1.Prebanked Autologous Transfusion using Recombinant Human Erythropoietin: Report of one case

In the orthopaedic field, some elective surgery such such as joint replacement, spinal surgery and limb sparing procedures for musculoskeletal tumors frequently need various amounts of blood trans- fusion. But homologous transfusion occasionally result in various side effect including allergic reaction, febrile reaction, coagulopathies due to reduced platelets and coagulating factors, graft versus host disease(GVHD) and transmission of infectious diseases such as malaria, syphilis, hepatitis and AIDS. Recently these complications especially in elective surgery might result in medicolegal and social problem. Risks of transfusion could be minimized with autologous blood. So many authors are interested in prebanked autologous transfusion in elective orthopaedic surgery to prevent these problems. But there might be need of additional homologous blood transfusion because of the time limited and low yield of autologous blood. Recombinant human erythropoietin(4-HuEPO) has been shown to increase erythropoiesis. The authors could yield 5 units during 14 days using r-HuEPO without any adverse reactions. Therefore we could performed decompression, posterolateral fusion with pedicular screw fixation in spondylolisthesis without additional homologous transfusion.

6.Survey of Patients in ICU from July 1981 to June 1982.

We have analyzed the patients who were admitted to ICU for postoperative care for over 36 hours and/or respiratory problems from July, 1981 to June 1982. 1) The total number of patients was 339, 198(57%) male and 146(43%) female. The mortality rate was 43.7% 2) Overall morality rate of patients in ICU was 49.7% and the 5th decade had the highest mortality rate. 3) The number of the patients who needed respiratory support was 217. 4) The number and type of airways were: orotracheal(174 cases, 96.4%), nasotracheal(9 cases, 4.1%) and trachestomy(34cases, 9.4%). 5) In the course of ventilator support, 72 patients (33.2%) among 217 could be weaned away from the ventilator. 6) The Mortality rate of ventilator supported patients was 59.9%(130). The mortality rate according to the type of the ventilator used was as follows: in Bird 73.2%, in Bourns 71.4% and in Servo 34.2%.

9.A Clinical Study of Serologically Diagnosed Yersinia Pseudotuberculosis Infection in Chidren.

A retrospective analysis of clinical findings in 23 patients with Y. pseudotuberculosis infection who visited Department of Pediatrics of Han Il Hospital from May. 1990 to June, 1992 was performed. 1) The most prevalent age group was 7 to 12 years (16 case: 69.6)and male-to-female ratio was 1.1:1. 2) Monthly distribution showed a high frequency in April, May and June (22 cases:95.7%) 3) The common symptoms were fever(23 cases:100%), abdominal pain (18 cases: 78.3%) vomiting(16 cases: 69.6%), diarrhea (13 cases: 56.7%), rash (13 cases: 56.5%)and acute renal failure (6 cases: 26.1%) in order of frequency, respectively. 4) The laboratory findings were anemia (17.4%), WBC>1(10E4/mm((73.9%), ESR>20mm/hr (91.3%), CRP>3+(91.3%), Positive Widal test (13.0%), BUN>20mg/dl (26.1%) and creatinine> 1.2mg/dl (26.1%). 5) The most common serotype was 4a (30.4%), followed by 4b, 5b, 2b, 2c, 5a, 1b, 1a, 6 in order of frequency. The results suggest that Yersinia pseudotuberculosis infection is one of the important causes of the important causes of fever, eruptive skin diseases and acute renal failure in Korean children and further studies including epidemiology, pathogenesis, etc, are needed.
