National Natural Science Foundation of China becomes a dominating competitiveness that reflects to hospitals' ability of scientific research in China.Based on application and fund availability of National Natural Science Foundation of Shanghai 6th people's hospital in recent years,this article compared data with other hospital's condition to analyze shortcoming and challenge.It also gave suggestions of selecting a subject,raising rate of support,training outstanding young researchers,enhancing interdisciplinary study,and advancing quality of academic papers.This article would give a reference to promote high-speed,harmonic and sustainable development of scientific research in our hospital.
2.Characteristic effect of carbonated hydroxyapatite cement in repairing skeletal defect in a verified experiment

BACKGROUND: Carbonated hydroxyapatite cement is a new type material for skeletal repair and hydroxyapatites have been applied in the clinical treatment of skeletal defect.OBJECTIVE: To observe the effective characteristics of carbonated hydroxyapatite cement on repair of skeletal defect by animal experiment.DESIGN: Paired design, self-controlled and verified experiment was applied in the research.SETTING: Orthopedic Institute and Animal Experimental Center of Chinese PLA.MATERIALS: The experiment was performed in Orthopedic Institute and Animal Experimental Center of Chinese PLA from May 2002 to January 2003, in which, 10 healthy adult male mongrel dogs were applied, body mass weighted varied from 20 to 22 kg.METHODS: Animal model of skeletal defect was prepared on proximal ends of humeri of 10 mongrel dogs thydroxyapatitet were randomized into experimental side and control side. Ceramics repair of skeletal defect was done by carbonated hydroxyapatite cement and high-temperature sintered hydroxyapatite respectively. The animals were sacrificed on the 5th day, 4th, 8th, 12th and 16th weeks successively after operation. The repair effects were performed with X-ray and histological observation.staining.Results of stereomicroscopic and X-ray observations on bilateral skeletal defect: Osseointegration with carbonated hydroxyapatite cement was tight on the experimental side and the interface became unclear gradually with time lasting. The interface between hydroxyapatite and bone was still clear on the and eosin staining and thydroxyapatitet of ground bone with Gimsa staining:On the 8th week on the experimental side, the new bone grew into carbonated hydroxyapatite cement, on the 16th week, the two parts were intermixed and integrated and the bone island was formed around newly generated vessels in carbonated hydroxyapatite cement. On the control side, hydroxyapatite still maintained integrated and the bone interface was clear between hydroxyapatite and bone. On the 16th week, the aggradation of newly generated bone presented on hydroxyapatite surface.CONCLUSION: Carbonated hydroxyapatite cement possesses solidification property in situ, biocompatibility and osseous conductive activity. It is the satisfactory new type material for repair of skeletal defect.
3.Curved vertebroplasty device for thoracolumbar osteoporotic vertebral compression fractures

BACKGROUND:Currently, the cement delivery device used in vertebral augmentation vertebral augmentation (vertebroplasty and kyphoplasty) has flat tips that cannot be bent. During surgery, we always choose injection cement by symmetric way to keep the balance of power on the biomechanics of the vertebral body, while the traditional cement delivery tube can only be used in ipsilateral vertebral body.
OBJECTIVE:To evaluate the feasibility of curved vertebroplasty device for the treatment of thoracolumbar osteoporotic vertebral compression fractures.
METHODS: Sixty patients (72 vertebral bodies) with osteoporotic vertebral compression fractures were enroled, including 8 cases of double-level vertebral fractures and 2 cases of three-level vertebral fractures. Al operations applied curved delivery device in percutaneous vertebroplasty. We recorded operation time, intraoperative blood loss, and X-ray imaging data through prospective self control study. Treatment effect was evaluated with modified Oswestry disability index, Visual Analogue Scale scores and the World Health Organization Quality of Life assessment. The patients were folowed up for 6 months to 2 years.
RESULTS AND CONCLUSION: (1) Surgery was performed successfuly on al 60 patients. The average operation time was 27 minutes (20-45 minutes) for 50 cases by unilateral transpedicular approach. (2) The mean amount of bone cement in every single vertebral of al 72 vertebrae was averagely 6.4 mL (4.2-9.5 mL). Bone cement distribution had not been found to be biased one-sided without severe leakage. (3) Visual Analogue Scale score and Oswestry disability index were significantly improved at 1 day after treatment (P < 0.05). During the final folow-up, no significant difference was detected as compared with 1 day postoperatively (P > 0.05). (4) The ratio of good quality of life in 1 month than postoperatively (87%) and in the last folow-up (92%) was significantly greater compared with the preoperative ratio (27%) (P < 0.05). (5) These findings confirmed that curved vertebroplasty device in unilateral pedicle puncture could ensure bilateral symmetric distribution of bone cement. Al patients are satisfied with treatment. It is proved to be a feasible simple and safe method.
4.Differential expression of haemolymph proteome of resistant strain and susceptible strain for BmNPV in Bombyx mori L.

