A diabetic 73-year-old man had admitted and examined gastrofiberoscopy due to dyspagia and weight loss of 6 kg during two months. He was confirmed diagnosis of esophageal squamous cell carcinoma. During the esophagography, we found the dye spillage into pericardial space from the esophagus with air collection. A following chest roentgenogram revealed a typical pneumopericardium with pericardial air filling. Although intensive care was done, he expired 4 days later. We report a case of typical pnemopericardium caused secondary to esophageal cancer.

Relapsing polychondritis (RP) is a rare inflammatory disorder of unknown etiology, causing recurrent inflammatory and degenerative reactions involving the cartilaginous structures throughout the body. Generally, RP is known as multiorgan disease presented as auricular chondritis, arthritis, nasal chondritis, ocular inflammation, audiovestibular damage and respiratory tract inflammation. Major airway involvement occurs in more than 50% of the patient and has been reported to be the primary cause of death. Rarely, it may be presented with only respiratory symptoms without typical clinical manifestation of RP. We experienced a 64-year-old male patient with RP involving diffuse airway tract without other characteristic clinical manifestation and present here with a review of literatures.

BACKGROUND: The estimation of ADA activity in pleural fluid has been proved useful tool in the diagnosis of tuberculous pleural effusions. However, there is controversy about its usefulness when estimated in bronchial washing fluid in the patients with pulmonary tuberculosis. This study aims at evaluating the usefulness of measuring ADA activity in bronchial washing fluid of tuberculous patients as biochemical marker in the early diagnosis of the disease. METHODS: We examined the difference of ADA activity in bronchial washing fluid among the group I (tuberculosis group), group II (lung cancer group) and group III (control group). RESULTS: There was significantly higher bronchial washing fluid ADA level in tuberculosis group compared to the lung cancer and control groups(p<0.01). CONCLUSION: These results suggest that bronchial washing fluid ADA activities seem to be a useful tool in the diagnosis of pulmonary tuberculosis.

OBJECTIVE: It is well known that X-ray induces follicular atresia, but the exact mechanism of atresia is not still unveiled completely. In addition, the role of macrophage related with clean-up the dead granulosa cells and other functions within the ovarian follicle is emphasized recently. The aim of this study is to assess the radiation-induced morphological changes of ovarian follicles and follicular macrophages. METHODS: 8 Gy X-ray irradiated on the 3-week old rats (Sprague-Dawley strain), sacrificed at 6, 12, and 24 hours after irradiation, and performed morphological studies with light and transmission electron microscopy, TUNEL, and macrophage immunohistochemistry. RESULTS: Follicular atresia increased significantly (p<0.01) at 6 hours after X-irradiation, and it was decreased significantly (p<0.01) at 12 and 24 hours after irradiation. X-ray induced chromatin condensation in the nucleus and nuclear fragmentation of granulosa cells, which were the typical features of apoptosis. Apoptotic granulosa cells were phagocytosed by the neighboring normal granulosa cells and the macrophages. During atresia of follicles, radioresistant granulosa cells were found in some follicles, which showed similar features morphologically with the granulosa cells of normal follicles. Macrophages were found both within the antrum and at the follicular granulosa layer. CONCLUSION: X-radiation induced follicular atresia by means of granulosa cell apoptosis, and radioresistant granulosa cells which have similar features morphologically with the granulosa cells of normal follicles were observed in some follicles. And the macrophages which phagocytose the apoptotic granulosa cells were located within the follicular antrum and at the follicular granulosa layer.

PURPOSE: To analyze the clinical results of revision arthroplasty for aseptic loosening after performing semiconstrained total elbow replacement. MATERIALS AND METHODS: We retrospectively analyzed fifteen patients that had undergone aseptic loosening after semiconstrained total elbow replacement who also had revision arthroplasty. There were 4 men and 11 women, with a mean age of 57 years. The average duration of follow-up was 54 months. The primary causes of arthroplasty were posttraumatic arthrosis (five cases), rheumatoid arthritis (four cases), primary osteoarthritis (four cases), ankylosis (one case) and Charcot joint (one case). For eleven (73%) elbows, the cementing technique was considered marginal or inadequate at the time of primary arthroplasty. An impaction bone graft was used in seven patients at revision, a strut allograft was used in four patients and cement alone was used in four patients. RESULTS: The average preoperative Mayo elbow performance score was 56.6 points; the average postoperative score was 84.5 points. At the latest follow-up, according to the Mayo elbow performance index, six patient elbows showed an excellent result, six patient elbows showed a good result, one patient elbow had a fair result and two patient elbows showed a poor result. Aseptic loosening occurred in three of four elbows that had been revised with cement only. CONCLUSION: Revision arthroplasty for aseptic loosening after semiconstrained total elbow replacement was useful for the relief of pain, maintenance of stability and the activities of daily living. A poor cementing technique and an inadequate selection of implant may be associated with loosening as a main risk factor. Revision using an impaction graft or strut allograft can be a reliable technique for treating a failed total elbow arthroplasty with massive bone loss.

Leiomyomas account for 2% of the benign tumors of the lower tracheobronchial tree. Tracheal leiomyomas most commonly occur in the fourth decade man. The most co mmon segment to be affected is the lower third of the trachea and the usual site of origin is the posterior mem branous portion. Symptoms are due to partial or com plete obstruction of the trachea. Intermittent or constant dyspnea and wheezing are the most common symptoms of tracheal leiomyoma and have been erroneously ascri bed to bronchial asthma. Clinical symptoms distinguished from bronchial asthma, chest CT scan and bronchoscopy can make diagnosis. The diagnosis of leiomyoma of the lower respiratory tract is by chest C-T scan and bronc hoscopy, which gives direct visualization and biopsy ma terial is sent for histologic examination. The treatment of leiomyoma is essentially conservative since there have been no reports of recurrence after limited resection. Intr atracheal leiomyomas have been dealt with in three ways: by bronchoscopic removal, by excision via a tra cheotomy, and by circumferential resection with primary end-to-end anastomosis. In recent years, successful mass removal with YAG laser and electrocautery technique were reported. We have experienced 36 year-old-male with hemopty sis, cough and sputum production. Tracheal leiomyoma was diagnosed by chest CT scan and bronchoscopy. Mass removal and tracheoplasty were done successfully. So we present this case with the review of literature.

Since Virchow first introduced the term myxoma to describe a tumor that recapitulates the structure of the umbilical cord, it has been increasingly recognized that many diverse neoplasms may acquire a similar myxoid appearance. Myxoma have evolved within the pathology literature from tumors often described in practically all sites to the currently recognized subtypes restricted to the heart, skin, soft tissue, and bone. Pulmonary myxoma is extraordinary rare. We experienced pulmonary myxoma in a 63 year old man. The pulmonary radiology showed mass in right upper lung field, and percutaneous transthoracic needle lung biopsy was performed to confirm the myxoma.
