1.The curative effect of music-regulated laser therapy and the quality of life of patients with primary hyper-tension

Objective To assess the therapeutic effect of music- regulated laser therapy on mild and moderate primary hypertension (PH) so as to provide clinical evidence for the development of an effective non-drug therapy for hypertension. Methods One hundred mild and moderate PH patients were divided into obser-vation ( n = 50 ) and control groups ( n = 50 ) according to their stage of hypertension. There were two phases of treatment with 6 weeks in each phase for a total of 12 weeks. In the first phase, patients in the observation group received music-regulated laser therapy while the control group received drug therapy. In the second phase, the treatments were reversed, with the observation group receiving the drugs. The blood pressure and quality of life (QOL) of all the patients was evaluated after each phase. Results Before therapy there were no significant differences in average blood pressure or QOL between the groups. After the first phase, systolic blood pressure (SBP) and diastolic blood pressure (DBP) had been reduced significantly in both groups, but the decrease was significantly greater in the control group. The score on each item of the QOL evaluation was not significantly different between the two groups. After the second phase, blood pressure had decreased further in the observation group, but no significant change was observed in the control group, and there was no significant difference in blood pressure between two groups. The scores on each item of the QOL evaluation were not significantly different between the two groups, but average somatic symptoms, healthy and pleasant feelings, task perform-ance and life satisfaction were significantly ameliorated in both groups. Conclusions Music-regulated laser therapy can lower the blood pressure of mild and moderate PH patients effectively. Compared with antihyperten-sion drugs, music-regulated laser therapy provides a weaker effect in lowering blood pressure, but plays an e-quivalent role in improving the QOL of patients.
2.Evidence summary of health workers′ self-protection during aerosol-generating procedures in patients with acute respiratory infectious disease

Objective:To evaluate and summarize the best evidence of health workers respiratory protection during aerosol-generating procedures in patients with acute respiratory infectious diseases.Methods:We searched EBM Guidelines, Essential Evidence Plus, Dynamed, UpToDate, JBI, BMJ, Clinical Key, Cochrane Library, NICE, AARC, PubMed, EMbase, CKNI, Wanfang to collect related literature including guidelines,evidence summary, recommended practices, standards, consensus and systematic reviews.Results:A total of 20 articles were included, including 10 guidelines, 1 consensus, 2 standards, 2 evidence summaries, and 5 systematic reviews. 24 best evidences including aerosol-generating procedures, training of protective equipment using, principles of protective equipment using, protective equipment using, aerosol operating environment control and medical staff self-monitoring were summarized.Conclusion:The study integrated the best evidence of healthcare workers respiratory protection during aerosol-generating procedures in patients with acute respiratory infectious disease. It is recommended to combine the current status of institutional protection resources and clinical practice experience to promote the conversion of the best evidence to clinical practice.
3.Patterns of Frequency Domain Indexes of Surface Electronic Signals of Adolescent Idiopathic Scoliosis Patients' Paraspinal Muscles

Objective To investigate the patterns of frequency domain indexes of surface electronic signals of adolescent idiopathic scoliosis (AIS) patients' paraspinal muscles by using surface electromyographic (sEMG) techniques .Methods 25 AIS patients enrolled, 7 males, 18 females, ages range from 11 to 21 years old. All of the enrolled patients undertook the Biering Sorensen test(BST) and the object-lifting test, a Finland made type ME3000P sEMG instrument was applied to record the electronic activities of paraspinal muscles(convex /concave) of all subjects, and the frequency domain indexes: median frequency(MF),mean power frequency(MPF), zero crossing rate(ZCR)were analyzed.Results The wave amplitudes and scales of paraspinal muscles electronic frequency domain indexes(MF, MPF, ZCR) were lower than the other tested positions when recorded at the zone of apex vertebrae, and MFslope, MPFslope and ZCRslope all showed a linear degressive tendency as the exercise time was extended.Conclusion Paraspinal muscles at the zone of apex vertebrae have low fatigue durabilities and more likely to be exhausted. sEMG ought to be one of the objective examinations used to evaluate the differences of electronic activities of paraspinal muscles(convex /concave) of AIS patients, and may have a promising value in clinical practice.
4.Comparative proteomic study of nasopharyngeal carcinoma cell lines with different radiosensitivity

