have become a widely used tool in bacterial systematics, bacterial typing and studies of the composition of micro-organisms in environmental samples. Consequently, the databases are continuously updated with new 16S rRNA sequence information from different species. This in turn facilitates the identification of new isolates, which is important for the diagnosis of various infectious diseases in research. To development differential identification technique in the Yersinia species, the 16S rDNA for total 17 serotypes of Y. pseudotuberculosis type strains and one of Y. enterocolitica wild strain were amplified by polymerase chain reaction and cloned into pGEM-T vector, partially sequenced and analyzed. The 16S rDNA nucleotide sequence homologies among Yersinia strains were ranged 100% to 93.6%. Phylogenetic tree generated from 16S rDNA sequencing data showed different phyletic line that could be readily separated from each serotypes and other enteric bacterium.

10.Psychopathology, Family Support System and Quality of Life in Patients with Chronic Renal Failure.

PURPOSE: The purpose of this study is to evaluate the relationship among psychopathology, family support system and quality of life in patients treated with hemodialysis, peritoneal dialysis and kidney transplantation and provide useful information for therapeutic approach to patients with chronic renal failure. METHODS: The subjects of the present study are 43 patients who have been treated with hemodialysis, 53 patients treated with peritoneal dialysis and 34 patients treated by renal transplantation. Beck Depression Inventory (BDI), Symptom Checklist-90- Revised (SCL-90-R) for psychopathology, Family Adhesion Scale, Family Adaptability and Cohesion Evaluation Scale II (FACES II) for family support system, and World Health Organization Quality of Life Assessment Instrument for quality of life were administered to subjects. To examine these, ANOVAs and Pearson correlation analysis were conducted. RESULTS: The patients who have been treated with hemodialysis or peritoneal dialysis showed higher scores of depression in BDI and SCL-90-R than patients treated with renal transplantation. Depression correlated with family cohesion & adaptability, negatively. Hostility correlated with family adhesion & cohesion, negatively. Somatization, obsessive-compulsive, interpersonal sensitivity, anxiety, phobic anxiety and psychoticism correlated with quality of life, negatively. CONCLUSION: The main findings of this study indicate that psychopathology significantly related with family support system and quality of life of the patients with chronic renal failure. These results suggest that the psychosocial intervention are necessary to promote the mental health, strengthen the family support system, and improve the quality of life in the patients with chronic renal failure.
