2.Phenotypic Changes of Intermediate Filament Proteins during Degeneration and Regeneration of Rat Leg Muscles following Sciatic Nerve Injury.

PURPOSE: The intermediate filament proteins, desmin and vimentin, are specific components of the cytoskeleton of striated muscle fibers and of mononuclear cells of mesenchymal origin including myoblasts, respectively. Desmin has also been found in presumptive myoblasts of mammals. The aim of this experiment was attempted to observe the phenotypic changes of intermediate filaments in skeletal muscle fibers during early stages of sciatic nerve crushing injury. MATERIALS AND METHODS: The sciatic nerves of rats were surgically crushed by hemostat and serial cryosections of soleus and extensor digitorum longus(EDL) muscles were prepared at 2, 4, 6, 8, 10, 15, 20 and 27 days after nerve injury. Serial cryosections were immunolabelled with desmin, vimentin and laminin and were histochemically reacted with NADH-TR. RESULTS: 1) Firstly, desmin positive fibers were appeared in fast-twitch type C fibers of both muscles at 6 days after nerve crushing, but were not reacted for vimentin. 2) Co-expressions of desmin and vimentin were firstly detected in fast-twitch type A fibers of EDL muscles at 8 days after nerve injury. In soleus muscles, co-expressions of desmin and vimentin were firstly seen in slow-twitch type B fibers at 10 days after nerve injury. Many atrophic fibers, that contained several central nuclei like myotubes and co-expressed desmin and vimentin, were appeared in EDL muscles at 10 days after nerve injury. Although whole regions of fibers were regenerated in EDL muscles, only peripheral regions of fibers were regenerated in soleus muscles at 15 days after nerve injury. Many atrophic fibers, co-expressed of desmin and vimentin, were appeared in EDL muscles at 20 days after nerve injury. These whole fibers represented various degrees of regenerating stages. Most of mature fibers containing several central nuclei, only expressed vimentin slightly, were seen in soleus muscles at 20 days after nerve injury. Most fibers of both muscles were matured at 27 days after nerve injury, but some fibers in EDL muscles were still in processing of degeneration and regeneration. No expressions of desmin and vimentin indicated that muscle fibers were almostly matured in soleus muscles at 27 days after nerve injury. 3) Targetoid or target fibers which informed reinnervation, were appeared firstly in soleus muscles at 20 days and were seen in both muscles at 27 days after nerve injury. All targetoid and target fibers were type B fibers. CONCLUSION: Desmin was revealed in processes of degeneration and regeneration and vimentin was appealed in regeneration process. At the same time, positive immunoreactivity of desmin and vimentin showed specific differences in degree of degeneration and regeneration according to different muscles and muscle fibers.

4.Facet Joint Angle in Lumbar Spine: comparative study between HNP and spinal stenosis and control group.