Objective To investigate the proteins which were associated with radiosensitivity of nasopharyngeal carcinoma (NPC) cells and could be used to predict the radiosensitivity.Methods A radioresistant subclone cell line CNE-2 (R743) derived from NPC cell line CNE-2 was established.Radiosensitivity and cell cycle characteristics of CNE-2 and CNE-2 ( R743 ) were examined and compared by clonogenic survival assay and flow cytometry.The total proteins from the two cell lines were extracted and separated by two-dimensional gel electrophoresis,and the images were analyzed by Image Master 7.0analysis software.Differentially expressed proteins in the two cell lines were identified through MALDITOF/TOF peptide mass fingerprint and searched in the protein sequence database.The protein expressions were confirmed by RT-PCR and Western blot.Results Totally seven differentially expressed proteins were identified,six of which were upregulated and one downregulated in the radioresistant CNE-2 (R743),compared with those of CNE-2.Three out of the seven,Annexin A2,Tropomyosin 4 and GRP78 were upregulated in the CNE-2 ( R743 ),which were confirmed by Western blot and RT-PCR ( t =24.22,24.20,29.19,P < 0.05).Conclusions Differentially expressed proteins might be involved in different radiosensitivities of nasopharyngeal carcinoma cell lines,among which Annexin A2,Tropomyosin 4 and GRP78 could be the candidate biomarkers for predicting radiosensitivity of nasopharyngeal carcinoma cells.
5.Effect of silencing Annexin A2 gene expression by siRNA on radiosensitivity of nasopharyngeal carcinoma cells

Objective To investigate the effect of silencing Annexin A2 gene expression by small interfering RNA (siRNA) on the radiosensitivity of nasopharyngeal carcinoma cells CNE-2 (R743).Methods siRNA targeting the Annexin A2 gene was chemically synthesized and transfected into R743 cells by HiPerFect.The mRNA and protein levels of Annexin A2 before and after transfection were measured by RT-PCR and Western blot,respectively.The change in radiosensitivity of R743 cells was analyzed by colonyforming assay.Cell cycle distribution and apoptosis after X-ray irradiation were analyzed using flow cytometry and terminal deoxynucleotidyl transferase dUTP nick end labeling assay,respectively.Results The results from RT-PCR and Western blot showed that the expression of Annexin A2 was down-regulated after transfection.The colony-forming assay indicated that the D0,Dq,and SF2 in transfected cells were significantly lower than those in untransfected cells with radiation alone and in cells transfected with control siRNA.The sensitization enhancement ratios (D0 ratios) of transfected cells relative to untransfected and control siRNA transfected cells were 1.30 and 1.27,respectively.After X-ray irradiation,the proportion of cells in G2/M phase was significantly higher in the transfected cells thin in untransfected and control siRNA transfected cells (32.46% vs.9.17% and 9.42%,respectively;P =0.000 and 0.000).The apoptosis rate was also significantly higher in the transfected cells than in the untransfected and control siRNA transfected cells (35.20% vs.10.87% and 11.33%,respectively;P=0.000 and 0.000).Conclusions Silencing Annexin A2 gene expression by siRNA can increase the radiosensitivity of R743 cells,which may be associated with DNA damage repair and change in cell cycle distribution.
6.Expression of WT1 gene in acute leukemia and its significance

Objective To explore the relationship between WT1 and prognosis of patients with acute myeloid leukemia (AML) or acute lymphoblastic leukemia (ALL), and to evaluate the possibility of WT1 as a potential marker for monitoring the minimal residual disease (MRD). Methods Bone marrow mononuclear cells from 58 patients with primary AML, 32 patients with primary ALL, 40 patients with AML-complete remission (CR), 28 patients with ALL-CR and 31 patients with trilineage hyperplasia (control group) were collected. Real-time fluorescent quantitative PCR method was used to detect the expression of WT1 in all patients. The expression threshold of WT1 in each group was established. WT1 copy number/ABL copy number ratio×100%denotes the relative expression level of WT1 gene. Results Median relative expression level of WT1 in the control patients was much lower than that in primary AML patients [0.026%(0-0.240%) vs. 20.880 % (3.550 %-48.500 %), Z=-7.74, P<0.000 1]. Relative expression level of WT1 between AML-CR patients [0.102%(0-5.380%)] and primary AML patients had significant difference (Z=-8.34, P<0.000 1). Moreover, the relative expression level rate of the first course in AML patients with higher WT1 expression level (>20.880 %) was 60.7 % (17/28), while the CR rate was 76.7 % (23/30) in those with lower WT1 expression. WT1 expression was increased dramatically in recurrent AML patients. Relative expression level of WT1 was significantly higher in primary ALL patients [0.350 % (0.021 %-10.780 %)] compared with that in the control group Z=-2.58, P<0.05. There was no significant difference in relative expression level of WT1 between ALL and ALL-CR patients [0.038 % (0-2.800 %), P=0.065]. Conclusion WT1 expression level in AML patients is relatively high, which could be used as an effective index of prognosis evaluation and MRD monitoring for AML patients, but not for ALL patients.
7.Effect of Sorafenib in Increased VEGF in Stimulating Effect of Walker-256 Transplanted Liver Cancer Model in SD Rats

