1.Two Cases of Squamous Cell Carcionoma of Upper Eyelid.

The authors present 2 cases of squamous cell carcinoma which occurred primarily in the eyelid. One is a 33-year-old Korean female who had the carcinoma in the right upper palpebral Conjunctiva, and the other is a 75 year-old Korean female who had the carcinoma at the margin of the left upper eyelid. These two cases were more likely to be confused with chalasion and were confirmed to be squamous cell carcinoma by the gistopathological examination.

2.Lowe's syndrome in a female child: A case report

Lowe et al. have described a disease characterized by organic aciduria, decreased renal ammonia production, hydrophthalmos, and mental retardation, this disease is now called as Lowe's syndrome or oculo-cerebro-renal syndrome which manifests growth plates, metabolic acidosis, and characteristic eye changes, including bilateral cataract, glaucoma and nystagmus. It is a congenital hereditary affectation. Most of the patients with this syndrome are male sex. We report a new case of Lowe's syndrome in a 5-year old girl.

3.Harrington Distraction Rods without Fusion for Thoraco-Lumbar Fracture or Fracture-Dislocation

Management of patients with unstable fracture or fracture-dislocation of thoraco-lumbar spine continues to be controversial. One area of disagreement involves spinal fusion with internal fixation for prevention of later spinal instability. Intemal fixation of the spine without fusion has been advocated in the treatment of the unstable thoraco-lumbar fracture or dislocation. During the period of January 1981 to March 1984, thirty-two patients underwent fixation by Harrington distraction rod without fusion for the treatment of thoraco-lumbar fractures. Eight cases among them had second operation for the removal of the Harrington rods in 13.4months after Harrington rod fixation and were followed up clinically and radiolagically for 1 year at the Department of Orthopaedic Surgery, St. Mary's Hospital, Catholic Medical College and Center. The rationales for this procedure were to minimize the number of permanently stabilized segments and to provide more spinal mobility. We have expected that spontaneous fusion of vertebral body occur by the prolonged immobilization with Hamngton distraction rods. After the Harrington rods, the average conection rate of kyphotic angle was 41.8% and average correction rate of height of collapsed vertebral body or displacement of vertebral body was 42.3% at one year follow up. Instrumentations in eight patient were removed at 13.4 months after the initial surgery and the patients were followed up for one year. At the time of removal of instrumentation, unilateral partial facetectomy was done for histologic examination of the facet joint which is above the lower Harrington hook in four patients. The average correction rate of kyphotic angle was 37.5% and the average correction rate of height of collapsed or displaced vertebral body was 40.6% at one year after the removal of H-rods. The range of spine motions were incresed gradually. But histologic findings of the immobilized facet joint revealed fibrillation, fissures, thinning of the normal cartilagenous surface, decreased cartilagenous cellularity, and vascular tidemark invasion which are characteristics of osteoarthritis. Through this study, we obtained following conclusions: 1. Immediate Harrington distraction rodding was effective for the anatomic reduction of the collapsed vertebral body. And it provided immediate spinal stability, and spontaneous fusion of vertebral body. 2. Prolonged immobilization of the spine by Harrington rods is believed to have deleterious effect on articular cartilage of the immobilized facet joint as the degenerative changes of the cartilage of the facet joint was consistently found in this study. And it was regarded as a predisposing factors in the development of symptomatic arthritis of the spine. 3. If a fractured and collapsed vertebral body can be reduced by Harrington distraction rods, a spontaneous fusion of vertebral body occur. Harrington rodding without fusion could be one of the useful surgeries for management of the fractures and fracture-dislocation of the spine.

4.Arthroscopic Synovectomy on Knee Joint.

The results of arthroscopic synovectomy of the knee joints were reviewd in 23 joints of 21 patients hetween May 1992 and June 1996. The results were as follows: 1. Pain, tenderness, swelling, ROM of synovitis syrnpknns were much nsiproved postoperatively. 2. The degrees of articular cartilage damage were closely related to posto-perative functional recovery. 3. Preoperative and postoperative ESR changes were not ditferent signihcantly, 4. Recurrence rate was 17%, 4 cases. Of them, 2 cases were rheumatoid arthritis, 1 case was pigmented villonodular synovitis, I case was nonspecipic synovitis 5. Complicatior of arthrosc:opic synovectomy was nol found. We concluded that arthroscopic synovectoiny was very useful method of early diagnosis and treatment of synovial memhrane diseases.

