1.Clinical Use of Cast

30 cases of distal femur fracture were treated with cast-brace for 2 years and 6 months from Feb., 1977 till July, 1979 at the department of Orthopaedic Surgery, Presbyterian Medical Center, in Jeon Ju. The results were obtained as follows: 1. In 9 cases, cast-brace were applied initially and other 21 cases were treated with cast-brace secondarily after operation. 2. 20 cases were male and 10 cases were female. 3. 12 cases were on mid 1/3 fracture and 18 cases were distal 1/3 fracture involving 2 condylar fractures. 4. The average duration of fracture healing was 14.7 weeks in the cases with cast-brace which is compared with 20.7 weeks of hip-spica cast control group. 5. A.G.F. of knee joint was 80–115 degree at removal of cast-brace, otherwise merely 50–65 degree at dlscarding of hip-spica cast. 6. There were no complication such as nonunion or delayed union in the cases with cast-brace.

3.The Statistical Analysis of Primary Bone Tumors

A total of 188 cases of primary bone tumor was reviewed and analyzed clinically and pathologically at the Department of Orthopaedic Surgery, Presbyterian Medical Center, Jeonju, Korea during the 10 years period from January, 1970 to December, 1979. The results were obtained as follows: 1. 58 cases were benign and 130 cases were malignant. 2. Osteochondroma was the most common benign bone tumor and followed by giant cell tumor, simple bone cyst, osteoma and fibrous dysplasia. 3. Giant cell tumor was 15 cases, which were divided pathologically into benign(Grade I & II) 8 cases and malignant (Grade III) 7 cases. and treated by curretage, curettage & bone graft, arthrodesis, irradiation and amputation. 4. The most common primary malignant bone tumor was osteosarcoma, and it was followed by chondrosarcoma, fibrosarcoma and multiple myeloma. 5. Osteosarcoma was 60 cases and it occured commoly around knee joint area. 6. Treatment of primary malignant bone tumor contained of amputation, chemotherapy and irradiation.

5.Contralateral Testicular Single Metastasis from Renal Cell Carcinoma.

Approximately one-third of all patients with renal cell carcinoma have metastases at the time of diagnosis. Metastasis of renal cell carcinoma (RCC) to the testis is a rare finding. The cases of testicular metastasis from renal cell carcinoma that have been reported in the literature are predominantly ipsilateral, and they are invariably found on the left side through the retrograde venous spread along the gonadal artery. We experienced a case of contralateral testicular metastasis from a right side renal cell carcinoma, so we discuss the clinical feature and pathophysiology of this case with a review of the related literature.

6.Intrathoracic Lesion Showing Multiple Cysts and Pneumothorax.

Pulmonary histiocytosis X is an idiopathic benign disease characterized by proliferation and infiltration of lung tissue by characteristic Langerhans cells and eosinophils. Pulmonary histiocytosis X is common in young male adults, and shows variable clinical characteristics. We experienced a case of pathologically proven pulmonary histiocytosis X in a 30-year-old man who visit to our hospital due to chest discomfort and cough. The chest radiograph of our patient shows right pneumothorax and characteristic multiple thin-walled cysts on the both upper lung fields. The HRCT shows multiple thin-walled cysts, a few scattered nodules in both upper and right middle lung, and right pneumothorax.

7.PaCOâ‚‚ at Early Stage is Associated with Adverse Cardiovascular Events in Acute Carbon Monoxide Poisoning

PURPOSE:
The objective was to determine the association between PaCOâ‚‚ and adverse cardiovascular events (ACVEs) in carbon monoxide (CO)-poisoned patients.
METHODS:
This retrospective study included 194 self-breathing patients after CO poisoning with an indication for hyperbaric oxygen therapy and available arterial blood gas analysis at presentation and 6 hours later. The baseline characteristics and clinical course during hospitalization were collected and compared. The mean PaCOâ‚‚ during the first 6 hours after presentation was calculated.
RESULTS:
The incidence rates of moderate (30 mmHg< PaCO₂<35 mmHg) or severe (PaCO₂≤30 mmHg) hypocapnia at presentation after acute CO poisoning were 40.7% and 26.8%, respectively. The mean PaCO₂ during the first 6 hours was 33 (31–36.7) mmHg.The incidence of ACVEs during hospitalization was 50.5%. A significant linear trend in the incidence of ACVEs was observed across the total ranges of PaCO₂ variables. In multivariate regression analysis, mean PaCO₂ was independently associated with ACVEs (OR 0.798 (95% CI 0.641–0.997)).
CONCLUSION
Mean PaCOâ‚‚ during the first 6 hours was associated with increased ACVEs. Given the high incidence of ACVEs and PaCOâ‚‚ derangement and the observed association between PaCOâ‚‚ and ACVEs, this study suggests that 1) PaCOâ‚‚ should be monitored at the acute stage to predict and/or prevent ACVEs; and 2) further study is needed to validate this result and investigate early manipulation of PaCOâ‚‚ as treatment.
8.A Case of Paederus Dermatitis.

Paederus dermatitis is a characteristic irritant contact dermatitis caused by paederin which is a chemical substance released by insects of the genus Paederus. Erythematous vesiculobullous eruption with a burning sensation occur within 24 hours after contact with Paederus. This dermatitis is frequently seen in regions with hot, tropical climates such as Africa and the Middle East. There have been a few reports of Paederus dermatitis in Korea. We recently had a patient with Paederus dermatitis who traveled to Kenya. We would like to highlight an awareness of this peculiar dermatitis due to the increasing numbers of travelers visiting tropical countries.

9.Sparganosis Combined with Inguinal Hernia.

10.A case of combined gastrojejunal and gastrocolic fistula secondary to gastric cancer.

A rare case of gastric cancer associated with gastrojejunal and gastrocolic fistula is presented. A 56-year-old man who had been diagnosed with advanced gastric cancer(Borrmann's type III) 5 months previously was admitted due to watery diarrhea and frequent vomiting for 2 weeks. Fluoroscopic examination was visualized two abnormal passage of contrast medium from the stomach, one to the colon, and the other to the jejunum. Gastrofiberscopy revealed that the tumor on the great curvature of the body appeared to penetrate into the colon, while the other one on the antrum directly invaded into the jejunum. The patient was treated conservatively with total parenteral nutrition and pain control.
