OBJECTIVE To evaluate the prevention of antibiotic-associated diarrhea(AAD) in surgical critical illness patients and strengthen the usage of antibacterials to AAD in hospital.METHODS Using prediction and retrospection to analyze the factors of AAD.RESULTS The main factors for AAD are changing antibiotics frequently,the age of patient,complicated disease of patient.CONCLUSIONS Rational use and rigorous control of antibiotics are the keys to prevent AAD in hospital.
2.Analysis of the Utilization of Shenfu Injection in Surgery Department of Our Hospital from 2013 to 2014

OBJECTIVE:To know the utilizatior of Shenfu injection(SFI)in our hospital,and provide reference for ratio-nal use of SFI. METHODS:In respective study,patients received SFI from the Hospital Information System(HIS)in surgery department in our hospital from Jan. 2013 to Dec. 2014 were analyzed statistically in aspects of general situation,indications, usage and dosage,course of treatment,drug combination,etc. And the incidence of ADR/ADE was monitored. RESULTS:The 1 247 surgery inpatients used SFI distributed in thyroid and galactophore department(634 cases),comprehensive minimal-ly invasive department (292 cases),gastrointestinal surgery department (250 cases),urology department (64 cases) and car-diothoracic surgery department(7 cases),including 466 male and 781 female,the ratio of male to female was 1∶1.68,aver-age age was 58.50 years;it mainly used in perioperative medication (78.11%) and treated for tumors (11.23%),treatment course concentrated on 2-7 d(71.45%),dosage mainly was 100 ml(70.01%);combination medication were mainly pantopra-zole sodium,Fructus bruceae injection,Xueshuantong for injection(lyophilized),etc. The nonstandard phenomenon was exist-ed in indications,dosage,solvent selection,medication frequency,drug combination,etc. There were no obvious ADR/ADE related to SFI. CONCLUSIONS:More attention must be paid to the irrational use of SFI in surgery clinic of our hospital,the safety of traditional Chinese medicine injection in clinical use should be correctly understood,achieve dialectical therapy and rational drug use.

Objective:To observe the effect of overdose fluoride on the expression of bFGF in rat's dental pulps.Methods:NaF at 20 mg/(kg?d) was given to each of 10 Wister rats by stomach perfusion for 8 weeks, another 10 rats were given the same volume of distilled water as the controls.After treatment,HE staining was used to observe the morphology of the teeth, immunohistochemical staining was adopted for study the expression of bFGF in the dental pulps of the rats.Results:The enamel layer and dentin layer of the incisors in NaF treated rats were thinner than those in the controls,the interglobular dentin was increased and the expression of bFGF in dental pulp and inner dentin was inhibited in the NaF treated rats(P

5.Clinical observation of bone marrow mesenchymal stem cells transplantation in treatment of 22 patients with aplastic anemia

ObjectiveTo explore the efficacy and safety of bone marrow mesenchymal stem cells (BMSC)in treatment of aplastic anemia(AA). MethodsTwenty-two patients with aplastic anemia were enrolled with median age of 31 (12-70) years old,including 11 severe aplastic anemia (SAA),and 3 of whom were cyclosporine and anti-thymocyte globulin-resistant. BMSC were isolated from bone marrow of healthy donors and cultured.The third to fifth generation cells were administered intravenously in 1×106/kg to patients once or twice a week.After infusion,complete blood count,bone marrow aspiration,bone marrow biopsy,flow cytometry analysis of lymphocyte subsets of CD3+ CD4+ and CD3+ CD8+ and clinical symptoms were involved in outcome measurement.ResultsAll patients finished 16-time median (5-83 times) infusions of BMSC with 13-month median (2-33 months) treatment course and 23-month median (2-34 months) follow-up.The total response rate was 72.7 %(16/22), including one patient with essential cure, 9 with remission, 3 with remarkably improvement and 3 with transfusion interval extension.Two of three front-line immunosuppressiveresistant SAA patients achieved remission. Ten of 14 patients recovered from inverted ratio of CD4+/C8+ cells after BMSC treatment. There were no treatment-related side effects observed in the course of treatment.ConclusionBMSC are effective and safe for the treatment of AA in our preliminary study and they deserve further research with larger-scale and long-term clinical trials.
6.Mutation analysis of the mtND4 gene associated with asthenospermia patients