6.AN IMMUNOHISTOCHEMICAL STUDY ON THE EXPRESS1ON OF TGF-beta IN THE AMELOBLASTOMA AND DEVELOPING TOOTH GERM OF HUMAN EMBRYO AND FETUSES.

Dysregulation of TGF-beta that is a modulator of cell growth and differentiation can result in uncontrolled growth and tumor formation. Monitoring these pattern of genes and protein expression during tumor development will provide a basis for understanding normal growth and distortion of osteochondrogenesis. Our comparative studies on the experssion of TGF-beta protein indicate that TGF-beta may primarily be a regulator of epithelial differentiation during tooth development (between 4 weeks and 40 gestational weeks) and tumorigenesis of odontogenic neoplasm (ameloblastoma). In early human tooth germ (cap/early bell stage) TGF-beta protein was found in the epithelial dentallamina and enamel orgen. TGF-beta experessions were seen in inner and outer dental epithelium during bell stage. During enamel and cementum appositional stage, TGF-beta expression shifted from the ameloblast to the odontoblast. In eruption stage, TGF-beta expressions look like moderate positive pattern in odontoblast and rare pattern in enamel, dentin and cementum. TGF-beta expressions of follicular and plexiform amelobalstoma are moderate positive reaction in ectodermal tumor components and mild positive in vessels of stroma tissue. In acanthomatous type, TGF-beta expression was shown severely positive finding in stellate reticulum cell. TGF-beta expressions were noted moderate reaction in margin of tumor epithelium and stellate reticulum cell of granular ameloblastoma. In unicystic ameloblastoma, TGF-beta expression was rare feature in cystic luminal epithelium. With these result, I should be suggested that TGF-beta may play an important role in epithelial-mesenchymal interactions in human tooth morphogenesis and development of odontogenic epithelial tumors.

9.A Survey of Accuracy of Reference Citations in Two Korean Dermatologyical Journals.

BACKGROUND: The reference list is an important part of a scientific article. To be useful it must be accurate. OBJECTIVE: The purpose of this study was to evaluate the accuracy of reference citations in the Korean Journal of Dermatology and the Annals of Dermatology. METHODS: We randomly selected 200 references (100 references from Korean language journals and 100 references from foreign language journals) from the Korean Journal of Dermatology and 100 references from the Annals of Dermatology and checked them against the .original articles. RESULTS: The overall rate of citation error was 32%, and errors in the title and author names of the citation were common, each occurring in about two fifths of the citation errors. CONCLUSION: This study shows that the rate of citation errors is unacceptably high in the Korean Journal of Dermatology and the Annals of Dermatology, which significantly diminishes the value of the reference list.

10.Changes of Bone Mineral Density and IL-6 Levels after Bilateral Ovariectomies in Rats.

OBJECTIVE: Estrogen is a major regulator/modulator of bone metabolism, and bone loss in estrogen deficiency is associated with increased bone turnover, But the mechanism for estrogen action on bone metabolism is still unknown. Recent studies have suggested that the increase in bone loss induced by estrogen deficiency is mediated by increased paracrine production of bone resorbing cytokines. Among cytokines, 1nterleukin-6(IL-6) is released from osteoblasts in estrogen deficiency and increases bone resorption by stimulation of osteoclastic activities and recruitment. Thus we performed this study to evaluate the effect of ovariectomies on bone mineral density and IL-6 in cultured monocytes of peripheral blood and bone marrow. METHODS: The experimental animals were 13 female Sprague-Dawley rats that were 8 weeks of age and weighed an average of 188.5 gram at the beginning of the study. Bilateral ovariectomies were performed in all rats from a ventral approach. Bone mineral density(BMD) of the total body, spine and level of IL-6 of cultured monocytes of peripheral blood and bone marrow were measured before and 8 weeks after ovariectomy. RESULTS: 1) BMD of total body and spine were lower after ovariectomy(0.257+/-0.069g/cm2, 0,208+/-0.005g/cm2) than before ovariectomy (0.276+/-0.005g/cm2, 0.229+/-0.011g/cm2), respectively (P<0.01). 2) Although IL-6 level of cultured monocytes in peripheral blood tended to be higher after ovariectomy than before ovariectomy, this difference was not statistically significant (P>0.05). 3) IL-6 level of cultured monocytes in bone marrow was higher after ovariectomy(82.78+/-4.99pg/ml) than before ovariectomy(48.85+/-2.42pg/ml)(P<0.05). CONCLUSION: It is possible that increased production of IL-6 in estrogen deficiency induced by ovariectomy occurs in the local environment of bone or bone marrow rather than in the pheripheral blood and stimulates bone resorption.