[Objective] To investigate association between the time point of sorafenib administered and suppress effect on tumor growth secondary to the increased expression of vascular endothelial growth factor (VEGF).[Methods] Fifty SD rats were performed intrahepatic implantation using tumor tissues from subcutaneous tumors in nude mice which were administered Walker 256 tumor cells.Ten days after the procedure,MR scans were used to choose forty SD rats with successful hepatic tumor transplantation among fifty experimental animals.Then they were randomly divided into four groups:(A,control group) mere injection of vascular endothelial growth factor (VEGF);(B) administration of sorafenib 72 hours prior to VEGF injection;(C) administration of sorafenib together with VEGF injection;(D) administration of sorafenib 72 hours later to VEGF injection.The tumor growth and median survival time of rodents were observed and compared.After each experimental animal died,immunohistochemical (IHC) methods were applied to detect the expression of VEGF in tumors.[Results] Ten days after the administration of sorafenib,MR showed significant growth of hepatic tumors,the tumor size in experiment group were significiant smaller,than control group (5.4 cm) with statistical significance.Median survival time of four groups were (19.6 ± 1.8) d,(31.2 ± 7.0) d,(27.4 ± 4.9) d,and (26.5 ± 4.6) d,respectively,which indicated that animals in sorafenib groups lived longer than those in control group (P < 0.05).Differences can be obseverd in sorafenib groups with statistical significance existing (P < 0.05).Harvest hepatic tumor tissues from dead animals and HE staining as well as IHC examination were performed.The expression of VEGF in four groups were 88.3 ± 13.6,42.8 ± 8.0,71.9 ± 15.7,and 73.6 ± 13.7.There were statistical significance between control group and sorafenib groups.And further in sorafenib groups,the expressions of VEGF also varied greatly.[Conclusion] Sorafenib can extend the survival time,reduce tumor angiogenesis.And we can conclude that administration of sorafenib before the transient increased expression of VEGF offers survival benefits than that after the evaluation of VEGF levels.

Objective:To provide reference for the development of a more intelligent and systematic nursing clinical decision support system based on the concept of precision nursing and data sharing, the nursing plan module of clinical decision support system.Methods:An evidence-based knowledge base was constructed based on the nursing process and the standardized nursing terminologies; the nursing plan module was designed according to clinical needs, and the logical reasoning rules were formulated from the generation, sequencing and stopping of nursing problems, objectives, measures and activities, and finally the nursing plan module of clinical decision support system was formed.Results:The nursing plan module of clinical decision support system included the basic information of patients, positive evaluation items and weight values, nursing problems, objectives, measures and activities, etc. the module could automatically deduce the nursing plan according to the patient's individual characteristic index (positive evaluation item), and sort the nursing problems and corresponding measures and activities according to the generation time, weight value and correlation degree. It could automatically distinguish nursing problems, goals, measures and the time of activity stop, and realize intelligent decision-making.Conclusion:The interface of nursing plan module of this system is clear and logical reasoning rules are rigorous. It breaks through the bottleneck of nursing decision-making based on personal professional knowledge and experience in clinical situation for a long time, which can ensure the homogeneity of nursing plan and improve the correctness of decision-making.
9.Design and development of clinical decision support system for unplanned extubation

Objective:To design and develop an unplanned extubation clinical decision support system, aiming to provide a reference for nurses' clinical decision-making.Methods:Through literature review and expert consultation, the knowledge base of the clinical decision support system for unplanned extubation was constructed, and the system function and interface were designed.Results:The authoritative coefficients of the two rounds of expert consultation were 0.853 and 0.867, respectively, and the Kendall ′s W were 0.458 and 0.492, respectively. The final built knowledge base included catheter evaluation module, unplanned extubation evaluation module, and knowledge reasoning rule module. At present, the knowledge base had sorted out 48 first-level items, 9 second-level items, 72 third-level items in the catheter evaluation module, and 5 first-level items, 12 second-level items, and 73 third-level items in the unplanned extubation evaluation module. Entry, 40 knowledge reasoning rules. Conclusions:The clinical decision-making system for unplanned extubation has changed the implementation of unplanned extubation risk early warning, optimized the management process, and realized the interaction of the hospital system. It can conduct dynamic unplanned extubation risk assessment based on individualization and assist nurses Make clinical decisions and promote the safety of nursing management.
10.Reflections on "Infant Safety Island" based on the Bioethics of Disabled Newborns and Kant’s "Responsibility" Claim

"Disabled babies" have been concerned by the state and society. In 2011, China’s first "infant safety island" was officially piloted in Shijiazhuang to receive abandoned babies from all over the country, bringing hope to abandoned disabled babies. As the pilot program has been widely promoted, the survival rate of abandoned babies has been effectively improved, but the infant safety island has been branded as "the cage of connivance with the crime of abandonment". At the same time, there are two controversial issues on disabled newborns, the main subjects of abandoned babies: should disabled newborns be rescued? Should a infant safety island exist under the pressure of reality? From the perspective of the bioethics of newborns with disabilities, this paper analyzed the practical significance of the existence of infant safety island based on Kant’s "responsibility" claim, and made a statement for the practicability of safety island from different perspectives.