Objective To investigate the relationship between mtND4 point mutation in sperms and asthenospermia. Methods Fifty-six asthenospermia cases and 44 control cases were collected using the WHO criterion for defining asthenospermia, the regions of mtND4 gene were amplified by using PCR of 3 pairs primers. Consequently, the point mutation, missense mutation and multiple single nucleotide polymorphisms (SNP) were analyzed by employing sequencing technology and bioinformatics tools. Results Six mutations never before identified were found. The frequency of single point mutation T10873C and T11944C in the control group were significantly higher than those in the asthenospermia group (P<0.05). Eight cases involved T10873C or T11944C among the 10 cases in control groups with missense mutations were found. But, there were only 2 cases with such mutation in the 10 asthenospermia cases with missense mutations (P<0.05). The previous 20 cases of missense mutations can be described as either multiple SNP group (with T10873C or T11944C) or nonmultiple SNP group. The percentage of a range and a plus b range of multiple SNP group of sperm was significantly higher than the non-multiple SNP group(P<0.05). Conclusions mtND4 gene mutation, especially the missense mutation may induce loss of sperm motility. The mutations of T10873C and T11944C may be useful for sperm motility or counteract the influence for the sperm motility caused by these harmful mutations.
7.Impacts of bladder filling status on dosimetric parameters of target volume and OAR in intensity-modulated radiotherapy for prostate cancer

Objective To study the impacts of bladder filling status on the dosimetric parameters of the target volume and organs at risk (OAR) in intensity-modulated radiotherapy (IMRT) for prostate cancer.Methods Ten localized prostate cancer patients without serious complications treated with IMRT were selected for this study.These patients underwent CT scans of the whole pelvic cavity three times in different bladder filling status (empty and injected with 150 ml and 300 ml of normal saline) to obtain three series of pelvic CT images.The three sets of CT images were transferred to the treatment planning system.The target volume and OAR such as the rectum,bladder,and femoral heads were contoured by the same doctor.The treatment planning was performed and optimized by the physicist.The dosimetric parameters of the target volume and OAR in three bladder filling status were subjected to analysis by paired t-test.Results If the bladder filling status was consistent in orientation and radiation,the bladder filling status was not associated with the dosimetric parameters of the target volume and femoral heads (P =0.077-0.998 ; P =0.219-0.969) ;it had significant impacts on the dosimetric parameters of the bladder (P =0.000-0.562) and some dosimetric parameters of the rectum (P =0.000-0.645),and bladder filling was favorable for the protection of the bladder and rectum.If the bladder filling status was not consistent in orientation and actual radiation,the calculated planning target volume,the dosimetric parameters of the bladder,and some dosimetric parameters of the rectum were different from those in actual treatment (P =0.000-0.913).Conclusions For the prostate cancer patients treated with IMRT,it is recommended to keep the bladder well and consistently filled.
8.Utilization of antibacterial agents for emergency patients with acute upper respiratory infections in tertiary hospitals in Beijing

Objective:To investigate the use of antibacterial agents for emergency patients with acute upper respiratory infections in tertiary hospitals in Beijing .Methods:We used the medical claim data for urban workers in 10 tertiary hospitals in Beijing from Oct .2010 to Sep.2012.Medical records of emer-gency patients with acute upper respiratory tract infections had been selected as the study sample .The proportions of antibacterial prescriptions and categories of antibacterial drugs were described and ana -lyzed.Results:This study included 135 979 visitors (male:42.7%;mean age:43.6 ±16.2 years).The average antibacterial prescription rate was 71.2% (95%CI 71.0%-71.5%), of which the single kind use was 80.0%(95%CI 79.7%-80.2%).Among acute upper respiratory tract infections , the antibacte-rial prescription rate for acute tonsillitis visits was highest (85.1%, 95%CI 84.5%-85.6%), followed by acute laryngitis and bronchitis (81.69%, 95%CI 80.4%-82.8%), acute pharyngitis (81.4%, 95%CI 77.7% -85.0%), acute sinusitis (77.0%, 95%CI 74.6% -79.4%), acute nasopharyngitis (74.3%, 95%CI 73.7%-75.0%), and common cold (67.6%, 95%CI 67.3%-67.9%).Compared with the female group, the antibacterial prescription rate for the male was higher (73.2%, 95%CI 72.8%-73.6%vs.69.7%, 95%CI 69.4%-70.0%).Compared with the <60 years age cases, the anti-bacterial prescription rate for the ≥60 years cases was higher (72.1%, 95%CI 71.8%-72.3% vs. 66 .8%, 95%CI 66 .2%-67 .5%) .In the visitors who used antibacterial drugs , the average percentage of injection use was 50.6%(95%CI 50.3%-50.9%).The top antibacterial drugs in the list of varie-ties were the second generation cephalosporins (28.4%) , followed by the third generation cephalosporins (21.7%), fluoroquinolones (21.0%) and macrolides (17.6%).Conclusion: The antibacterial pre-scription rate for acute upper respiratory tract infections in the general hospitals in Beijing is high , and the second generation cephalosporins , third generation cephalosporins , fluoroquinolones and macrolides take the lead in the total antibacterial drugs .
9.Effect of DC-CIK cells combined with oridonin on cytotoxicity against RPMI 8226 cells

