1.A Comparison of UVB-induced Minimal Erythema Dose ( MED ) s to the Skin of the Back and Extremities in Young Adult Koreans.

BACKGROUND: The erythemal response of the skin to UVB radiation is used as a diagnostic phototest and guideline to phototherapy. OBJECTIVE: The purpose of this study was to compare the UVB-induced MEDs to the back, arm, and thigh skin METHODS: A sunlight fluorescenct lamp(Waldmann UV 7001K) was used as a UVB radiation source. The back, arm, and thigh skin were irradiated with the dose, from 40mJ/cm2 to 180mJ/cm2. The minimal doses for erythema responses to the skin were assessed visually at 24 hours after irradiation. RESULTS: MEDs of the back, arm and thigh skin were 92.6 +/- 17.3mJ/cm2(mean +/- S.D.), 123.0 +/- 24.2mJ/cm2, and 126.6+/- 28.3mJ/cm2, respectively. The most frequent MED was 100mJ/cm2 for the back skin and 120mJ/cm' for the arm skin and thigh skin. CONCLUSION: In this study, UVB-induced MEDs to the back, arm, and thigh skin in young adult Koreans were assessed. A significant difference in the MED was found between the back and extremities skin, with a lower value for the back skin(92.6 +/- 17.3mJ/cm2) than for the arm skin(123.0 +/- 24.2mJ/cm2) or for the thigh skin(126.6 +/- 28.3mJ/cm2).

2.Vacuum Arthrography of the Hip Joint

It is possible to create a diastasis in the hip by traction. The space thus created between articular surfaces can be visualized radiographically and is known as “a vacuum phenomenon”. Twenty one cases of unilateral hip joint disease were studied with vacuum arthrography from Oct. 1983 to July 1984 in Catholic Medical College and Center. The patients ages ranged from a month to 53 years of age but most of them(18 out of 21 patients) were below 10. There were 5 female patient's and 16 male patients in this series. The results were as follows: 1. In the absence of an effusion in all normal hip joints the vacuum phenomenon was demonstrated. 2. In the absence of an effusion in the hips of a transient synovitis, 4 Legg-Calve-Perthes disease and 3 sprain the size of vacuum was normal. 3. The size of vacuum of 3 transient synovitis and a pyohip of healing stage which had only mild effusion was smaller than those of normal hips. 4. In the presence of large amount of effusion in the hips of 5 transient synovitis, 2 pyogenic arthritis and 2 tuberculous arthritis it has not been possible to produce the vacuum phenomenon. 5. This study, in summary, shows that the size of vacuum represents the amount of effusion. 6. Non-ossified femoral head in the new born, articular surface of acetabulum and thickness & local changes in the cartilage of the femoral head could be demonstrated using the vacuum arthrography. 7. It has not been possible to demonstrate the articular surface lateral to the limbus, ligamentum teres and limbus in the vacuum arthrogram.

3.A case of occupational asthma induced by latexin a hospital personnel.

IgE-mediated sensitization to natural rubber latex can induce immediate hypersensitivity reactions ranging from contact urticaria to life threatening anaphylaxis. Recent reports suggest that asthma is also relatively frequent manifestation of latex allergy. In this case report, lat,ex induced asthma is described in an operat.ing room nurse regularly exposed t,o latex gloves. Her latex sensitivity was detected by skin prick testing. Specific bronchial provocation test with latex extract showed an early asthmatic reaction. Her symptoms had been much improved after avoidance. Occupational asthma induced by latex may be not uncommon among health care workers.

5.The Outcomes of the Out-of-Hospital Cardiac Arrest: A collaborative research of three hospitals.

BACKGROUND: There has been a lot of changes in prehospital medical environment with development of EMSS(emergency medical service systems). Especially in out-of-hospital cardiac arrest, the patients could survive when they are moved to the hospitals earlier. The purpose of this research is to know the status of EMSS in Korea by analyzing CPR(cardiopulmonary resuscitation) outcomes of out-of-hospital cardiac arrest patients at 3 hospital in the western area of Seoul and Incheon. METHODS: From July 1997 to June 1998, we collected data about out-of-hospital cardiac arrest victims at Ewha Womens University Mokdong Hospital, Catholic University Medical College St. Mary Hospital, and Kachon University Chung Ang Gil Hospital. We used same record form based on the 'Utstein Style'. RESULTS: CPR were performed in 265 out-of-hospital cardiac arrest patients at 3 hospitals. One hundred twelve(42.3%) patients recovered the spontaneous circulation at least once and eight(3.0%) patients discharged alive. One hundred ninety four(73.2%) patients died of medical causes, one hundred two(38.5%) cardiogenic and ninety two(34.7%) non-cardiogenic, and seventy(26.4%) patients died of traumatic causes. Initial EKG showed VT/VF(ventricular tachycardia/ventricular fibrillation) in thirty one(11.7%) patients, asystole in one hundred fifty one(57.0%) patients and other rhythms in eighty three(31.3%) patients. Among one hundred two cardiogenic cardiac arrest patients, two(2.0%) patients was discharged alive. CONCLUSION: Overall survival rate of out-of-hospital cardiac angst patients was 3% which was poorer than that of the western country. The proportion of the cardiogenic cause was 3% which was only hart of the western country. VT/VF is relatively not common as a initial EKG rhythm. These differences might be due to difference in the prevalence pattern of out-of-hospital cardiac arrest as well as prematurity of the EMSS.

