2.Dermatophytoses in Pusan Area.

This study aims to investigate the recent clinical and mycological trends of dermatophytoses in Pusan area. The number of patients with dermatophytoses diagnosed by KOH examination and culture at Maryknoll Hospital was 1, 213 during past 2 years from November 1982 to October 1984. The incidence rate of dermatopbytoses was 9. 8% out of 12,377 cases of out patients in total. Tinea pedis (33 .7%,) was the most frequent diagnosis, followed by tinea cruris, tinea unguium, tinea corporis, tinea faciale, tinea manus and tinea capitis, in the order of decreasing frequency. The majority of cases were found in the age group from 20 to 39 4, The over all ratio of male to female patients was ] 2: 1 5 The positive rate in KOH examination was 96. 2A, and it was especially low in the areas of scalp, face and toe webs. The positive culture rate was 55, 9g, and it was higher in the covered area than in the exposed area. 6 678 strains out of the 1, 213 patients were isolated and identified. The species isolated were, in the order of decreasing frequency, Trichophyton rubrum (558 strains, 82.3% of total organisms isolated), Tn'chophyton mentagrophytes (55,8, 1 Microsporum canis (48, 7. 1g) Epidermophyton floccosum (12, 1 8g) and Microsporum gypseum (5, 0. 7gp) Trichophyton rubrum was the major causative organism in all the types of dermatophytoses except tinea capitis and tinea faciale. In tinea capitis, Microsporum canis was the main causative organism, and Trichophyton ferrugineum was not isolated, at all, which was a specific point, compared with the past tendency. In tinea faciale, zoophilic organisms were mainly isolated.

3.A Case of Early Lepromatous Leprosy Showing Unusual Skin Manifestations.

The skin lesions of leprosy have a great similarity to various other lesions. Fasal has called it the great imitator. Sometimes these skin lesions can b misdiagnosed because the physician does not consider leprosy. We reported a case of early lepromatous leprosy in 42 year old male. His skin lesions were similar to secondary syphilid. There were no sensory changes on the skin lesions and no enlargement of the peripheral nerves. Numerous AFB(+) bacilli from the skin smear confirmed our diagnosis of leprosy.

4.Hyperinfection of strongyloides stercoralis.

Strongylodiasis is universal in distribution but is most abundant in countries with a tropical climate. Although infestation by Strongyloides stercoralis is usually limited to the intestines, dessemination of this helminth in debilitated host can be lead to death with various clinical disorders. characterized by profound malabsorption, diarrhea, electrolyte imbalance, gram negative or opportunistic fungal sepsis, coma and death. Cell-mediated immunity contributing significantly to the control of helminthic infections, may be suppressed by carcinoma, immunosuppressive chemotherapy and use of corticosteroids. Diagnosis of Strongyloidiasis is achieved by an examination of samples of feces, duodenal aspirates and sputum of patients for Strongyloides stercoralis. Treatment of strongyloidiasis is twofold : correction of the immunosuppressive state by withdrawal of immunosuppressive drug, if possible, and vigorous treatment with thiabendazole. Testing for strongyloidiasis is especially recommanded before treating a patients should be monitored for infection by Strongyloides stercoralis and other opportunistic infection. We are reporting a case patient with Strongyloides stercoralis hyperinfection and pulmonary tuberculosis who had been. used corticosteroid for persisting polyarthritis.

5.A Case of Multiple Lentigines Syndrome.

We report a case of multiple leatiginea syndrome in an 8 year old boy. He had numeroua lentigines acattered over his face, trunk, buttock and thlghe, and eome larger black macular leeians on the trunk and thighs. Gn phyaiaal examinatlon, he wae well developed but he had ocular hyperteloriem. Chest roentgenogram showed hypertrophy of both ventricles. Electrocardiogram and audiogram revealed conduction defects and severe sensorineural deafness, reepectively. Blopsy af dark brown lesion from the back showed the histopathologlc pattern of lentigo.

6.Evaluation of Methylprednisolone Dosing Regimen in Experimental Diffuse Brain Injury.

The present study was undertaken to examine the ability of a single large intraperitoneal dose of methylprednisolone(15, 30, 40, 60mg/kg) to modify the evolution of diffuse brain injury(DBI) in rats undergoing a contusion injury of 400gm-1m. Twenty five Sprague-Dawley rats were divided to five groups. Group 1 was not treated after cranial impact as control. Rats in group 2, 3, 4, 5 were treated with 15-mg/kg, 30-mg/kg, 40-mg/kg, 60-mg/kg methylprednisolone dose at 1 hour after injury, respectively. After 24 hours, these rats were sacrificed, and the water content of the whole brain were measured. When the water content was assumed as the criterion of brain edema, there was significant difference between control group(71.03+/-.42%) and group 2(67.34+/-.07%), group 3(66.43+/-.36%), group 4(64.52+/-.11%)(p<0.01). On the other hand, there was no significance between control group(71.03+/-.42%) and group 5(69.74+/-.53%) (p>0.01). This study shows that megadose of methylprednisolone in 15, 30, or 40mg/kg reduces the brain edema, and the its effect is biphasic in that the 60-mg/kg methylprednisolone dose is ineffective.

7.Patient Satisfaction after Arthroscopic Repair of Acetabular Labral Tears.