Objective To investigate the changes in cytotoxicity of DC-CIK cells to human multiple myeloma RPMI 8226 cells before and after treatment with oridonin. Methods Normal human peripheral blood mononuclear cells were isolated and induced to obtain DC-CIK cells. Cytotoxicity of DC-CIK cells against RPMI 8226 cells which were treated by oridonin was analyzed by LDH releasing assay. The variation for expression of NKG2D ligands on RPMI 8226 cells were measured by flow cytometry. Results DC-CIK cells were successfully induced from the peripheral blood mononuclear cells. At the same effector to target ratio, oridonin obviously enhanced the cytotocixity of DC-CIK cells against RPMI 8226 cells (P<0.01). Flow cytometry showed the expression of NKG2D ligands ULBP1 of RPMI8226 cells was most significantly increased as the cells were treated by oridonin [(9.19 ± 1.85) vs. (15.47 ± 0.67), P<0.01]. Correlation analysis indicated that cytotocixity was positively correlated with changes in ULBP1. Conclusions Oridonin can improve the cytotoxicity of DC-CIK cells against RPMI 8226 cells, which may be related with the increased expressions of NKG2D ligands on the tumor cell surface.
10.Real Time Three-dimensional Speckle Tracing Imaging in Evaluating Left Ventricular Function in Patients with Hypertrophic Cardiomyopathy

PurposeTo evaluate left ventricular function in patients with non-obstructive hypertrophic cardiomyopathy (HCM) by using three-dimensional speckle tracking imaging (3D-STI).Materials and Methods Twenty-eight patients with non-obstructive hypertrophic cardiomyopathy (HCM group) and 30 healthy volunteers (control group) underwent both conventional two-dimensional echocardiography and 3D-STI. Indexes of conventional echocardiography including interventricular septal thickness (IVST), left ventricular posterior wall at end-diastolic thickness (LVPWT), left ventricular diameter, left ventricular volume, end-systolic left ventricular volume, peak velocity of early diastolic mitral valve, peak diastolic of late diastolic mitral valve, outflow tract frequency spectrum of the aortic valve and left ventricular were compared between the two groups to get the pressure difference and velocity time integral of the left ventricular outflow tract. Early and late diastolic peak velocity of mitral annular were calculated and E/A, Ea/Aa, E/Ea were also obtained. 3D-STI indicators including three-dimensional left ventricular ejection fraction (3D-LVEF), sphericity index (SPI) and global area of strain (GAS), global longitudinal train (GLS), global circumferential strain (GCS), global radial strain (GRS) were also compared.Results In HCM group, IVST, LVPWT and E/Ea increased (P<0.01), Ea/Aa and E/A decreased significantly (P<0.01), LVOT-PG and LVOT-VTI increased significantly (P<0.05 orP<0.01). GCS, GAS, GLS and GRS were significant lower in HCM group than those in control group (P<0.01). Local strain values at the level of papillary muscle and apex were decreased, which were significantly decreased at ventricular septum, anterior wall and inferior wall.Conclusion HCM patients have various degree of decreased myocardial contractility. The real time 3D-STI offers a novel way to assess the left ventricular function of patients with HCM.