The importance of facet joint asymmetry has been debated as a cause of the low back pain and sciatica. And many studies have been proposed about the relationship between facet joint angle or facet joint asymmetry and the development of the disc degeneration or disc herniation. In the diagnosis of the spinal disorder, we have been used mainly myelogram and computed tomography. Several years ago, more advancement has been achieved by use of magnetic resonance image (MRI). Author studied the relationship between the facet joint angle and facet joint asymmetry and the development of the herniated nucleus pulposis (HNP) or spinal stenosis by comparision of the facet joint angle and facet joint asymmetry in each groups. Author reviewed the patients who were diagnosed as HNP or spinal stenosis with computerized tomography (CT) or MRI and treated with operative method at the department of orthopaedic surgery, Dong-A University Hospital. Facet joint angles were measured on the mid-disc cut, which was parallel to the inferior vertebral end-plate of the superior vertebra. The angles were measured by a midsagittal line through the disc and intersecting lines formed by conneqting the two end points of each facet. Facet joint asymmetry was determined by the difference between right and left facet joint angles. The results were compared between each groups and comparison group and statistical analysis was performed with the Kruskal- Wallis test in 95% confidence interval. The patients had only one level involved and had no other spinal disorder. 85 cases of HNP (central in 35 cases and lateral in 50 cases) and 50 cases of spinal stenosis were evaluated. The results were as follows: l. At the level of L4-5, the mean facet joint angles (+/-SD) were 42.50+/-5.52 (Rt.), 46.71+/-9.68 (Lt.) in central HNP, 40.91+/-9.11 (Rt.), 41.23+/-8.51 (Lt.) in lateral HNP, 37.77+/-11.41 (Rt.), 37.95+/-11.91 (Lt.) in spinal stenosis and 42.38+/-5.07 (Rt.), 41.25+/-5.85 (Lt.) in control group. 2. At the level of LS-S1, the mean facet joint angles were 48.83+/-5.64 (Rt.), 48.17+/-4.62 (Lt.) in central HNP, 52.20+/-11.30 (Rt.), 51.60+/-9.06 (Lt.) in lateral HNP, 42.67+/-8.89 (Rt.), 43.50+/- 9.85 (Lt.) in spinal stenosis and 43.91+/-7.88 (Rt.), 43.76+/-7.81 (Lt.) in control group. 3. At the level of L4-5, the mean facet asymmery was 6.35+/-6.04 in central HNP, 6.95+/-6.76 in lateral HNP, 6.47+/-5.56 in spinal stenosis and 4.31+/-5.42 in control group. 4. At the level of L5-S1, the mean facet asymmetry was 4.95+/-5.18 in central HNP, 4.72+/-5.25 in lateral HNP, 5.50+/-4.83 in spinal stenosis and 4.42+/-4.96 in control group. In conclusion, there were no statistically significant relationships between the magnitude of the facet joint angle and development of the HNP and spinal stenosis, and between the facet asymmetry and development of the HNP and stenosis.

5.Radiologic Evaluation of Aseptic Loosening in Uncemented Total Hip Replacement Arthroplasty.

The aseptic loosening has emerged as one of the most common and serious long term complications, which requires the revision of total hip replacement arthroplasty. We studied the radiologic finding of the aseptic loosening in 79 patients of uncemented total hip replacement arthroplasty which had been performed in our hospital from March 1991 to December 1994. We evaluated the white reaction lines, osteolysis state, translation and migration in postoperative and last follow-up radiographies. The white reactive lines around the femoral stem and the acetabular cup were most frequent in zone IV and zone II. But it was not progressive and need more long term follow-up evaluations to confirm whether these lines are related to loosening or not. In assessment for stable state, the more stable at initial fixations, the better state of long term stability. Some patients have pain without loosening but all patients with loosening have pain. Press fit is the most important factor of the late stability. The osteolysis is not directly proportional to femoral loosening in this study, but it might be due to short follow up time.

6.The Interlocking Kuntscher IM Nailing for Femur Shaft Fracture

The intramedullary nailing is a good method of treatment in femur shaft fracture but it is not available in some fracture pattern or fracture level. And it is very difficult with conventional Kiintscher nailing to prevent torsional stress and fix rigidly in unstable fracture of the femur shaft. The unstable fracture by comminuted fracture or segmental fracture, nonunion and pathologic fracture needed the rigid fixation. Interlocking Kuntscher IM nailing can provide antitorsional stability, good axial alignment and prevent shortening of the fracture site, also allow early ambulation and joint exercise. We analysed 51 patients 52 cases of interlocking Kiintscher IM nailing from May 1981 to March 1988. The interlocking Kuntscher IM nailing prevents the rotational and axial roading. The interocking Kuntscher IM nailing has expanded its application in fracture pattern and fracture site. The interlocking IM nailing provides rigid fixation in severe comminuted fracture. segmental fracture, long spiral fracture, and other several unstable fractures and eliminates splinting or external supports, so it is possible doing early joint exercise. The interlocking Kuntscher IM nailing was also considered best internal fixation method in pathologic fracture or sever osteoporosis. The static interlocking for rigid fixation and the dynamic interlocking method for axial compression to fracture site during weight bearing can be adapted appropriately to fracture pattern. Radiation hazard during the interlocking nailing is not considerable.