Three model silkworms, highly resistant strain, highly susceptible strain and their near isogenic line were established by hybridization and backcross. The resistance of silkworm (Bombyx mori L.) to BmNPV was studied at proteomic level using two-dimensional gel electrophoresis and MALDI TOF/TOF MS. Differential expression profiles of proteome in resistant strain, susceptible strain and near isogenic line were obtained, and 180, 190 and 187 of protein spots were shown, respectively. Among them, 80% were concentrated in pI 5-9. Twelve differential protein spots in total were obtained from 3 gels. Using MALDI TOF/TOF MS, five proteins, including aminoacylase, were identified from these spots. The exclusive expression of aminoacylase in highly resistant strain and near isogenic line and its absence in susceptible strain suggest that it might be a BmNPV-resistance related protein.

5.Outcome of childhood acute promyelocytic leukemia treated with all-trans-retinoic acid and chemotherapy

Objective To determine the results of treatment combining all-trans-retinoic acid(ATRA)in childhood acute promyelocytic leukemia(APL).Methods 22 children with newly diagnosed APL received induction therapy with ATRA followed by 3 courses of consolidation chemotherapy:daunorubicin,idarubicin,homoharringtonine or aclacinomycin plus cytosine arabinoside.A maintenance therapy was then administered with ATRA and these reigems for 36 months.Results Early deaths from diffuse intravazcular clotting and intracranial hemorrhage occurred in two patients.The other children achieved a complete remission(CR).By June 2007,the estimated disease-free survival rates at 1,3 and 5 years were 100%,93.3% and 84.7%;respectively.The side effects of ATRA were xerosis eutis and xerocheilia,headaches,nausea and vomiting,hepatic function lesion and ATRA syndrome.Conclusion Remission induction therapy with ATRA is effective and safe for newly diagnosed childhood APL.The maintenance therapy combined chemotherapy with ATRA can improve the long-term effects of APL patients.The main causes of death in APL children is diffuse intravascular clotting and intracranial hemorrhage.The side effects of ATRA can be tolerated.
6.Clinical investigation of sequential therapy with micafungin and reduced -dose voriconazole in prevention of fungal in patients after allogeneic hematopoietic stem cell transplantation

Objective To observe the clinical effect of sequential therapy with micafungin and reduced -dose voriconazole in prevention of invasive fungal infections in patients received allogeneic hematopoietic stem cell transplantion (Allo -HSCT).Methods 28 patients received the treatments for prevention of fungal infection with micafungin 50 mg per day from pretreatment to 30 days,then oral voriconazole at a dose of 1 00 mg two times per day until 90 days after Allo -HSCT.The occurrence of invasive fungal infection and the side effects of both medicine were observed during 1 80 days after Allo -HSCT.Results 8 patients(28.6%)developed above grade 2 acute graft verse host disease(GVHD),2 patients developed grade 3 GVHD among the 8 patients.Two case with GVHD were cured by voriconazole with the therapeutic dose who occurred probably pulmonary invasive fungal infection at two months after Allo -HSCT.There were no other patients diagnosed fungal infection.No toxic efect were observed during the clinical observation during treatment with micafungin.5 patients appeared mild liver function abnormalities during treatment with voriconazole,and liver dysfunction were improved by symptomatic treatment.2 cases developed transient auditory hallucination and visual impairment induced by voriconazole.Conclusion Micafungin and reduced -dose voricon-azole are effective and safe prophylaxis in prevention early invasive fungal infection after HSCT.
7.Live born monochorionic dizygotic twins after in vitro fertilization: a case report

This paper reported a pair of twin babies who were diagnosed as monochorionic (MC) twins (MC) in the first-trimester ultrasound scan after in vitro fertilization.Fluorescence in situ hybridization and chromosomal karyotype analysis of amniotic fluid that was randomly collected from one of the twins were performed in the second trimester,but no abnormality was detected.Chromosome karyotype analysis of peripheral blood of the twins at the age of two days and three years showed that both were chi 47,XY,+21/46,XX.And the single nucleotide polymorphism microarray analysis of oral mucosa cells of the twins at three years old showed that the boy was 47,XY,+21 and the girl was 46,XX.Therefore,we confirmed that they are MC dizygotic twins presenting with blood chimerism but no tissue chimerism.Simply the boy presented with facial features of Down syndrome.
8.Wu-Mei-Wan improves colonic mucosal inflammation and pyroptosis in rats with ulcerative colitis via regulating NLRP3 inflammasome

