2.The Properties of Glycoconjugates of Apoptotic Cell in the Rat Accessory Sex Gland after Castration.

The present study was carried out to investigate the structure and the glycoconjugate properties of the apoptotic cells in the rat accessory sex glands (prostate gland, seminal vesicle and coagulating gland) after castration. After identifying the apoptotic cells by TUNEL method, the structure and the glycoconjugate property of these cells was observed with the light and electron microscopes, using nine kinds of lectins (PSA, UEA I, GSL I B4, ECL, DBA, SBA, DSL, GSL II or WGA). And also, the glycoconjugate properties of the apoptotic cells originated from the prostate epithelial cell culture were investigated. The apoptotic cells have condensed and fragmented nucleus, and also have degenerated and condensed cytoplasm. The numerous apoptotic cells were observed in the accessory sex gland at 2nd day after castration, and in the cultured prostate epithelial cell which were grown in the medium lacking growth factors for 12 hours. The cytoplasm and nucleus of the apoptotic cells induced by castration showed positive reaction with PSA, UEA I, SBA, DSL and WGA, which indicate apoptotic cells have the glycoconjugates contain alpha-D-mannose, alpha-L-fucose, terminal beta-N-acetyl-D-galactosamine and internal beta-1, 4-N-acetyl-D-glucosamine oligomer. But normal epithelial cells of the accessory sex glands showed different glycoconjugate property from the apoptotic cells. These facts suggest that the glycoconjugate property was changed by apoptosis. The apoptotic cells induced by culture showed positive reactions with the nine different lectins.

3.A Case of Double Pylorus.

The double pylorus is a rare condition consisting of a double communication between gastric antrum and duodenal bulb. Some investigators postulate that the doubling of the pyloric channel is a congenital phenornenon, but others believe that it is an acquired lesion. A 72 year-old-man was admitted to this hospital because of epigastric pain for 1 month. Upper G-I series revealed thickened rnucosal folds of pylorus and duodenal bulb and dilated, deformed duodenal bulb filled with barium materials. Endoscopic findings also showed two ovoid large openings of the pyloric channel divided by smooth thickened septum and multiple gastroduodenal ulcers. We thought that this case was an acquired lesion. The relevant literatures on the subject were reviewed.

4.MR Findings of Sturge-Weber Syndrome : Emphasis on Vascular Abnormality.

PURPOSE: To observe MR findings of vascular abnormality in Sturge-Weber syndrome and to determine the value of MRI in diagnosis. MATERIALS AND METHODS: Ten patients with Sturge-Weber syndrome (age : 3 months-32 years)were evaluated by MR imaging ; in six and four cases, respectively, the results were correlated with those of CT and angiography. We retrospectively analysed changes in the cortical vein and deep venous system, including the medullary and subependymal vein, as well as an largement of the choroid plexus, leptomeningeal enhancement, and changes in diploic space. RESULTS: In all cases except one, in which non-contrast enhanced study had been performed, varying degrees of leptomeningeal enhancement were seen. In nine cases the cortical vein became smaller; enlargement of the choroid plexus was seen in eight cases, change in the diploic space in seven (including three in which there was angiomatous involvement), and enlargement of the deep venous system in seven. In younger patients, collateral pathways were less developed and leptomeningeal angiomatous changes were more pronounced than in those who were older. CONCLUSION: MRI is a useful modality for the evaluationn of vascular changes in Sturge-Weber syndrome. These changes vary according to a patient's age and the duration of the disease.

5.Development of Rheumatoid Arthritis during Treatment of Cushing's Disease.

Exogenous glucocorticoid has anti-inflammatory effect to reduce symptoms and signs of rheumatoid arthritis. However there are few reports about the role of endogenous glucocorticoid in rheumatoid arthritis. Recently we experienced a case of rheumatoid arthritis developed in a female patient with Cushing's disease as her cortisol level decreased during medical management of hypercortisolemia. She underwent trans-sphenoidal surgery for Cushing's disease (pituitary microadenoma). Six years after surgery she had disease recurrence and received brain radiotherapy and ketoconazole medication. Four years later she developed seronegative rheumatoid arthritis. 24 hour urine cortisol level was below the normal value at that time. Polyarthralgia improved with prednisolone (5mg/day). But polyarthralgia was aggravated as cortisol level decreased below the normal value after prednisolone was discontinued. This case and previous reports suggest that the development and aggravation of rheumatoid arthritis is associated with relative cortisol deficiency, or alteration of balance between neuroendocrine system(hypothalamic- pituitary-adrenal axis) and inflammatory process.

7.The effect of injection speed on remifentanil-induced cough in children.

