2.The Changes of Postural Balance in Patients with Total Hip Arthroplasty.

PURPOSE: To investigate the changes in postural balance before and after total hip arthroplasty (THA) using computerized dynamic posturography MATERIALS AND METHODS: This was a prospective study of 18 patients undergoing total hip arthroplasty. We evaluated each patient's postural balance prior to, and at least 12 months after, total hip arthroplasty by using computerized dynamic posturography. We compared the preoperative equilibrium scores with the postoperative equilibrium scores. Clinical results were assessed for all patients preoperatively and postoperatively using the Harris hip scoring system. We investigated whether postural balance improves, and what factors were related to this parameter. RESULTS: Patients showed significant improvement in postural balance by means of proprioception after total hip arthroplasty (p<0.05). We found, during intercepting visual compensation, that an increase in the Harris hip score was correlated with an improvement in balance (p<0.01, r = 0.649). CONCLUSION: Patients with Total hip arthroplasty showed improvement in dynamic postural balance through proprioceptive recovery, and balance improved as the Harris hip score increased. This resulted in decreased pain, recovery of articular function, enhancement of physical activity, and ultimately improvement in postural balance by means of total hip arthroplasty.

3.Sacrococcygeal Chordoma

Sacrococcygeal chordoma is a rare malignant tumor which is believed to take its origin from remnants of the fetal notochord, and presents in the sacral hollow or occasionally posteriorly. The growth is leisurely and it is invasive rather than metastatic. In treatment, there is evidence that early diagnosis of sacrococcygeal chordoma followed by radical resection c palliative radiotherapy does provide the opportunity for cure. We have experienced two cases of sacrococcygeal chordoma, which were treated with one case of a complete surgical resection and the other case of only biopsy.

4.Experimental Study for Development of New Alloy for Domestic Total Knee Prosthesis

The total knee replacement is an excellent method for the treatment of osteoarthritis, rheumatoid arthritis, etc. of the knee joint. The tendency of the use of total knee prosthesis is increasing now in Korea. But domestic production of the prosthesis is not avaliable and all are imported from abroad. Moreover, these prosthesis are not well adapted to Korean people, and in occation, the custom-made prosthesis should be needed. So, domestic production of the prosthesis is required. For production of new prosthesis, in cooperation with KAIST(Korea Advanced Institute of Science and Technology), we developed the new alloy 15-57 for total knee prosthesis. We conducted an animal experimental study to confirm the biocompatibility of the new alloy. In this experiment, the Zimmer® company vitallium was compared with this new alloy as control study. The results are as follows: 1. Biocompatibility of the new alloy 15-57 is similar than that of Zimmer® company vitallium in histological study of metal-bone interspace area. 2. Surface corrosion of both metal was nearly absent. 3. In summary, the new alloy 15-57 is suggested as an acceptable metal for the production of total knee prosthesis.

5.A Case of syringocystadenoma Papilliferum.

Syringocystadenoma papilliferum is a benign skin appendage tumor. The typical lesion occurs most frequently on the scalp or face. The lesion consists most commonly of flesh nodule or plaque that may have a papillary, hyperkeratotic surface. A number of hypotheses as to the origin of this disorder have been put forward by different authors, but its origin still remains obscure. We have recently experienced a case of 31 years old female who had a flesh colored, lobulated tumor mass of 3 * 10cm in the left flank area. The histopathological examination revealed rather typical findings of syringocystadenoma papilliferum.

7.Change of Electrolytes after Transfusion of A.C.D. Stored Blood Warmed to 36 degrees C.

In the operation room of S.N.U.H., 10 patients were transfused with A,C.D. stored blood after warming to 36 degrees C by using a blood warmer. Observed changes of Na+, Cl-, K+ and. Ca++ were as follows. 1) Before warming, the electrolytes of A.C.D. stored blood were Na+ 148. 2 mEq/1, Cl-; 117. 0 mEq/1 and K+; 8. 4 mEq/l. After warming to 36C, these were significantly reduced(P <0. 05) and became 144. 6mEq/1, 112. 4mEg/1 and 68. 13mEq/1 respectively. 2) There was a tendency to increasing of K+ cencentration in proportion to storage duration. 3) In the case of calcium, pre-warmed concentration of calcium was 4. 09 mEq/1 and there was no significant change after warming(P>0.05).

9.Gallbladder carcinoma: Regional lymph nodes metastases on CT scan.

CT scans of 23 patients with gallbladder carcinoma were retropectively reviewed to determine the incidence of lymphatic spread and to access the pattern of regional lymph node metastases. The lymphatic spread were noted in 74%(17/23). The involved patterns of regional lymph node metastases were nodes of porta hepatis 65%, portocaval nodes 41%, superior pancreaticoduodenal nodes 47%, posterior pancreaticoduodenal nodes 59%, nodes around the celiac trunk 35%, nodes around the superior mesenteric artery 29% and paraaortic nodes 35%. The findings suggested that the pattern of regional lumph node metastases in gallbladder carcinoma occurred along the normal lymphatic pathway of the gallbladder.