BACKGROUND: Acetabular labral tear is a main cause of hip pain and disability, often requiring surgical treatment. Improvements of hip arthroscopic technique have produced positive outcomes after labral repair with arthroscopy. The purpose of this study was to determine clinical outcomes and patient satisfaction after arthroscopic repair of acetabular labral tear. METHODS: We interviewed 21 patients (10 men and 11 women; mean age, 36 years [range, 22 to 57 years]) with acetabular labral tears that had been repaired arthroscopically in terms of satisfaction of the procedure. In addition, clinical outcome was assessed using visual analog scale (VAS) score, University of California, Los Angeles (UCLA) activity, Western Ontario and McMaster Universities (WOMAC) osteoarthritis index, and Harris hip score, and radiologic outcome was assessed using serial radiography. The patients were followed for 24-50 months. RESULTS: The mean Harris hip score was 73 points (range, 64 to 84 points) preoperatively and 83 points (range, 66 to 95 points) postoperatively. Fifteen hips (71%) were rated excellent and good. The mean WOMAC osteoarthritis index and VAS scores were improved at final follow-up. UCLA activity at the latest follow-up improved in 16 patients. The Tonnis grade of osteoarthritis at the latest follow-up did not change in all patients. Eighteen of the patients (86%) were satisfied with the procedure. CONCLUSIONS: High rate of satisfaction after arthroscopic repair of acetabular labral tears is an encouraging outcome. Arthroscopic treatment of labral tears might be a useful technique in patients with hip pathologies, such as femoroacetabular impingement with labral tears.

8.Effect of Rosiglitazone on Myocardial Ischemia-Reperfusion Injury in Rat Heart.

This study was undertaken to evaluate whether peroxisome proliferator-activated-receptor-gamma (PPAR-gamma) agonist-rosiglitazone (ROSI) induces postischemic functional recovery in Langendorf heart model. Hearts isolated from normal rats were subjected to 20 min of normoxia or 25 min zero-flow ischemia followed by 50 min reperfusion. In this acute protocol, ROSI (20 microgram/ml) administered 10 min before ischemia had no effect on hemodynamic cardiac function, but had protective effect on lipid peroxidation in in vitro experiments. In chronic protocol in which ROSI was given by daily gavage (4 mg/kg) for three consecutive days, ROSI could not prevent the hemodynamic alteration on cardiac performance, but has protective effect on the activity of superoxide dismutase (SOD). There was no significant difference in the contents of reduced glutathione (GSH) and catalase activity between ischemia-reperfusion (IR) and ROSI treated IR hearts. Although ROSI had no effect on hemodynamic factor, it had effect on antioxidant activity. Our results indicate that ROSI provides partial beneficial effects by inhibiting lipid peroxidation and/or recovering normal level of SOD activity in the ischemic reperfused heart.

9.What is the Role of Coordinators in the Secondary Fracture Prevention Program?

Background:
The purpose of this study is to search for reports on the clinical effectiveness of FLS being implemented worldwide through the systematic review, analyze the roles of coordinators in each study, and provide basic data for the development of future coordinator education programs.
Methods:
A systematic search of the literature using the Medline, PubMed, and EMBASE databases and the Cochrane Library was conducted for using the following keywords: ‘osteoporosis’ AND ‘fractures’ AND ’secondary prevention’. Finally, 65 studies are included in this study.
Results:
At the coordinator-based fracture liaison service (FLS) center, the coordinator (often a nurse) acts as a central player in the establishing of patient connections, orthopedic surgeons, radiologists, and attending physicians. Coordinators help bridge the nursing gap by supporting identification, investigation, initiation of treatment, and patient follow-up. Medics has opened the way to effectively manage patients at high risk of developing another fracture. In addition, nurses are in a unique and important role as nurses responsible for enhancing their daily lives by building relationships with patients and families.
Conclusions
The coordinator in the FLS program plays an important role in the multidisciplinary management of vulnerable fractures, as well as in the diagnosis and treatment of osteoporosis and in maintaining continuity of treatment. In the future, the broader role of coordinators should be systematically organized and developed into accredited educational programs.
10.Induced Hypertension Therapy for Postoperative Cerebral Ischemia in Aneurysm Surgery.

The purpose of this study is to clarify the effectiveness and the indications of the dopamine induced hypertension therapy(IHT) in the treatment of symptomatic cerebral ischemia secondary to aneurysm surgery. Eight patients suffering from ischemic complication of postoperative vasospasm were treated with dopamine induced hypertension therapy and intravascular volume expansion. All of patients underwent CT scan in order to ascertain if their neurological deteriorations were due to vasospasm. The criteria of the indication of IHT are as follows : 1) ischemic symptoms were progressively advanced, 2) there is no hematoma or infarction on CT scan, 3) there is no responses to ischemic symptoms with hyperventilation, intravascular volume expansion, 4) there is no hypovolemia. The blood pressure was raised to 30% above the mean arterial pressure that required for reversal of the ischemic deficit with dopamine induced hypertension and increasing the intravascular volume. In seven of eight patient, a marked improvement in ischemic symptoms occurred after raising blood pressure, and blood volume. In 4 cases, the level of consciousness and neurological deficits were improved within 12 hours after IHT started. IHT is expected to restore the brain tissue from ischemia by increasing blood flow through the arteries of vasospasm and collateral circulation.