BACKGROUND: Remifentanil sometimes elicits cough during induction of anesthesia, as with the use of other fentanyl congeners. We designed this study to investigate the incidence of remifentanil-induced cough (RIC) in children and to evaluate the effect of injection speed on RIC. METHODS: One hundred twenty ASA physical status I-II patients, aged 3-12 yr, undergoing general anesthesia were enrolled in the study. Patients were randomly assigned to one of the three groups. Patients in Group R30 received remifentanil 1.5 microg/kg at a constant rate over 30 s. Patients in Group R45 received remifentanil 1.5 microg/kg over 45 s, and patients in Group R60 received remifentanil 1.5 microg/kg over 60 s, respectively. Episodes of cough were recorded and graded as mild (1-2), moderate (3-4), or severe (5 or more). Mean arterial pressure (MAP), heart rate (HR), and SpO2 were recorded on arrival in the operating room (baseline) and 1 min after remifentanil infusion. RESULTS: The incidence of RIC was 33.3% in Group R30 (13 of 39 patients), 17.9% in Group R45 (7 of 39 patients), and 5.0% in Group R60 (2 of 40 patients). Patients in Group R60 had a significantly lower incidence of RIC than those in Group R30 (P = 0.001). The MAP, HR, and SpO2 values were not significantly different between groups. CONCLUSIONS: When intravenous remifentanil 1.5 microg/kg was administered in pediatric patients, the incidence of RIC decreased from 33 to 5% by increasing the injection time from 30 to 60 s. Remifentanil should be administered slowly over 60 s in children to suppress cough during anesthesia induction.

8.Expression and roles of NUPR1 in cholangiocarcinoma cells.

Nuclear protein-1 (NUPR1) is a small nuclear protein that is responsive to various stress stimuli. Although NUPR1 has been associated with cancer development, its expression and roles in cholangiocarcinoma have not yet been described. In the present study, we found that NUPR1 was over-expressed in human cholangiocarcinoma tissues, using immunohistochemistry. The role of NUPR1 in cholangiocarcinoma was examined by its specific siRNA. NUPR1 siRNA decreased proliferation, migration and invasion of human cholangiocarcinoma cell lines (HuCCT1 and SNU1196 cells). From these results, we conclude that NUPR1 is over-expressed in cholangiocarcinoma and regulates the proliferation and motility of cancer cells.

The authors report 2 patients presenting with visual disturbance due to retinal hemorrhage which developed 1 week after clipping of aneurysm in subarachnoid hemorrhage. Funduscopic findings and pathogenesis of retinal hemorrhage are presented & reviewed. Fortunately funduscopic examination seems to be mandatory for early detection of delayed retinal hemorrhage in subarachnoid hemorrhage.

10.Clinical Study of 79 Surgical Cases in Cervical Disc Disease.

Cervical disc disease and spondylosis have troubled human beings since the earliest recording of human ailment, encountered in term of one's life and still is one of the most intricated neurosurgical disorders, although there are a good many kind of diagnostic and surgical advances. Cervical disc disease has been studied more extensively and also diagnostic studies have developed considerably in recent times including myelography, computerized tomographic scan and magnetic resonance imaging. The treatment for cervical disc disease may be medical, surgical, or both but surgery, is increasingly popular owing to development of microanatomy, pathology, diagnostic tools and microsurgery. The surgical approaches to cervical disc disease have developed as the clinical syndrome, mechanisms of disease, and pathological changes that ensue have been better defined. Dramatic result of posterior decompressive laminectomy was reported by Horsley in 1901 and modified by Scoville, Kahn, Fager, and others. The anterior approach to compressive lesions, a more recent development by Robinson in 1955, by Cloward in 1958, and Boldrey, has become increasingly popular. The anterolateral approach, offered by Verbiest in 1968, provided an third avenue to the pathological cervical disc disease. Surgical results for cervical root compression can be relieved well, but myelopathy responds less well to decompression, provided the preoperative diagnosis is accurate and the operation is done carefully from either anterior or posterior approach. The authors has reviewed 79 cases cervical disc disease which treated surgically in Kyung Hee Medical Center from March, 1984 to May, 1990, the result as follows: 1) Cervical disc disease were more frequently encountered between 5th and 6th decades(67%) and the ratio of male to female was 2:1. 2) The symptom onset was acute in 58% of patients but the causes could not define clearly in 76% of patients. 3) The most frequent clinical symptom was pain in the neck(88.6%). 4) The most frequent site of disc herniation was C5-6 level as a single lesion, but the multiple level was effected in 12.7% of patients. 5) Postoperative complications were occurred in 11.4% of patients. 6) The prognosis was influenced by age, sex, time interval between the symptom onset and operation, and direction and character of disc herniation.