Objective:To study effect and mechanism of Wu-Mei-Wan on improving colonic mucosal inflammatory response and pyroptosis in rats with ulcerative colitis(UC)via regulating NOD-like receptor family pyrin domain containing 3(NLRP3)inflam-masome.Methods:Male SD rats were selected for animal experiments.UC model was established by enema with 2,4,6-trinitroben-zene sulfonic acid.After model was established,different doses of Wu-Mei-Wan were given by gavage,negative control(NC)-lentivi-rus(LV)or NLRP3-LV were injected by tail vein.Disease activity index(DAI)of rats in each group was evaluated,length of colon was measured,pathological changes and histopathological scores,contents of IL-1β,IL-18,GSDMD-N,NLRP3 and Cleaved cas-pase-1 expressions in colon tissues were detected.Results:Typical UC pathological changes were found in colon mucosa of UC group,DAI,histopathological score,IL-1β,IL-18 contents,GSDMD-N,NLRP3 and Cleaved caspase-1 expressions in colon tissues of UC group were higher than control group,and colon length was shorter than control group(P<0.05).UC pathological changes of colon mucosa of rats in different concentrations of Wu-Mei-Wan groups were improved,DAI,histopathological score,IL-1β,IL-18 con-tents,GSDMD-N,NLRP3 and Cleaved caspase-1 expressions in colon tissues were lower than UC group,and colon length was longer than UC group(P<0.05).LV was injected simultaneously with intragastric administration of high-dose Wu-Mei-Wan,pathological changes of UC in colon mucosa of rats in NLRP3-LV+NC+high-dose Wu-Mei-Wan group was aggravated,DAI,histopathological score,IL-1β,IL-18 contents,GSDMD-N,NLRP3 and Cleaved caspase-1 expressions in colon tissues were higher than NC-LV+NC+ high-dose Wu-Mei-Wan group,and colon length was shorter than NC-LV+NC+high-dose Wu-Mei-Wan group(P<0.05).Conclusion:Wu-Mei-Wan improves colonic mucosal inflammation and pyroptosis in UC rats by inhibiting NLRP3 inflammasome.
9.Alteration of circulating endothelial cells from acute promyelocytic leuke-mia patients before and after treatment and its influential factors

AIM:To determine the biological feature of circulating endothelial cells (CECs) in acute promye-locytic leukemia ( APL) patients before and after treatment , and to analyze the relationship between CECs and the clinical characteristics .METHODS: The CECs were sorted from peripheral blood by magnetic-activated cell sorting and then counted by 3-color flow cytometry.The cells were identified by immunofluorescence staining for the expression of CD 146, CD31, CD144, VEGFR-2, CD45 and CD133.The CECs were cultured in vitro, and the tube formation and colony-forming rate were determined .RESULTS:Increased quantity of CECs was observed in CD 34 positive group and group with WBC >10 ×109/L (P<0.05).The quantity of CECs had a significant difference among low risk , medium risk and high risk groups (P<0.05).The positive rate of CD133 and quantity of CECs significantly reduced in 32 APL patients when they gain complete remission after treatment (P<0.05).The amount of tube formation and colony-forming rate were significant-ly reduced after treatment (P<0.05).The ratio of CECs quantity from APL patients after treatment to that before treatment had a negative correlation with arsenic concentration in urine on day 7 during As2O3 treatment (P<0.05).CONCLU-SION:Accurately counting CECs may be helpful for evaluating prognosis and designing treatment strategy .
10. Breakthrough of invasive fungal disease with posaconazole as primary prophylaxis after induction chemotherapy for acute myeloid leukemia

Objective:
To investigate the breakthrough incidence of invasive fungal disease(IFD) and side effects of posaconazole as primary prophylaxis during induction chemotherapy for acute myeloid leukemia(AML).
Methods:
A total of 206 newly diagnosed AML patients admitted to our department during January 2016 and December 2018 were enrolled in the study. Exclusive criteria were as followings including patients diagnosed as acute promyelocytic leukemia; those who received intravenous antifungal therapy after admission or had history of IFD one month before induction chemotherapy, or those with functional insufficiency of vital organs and those older than 65. Forty-seven patients received posaconazole (posaconazole group), 61 cases received voriconazole (voriconazole group) and 98 cases did not receive any prophylaxis (control group) during induction chemotherapy. Prophylactic efficacy and safety between posaconazole and voriconazole were compared.
Results:
During induction chemotherapy, five possible cases of IFD occurred in posaconazole group (10.6%); while 11 cases (18.0%) were in voriconazole group including 7 possible, 3 probable and 1 proven. Thirty-five cases (35.7%) in control group were diagnosed as IFD including 19 possible, 11 probable and 5 proven ones. The incidences of IFD in posaconazole and voriconazole group were significantly lower than that in control group (
