The flexibility of surface layer of skin is largely dependent upon the layer's water content. This, in turn, depends primarily on the quantity of water soluble hygroscopic and surface active msterials within the straturn corneum, often known as the natural moisturizing fsctors(NMF). It consist of amino acid, lactic acid, pyrrolidone carboxylic acid and inorganic salts etc.. Some of these components are greatly influenced by hydrogen ion content and ionic form of them were found to be highly hygroscopic. In this experiment, sunburn exfoliated human stratum corneum and pig epidermis separated by 2N-NaBr were used as in vitro test. These tissues were so treated to hsve each pH value (3, 5, 7, 9) and their water holding capacity and hygroscopicity were measured by gravimetric method. The results are summarized as follows : 1, Water holding capactiy of sunburn exfoliated human skin showed significant increase at pH 5 (p<0.1), 7 (p<0.01), 9 (p<0.01) compared to pH 3, and increase in pH 7 (p<0.1), 9 (p<0.05) compared to pH 5 in lactic acid. In pig epidermis, there was significant increase at pH 5 (p<0.05), 7 (p<0.01), 9 (p<0.05) cornpared to pH 3 in lactic acid. 2. We have experienced a case of the more unusual form of supernumerary breast tissue which is characterized by the presence of aberrant gland tissue alone. The case was a 30-year-old female patient in puerperium with bilateral axillary masses of seven years duration. Gentle pressure over the right-sided one caused milk to exude from a skin pore. Histopathologic findings of the tumors revealed the lobules of the breast forming islands of glandular tissue.

We present a case of Kyrle's disease. The pstient was a 48-year-old male who had been undergoing hemodialysis for 6 years due to chronic renal failure caused by polycystic kidney. The lesions were hyperkeratotic papules on the lower legs and abdomen witb 3 months' duration. Histopathologically, the epidermis showed central invsgination filled with heavy keratotic, partly parakeratotic plug, basophilic debris and neutrophils. There were a few premature keratinized cells below the parakeratotic plugs and complete epidermal perforation was present.

We present a case of angioimmunoblastic lymphadenopathy with dysproteinemia (AILD) in a 57-year-old male who had suffered from fever and general weakness of one month's duration. He showed generalized lymphadenopathy, hepatomegaly and purpuric maculopapules on the whole body surface. Lahoratory findings show anemia, polyclonal gammaglobulinemia and thrombocytopenia. The lymph node biopsy specimens show morphologic triad of AILD except lack of interstitial deposits of eosinophilic materials and the skin biopsy specimen shows lymphohistiocytic vasculitis. Bone marrow biopsy specimen shows similar findings in lymph node, suggesting bone marrow involvement. He have been treated with COPP chemotherapy.

PURPOSE: The purpose of this study was to compare mammography and breast sonography in detection of breast cancer and to suggest reasonable guideline of breast imaging in breast cancer patients under 30 years of age in whom breast cancer shows different clinicopathologic characteristics compared with breast cancer in older women. MATERIALS AND METHODS: A. uthors reviewed medical records of 27 patients under 30 years of age with pathologically-proven breast cancer. Age, family history, physical examination findings, indications for breast s0nography were reviewed. Cases in whom breast cancer lesion is detectable and cases in whom not detectable using mammography or breast sonography were reviewed. And then, authors evaluated the usefulness of each method and reasons for nonvisualization of lesion on mammography. RESULTS: Among 27 patients, 25 patients had palpable breast mass as indication of mammography and breast sonography. Cancer lesions were detectable in 16 of 25 patients (64%) on mammography and 24 of 25 patients (96%) on breast ultrasonography. Reasons for nonvisualization of cancer lesions on mammography were dense breast with nodular parenchyma pattern and minimal breast change of ductal carcinoma in situ. CONCLUSION: In breast cancer patients under 30 years of age who have palpable breast mass as a initiaJ, and main clinical problem, breast ultrasonography is superior to mammography in detecting and diagnosing breast cancer. We suggest that guidelines can avoid unnecessary mammography in these patients.

Primary gout is a metabolic disease that has its basis in an inborn error in the intermediary metabolism of purines and related compounds. It is characterized by hyperuricemia, recurrent attacks of acute arthritis, deposition of monosodium urate crystals in and around the joints of the extremities, urolithiasis, renal and cardiovascular disease. Thirty-eight patients with gout managed at the department of orthopaedic surgery, Dong-A University Hospital, from April, 1990 to November 1993, had been reviewed retrospectively and the following results were obtained. 1. Most of cases were male(95%) and the mean age was 55.4 years old. 2. The case of first attack was 8 cases(21%), the most common age of first attack was fifth decade and mean duration of disease was 7.4 years. 3. The most common precipitating factor was overindulgence in food and alcohol. 4. The avarage uric acid level of serum and that 24-hour urine collection in acute attacks were 11.2 mg/dl and 24.5mg/D. Respectively all of the cases except four revealed increased ESR (average 43 mm/hr). 5. When the acute attack was developed, the most common involved joint was the first M-P joint (83%) of the foot, but as the acute gout was progressed to the chronic form, multiple joints were affected. 6, The punched out lesion in the radiograph was found in 7 cases(25 joints), among them 6 cases had suffered from the disease for longer than 3 years. 7. The most common complication was nephropathy(18 cases). 8. In case of acute gouty attack, we administered colchicine in the 22 cases. The therapeutic method of colchicine usually consisted of oral administration of 2 tablets(1.3mg) initially, followed by 1 tablet(0.65mg) every hour untill the relief of joint pain or the development of gastointestinal symptoms. We rocomended 6-8 tablets of colchicine for the management of acute gouty attack with safety. 9. Colchicine in small daily dose(0.65mg qd or bid) could be effective to prevent the recurrent attacks of high risk gouty arthritis.

PURPOSE: The purpose of this study is to evaluate the CT features and pathogenesis of the electric brain injuries. MATERIALS AND METHODS: We reviewed the CT scans of 3 patients injured by high-voltage electricity. We evaluated the findings early and delayed periods in each patients. RESULTS: The early CT findings were diffuse brain edema, scalp swelling, and focal hemorrhagic contusion. The findings of delayed period were cerebral infarction, pneumocephalus, brain abscess, and pneumatocele. CONCLUSION: CT was useful to correlate the pathogenesis and variable features of electric brain injuries.
