4.Diagnostic Analysis for Meniscal Lesions of the Knees

We assessed the accuracy of clinical evaluation, arthrography and arthroscopy in the diagnosis of meniscal lesions in ninty knees in which arthrotomy was performed for disabling symptoms after evaluation by these three methods. At surgery, ninty-two menisci were removed, of which seventy-two were abnormal and twenty were normal. In these ninty-two menisci, correct diagnostic rate was made clinically 78%, arthrographically 70% and arthroscopically 86%. Arthrographic diagnosis was least accurate for lesions of the lateral meniscus and arthroscopic diagnosis was least accurate for lesions of the posterior horn of the medial meniscus and peripheral lesions. Based on this study, it was concluded that both the arthrography and arthroscopy are valuable technique for diagnosis of meniscal lesions.

8.A Experimental Study of PTEN (Phosphatase and Tensin) Role in Mesothelioma.

BACKGROUND: Conventional treatment for mesothelioma is largely ineffective. We evaluated the novel approach of adenoviral gene transfection of PTEN gene in mesothelioma cancer cell lines, inflammatory and epithelial subtype, which are sensitive to adenoviral p53. MATERIAL AND METHOD: Binary adenoviral PTEN and LacZ (Ad/GT-LacZ and Ad/GV16) vectors were used for transduction of the mesothelioma cell lines, REN (p53 sensitive). Protein levels were determined by Western blotting assay. Apoptosis was assessed by fluorescence-activated cell sorter analysis of subdiploid populations. Cell viability was determined with the XTT assay. Statistical analysis was performed with analysis of variance and the Student t test. RESULT: 72 hours after the treatment of adenoviral PTEN gene, cell killing were 32.9% for REN compared to control cell (2.5%) at MOI of 20. Also we observed the over-expression of proapoptotic protein, bax and decreased expression of bcl-2 protein in REN cells. But the expression of BCL-xl, Bak, Bad proteins were not altered. CONCLUSION: Adenovirus Pten-mediated overexpression of the Bax gene induces apoptosis and decreased cellular viability in p53-sensitive mesothelioma cells. These data suggest that the transfection of PTEN gene may represent a alternative gene therapy strategy to treat mesothelioma.

10.Clinical Observation of Pyogenic Osteomyelitis

Pyogenic osteomyelitis is a frequent and difficult disease in treatment in orthopedic surgery. The various manifestation of the disease related to the causative organisms, the sites of involvement, the pathogenesis and the clinical course tend to give a bad prognosis after treatment. This study consists of three hundred and twenty four cases of pyogenic osteomyelitis which are studied at the Department of Orthopedic Surgery, Yonsei University hospital for 7 years and 8 months from January 1973 to August 1979. The results are as follows: 1. The frequency of the disease was highest in 1977 but not significant. 2. Femur and tibia were most frequently involved. (65.4%). 3. Age distribution showed the highest incidence between 6 and 15 years of age. (60.6%) Sex ratio was 2(M):1(F) 4. The causative organism was mostly Staphylococcus. (80.5%) The coagulase(+) Staphylococcus showed the highest sensitivity to cefamezine (88.8%) and the highest resistance to penicillin. (89.7%) 5. The treatment had been carried out case by case with variable methods. Author considered the best method of treatment was early decompression and massive antibiotic therapy in acute osteomyelitis, and saucerization with closed irrigation system in chronic osteomyelitis.