6.Cutaneous Metastasis from Internal Malignancy.

We reviewed clinical records and histologic materials of 96 cases of metastatic skin cancer in Seoul National University Hospital from January 1973 to July 1986. The metastatic skin cancers of male were twice as many as that of female and they occurred most frequently in male at their fifties, female, forties. The most frequent primary cancers in men were carcinoma of the stomach, lung, colorectum and pancreas; in women, carcinoma of the breast, stomach, lung and uterine cervix. Cutaneous metastases from the carcinomas of the liver, uterine cervix and stomach which were many in Korea were smaller in number than that from the carcinomas of the lung and breast. Metastatic lesions were recognized before the primary tumor relatively often in carcinoma of the lung, pancreas and rarely in carcinoma of the uterine cervix, stomach. Though localizations of cutaneous metasta.sis were widespread over all body surfaces, regional areas of primary tumor were predisposed to metastasis. Metastatic carcinoma usually produced nonspecific nodules in the skin. Less commonly, some lesions of metastasis showed inflammatory or sclerotic plaque. The histologic findinga of cutaneous metastases generally mimicked that of primary tumors; nevertheless, in most instances, it was not possible to recognize the original tumor from a histologic examination of metastatic skin cancer and it was possible to classify the metastatic carcinoma only as an adenocarcinoma, a squamous cell carcinoma or an undifferntiated carcinoma.

8.Postracheostomy Scar Revision.

The tracheostomy is increasingly being performed. In most cases, the tracheostomy sites are left to heal by secondary intention, so it leaves a depressed and wide scar that is cosmetically disfigured. Another problem os that the scar is also attached directly to the trachea itself and will move vertically with the trachea during the act of swallowing. Even though the tracheostomy scar is cosmetically acceptable, the mobility and retraction of the scar is a continual nuisance to the patient. We performed a retrospective study on 9 patients who had undergone revision of the depressed thracheostomy scar by the Renner Method from June, 1997 to February, 1999. The method includes transverse fusiform incision of the original scar and excision of the depressed portion of the scar to the level of the trachea itself. To prevent attachment of the skin and trachea, a bilateral subcutaneous flap and muscle flap were simply advanced to the midline and overlapped. Then the rest of scar that was not depressed was deepithelized and flipped to augment the soft tissue volume in the central depressed area. Satisfactory results were achieved in all patients without hematoma, infection, hypertrophic scar, and keloid formation. We believe this simple Renner method is one of the best ways of performing posttracheostomy scar revision.

9.The History of Parkinson's Disease and Famous Patients

BACKGROUND: Parkinson's disease (PD) is one of the most common neurodegenerative diseases. However, the history of PD and famous persons with PD have not been described in detail yet. METHODS: We summarized the history of PD before the first description of James Parkinson's. The four famous patients who were suspected or diagnosed with PD were reviewed through peer-reviewed journals as well as biographies, books, and media. RESULTS: Before the definition of PD was established, there were descriptions of various Parkinsonian symptoms in several literatures. The diagnoses of Adolf Hitler and Na Hyeseok are not certain and we only suspect that they had parkinsonism. The diagnoses of PD of the Pope John Paul II and Muhammad Ali are certain as they had medical records as well as video records that shows progressive deterioration. CONCLUSIONS: Even before James Parkinson, PD have been recognized and described focusing on the bradykinesia and tremor. We should keep in mind that detailed examination as well as transcriptions are important, and that long-term follow-up is needed to document or differentiate PD and its mimics.

10.Effects of Olibanum Extracts on the Activity and Differentiation of MC3T3-E1 Cells.

Recently, many natural medicines, which have advantage of less side effects and possibility of long-term use have been studied for their capacity of anti-bacterial, anti-inflammatory and regenerative potential of periodontal tissues. Olibanum has the effects to hemostasis, analgesic and anti-inflammatory, and it also has been traditionally used as a drug for the treatment of bone disease in oriental medicine. The purpose of the present study was to investigate the effects of Olibanum extracts on the activity and differentiation of MC3T3-E1 cells, alkaline phosphatase(ALP) synthesis, formation of bone nodules and expression of type I collagen of MC3T3-E1 cells. To examine the cellular activity, MC3T3-E1 cells were cultured with alpha-MEM(control) and each concentration of Olibanum for 2 days and 4 days. To compare the ALP synthesis, MC3T3-E1 cells were cultured with alpha-MEM(negative control), dexamethasone(positive control), and each concentration of Olibanum for 2 days and 4 days. To compare the bone nodule formation, MC3T3-E1 cells were cultured for 21 days, and to compare the type I collagen expression, MC3T3-E1 cells were cultured for 4 days. The cellular activity of MC3T3-E1 cells treated with 1 microgram/ml of Olibanum extracts was significantly increased at 4-day(p<0.05) to control. The activity of ALP in MC3T3-E1 cells treated with 1 microgram/ml Olibanum extracts was significantly increased at 4-day(p<0.05). All the experimental groups showed much more bone nodule formation than control groups. The group treated with 1 microgram/ml of Olibanum extracts was the highest bone nodule formation, and showed much more type I collagen expression than negative control. These results indicate that Olibanum extracts may be considered effective in the activity and differentiation of MC3T3-E1 cells.