7.Herniated Intervertebral Disc of Lumbar Spine in the Teenager

The incidence of herniated lumbar disc under the age of 19 years is increasing due to increased sports activity of teenager and development of diagnostic methods. 33 cases of herniated lumbar disc of the teenager was analyzed at Department of Orthopaedic Surgery, Dong-A College of Medicine from March 1990 to May 1994. The results were obtained as follows: 1) Among 413 patients, 33 patients(7.9%) were teenagers and the sex ratio was 2.3:1 in male to female. 2) Average intervals between the onset of clinical symptoms and the admission were 13.2 months. 3) 20 cases(60.6%) had a definite trauma history, and among these cases, 14 cases(70.0%) were associated with sports activity. 4) The most common level of herniation was L4-5 in 29 cases(64.4%), straight leg raising test was positive in 32 cases(97.0%), sensory change in 13 cases(39.4%), motor weakness in 10 cases(30.3%). 5) Among positive MRI findings(32 cases), 17 cases(53.1%) were the protruded type. 6) Among 23 operative cases, 21 cases had excellent or good results.

8.Patency Rate of Prosthetic Arteriovenous Fistula Correlated with the Sites.

PURPOSE: Patency of the arteriovenous fistula remains the most important support for patients with end stage renal disease. Prosthetic materials have been used only when autogenous arteriovenous fistula procedure is not feasible, because of the relatively low patency in prosthetic arteriovenous fistula. We designed this study to determine the appropriateness of primary prosthetic arteriovenous fistula. METHOD: Twenty-one consecutive patients who underwent prosthetic arteriovenous fistula formation on the arm in Chosun university hospital from Mar. 1998 to Feb. 2001 were retrospectively analysed. RESULT: The median patency in radio-antecubital fistula, brachio-antecubital fistula, and brachio-axillary fistula was 9.6, 11.4, and 16.6 months, respectively. The primary assisted patency rates in radio-antecubital fistula and brachio-antecubital fistula were 44.9% and 70.0% at the first year, respectively (P=0.03). The secondary patency rates in radio-antecubital fistula and brachio-antecubital fistula were 71.4% and 80.0% at the first year and 28.6% and 48.0% at the second year, respectively (0.02). CONCLUSION: The patency rates according to the site of prosthetic arteriovenous fistula were statistically significant in the radio-antecubital fistula and brachio-antecubital fistula groups. Nevertheless, the patency rates of the brachio-axillary fistula group were statistically insignificant. Primary brachio-antecubital fistula with PTFE creation showed favorable patency rates and many other benefits. Therefore, we suggest primary brachio-antecubital fistula with PTFE as the appropriate arteriovenous access in patients with poor vessel condition.

10.THE CLINICOSTATISTICAL STUDY OF FACIAL BONE FRACTURE IN RECENT FIVE YEARS.

This study was aimed to furnish the data of facial bone fracture which was related regional and social environment and to aid diagnosis and treatment of the fracture. This is the retrospective and clinicostatical study on 625 patients with facial bone fracture. The patients were treated in the Dept. of Oral and Maxillofacial Surgery of Wonkwang University Hospital from Jan. 1, 1993 to Dec. 31, 1997. The result were as follows : The facial bone fractures occured most frequently in the twenties(32.3%) and male were predominent(77.4%) than female. The most frequent etiologic factor of facial bone fracture was fall down(42.8%) and midface fracture was traffic accident(63.4%). The most common site of fracture was symphysis(51.1%) and angle(29.7%), condyle(27.5%), ZMC(13.4%) were next in order of frequency. Of the 626 patients, 590 patients(89.5%) were treated with open reduction and only 36 patients(10.5%) were treated with closed reduction. In the blow, the ratio of angle fracture(50%) is especially high(the overall ratio 29.7%) and in the Out car TA, the ratio of ZMC fracture(34.6%) is especially high(the overall ratio 13.4%). In the fall-down, the ratio of symphysis(58.4%) and condyle fracture(33.6%) is especially high(the overall ratio 51.1%, 27.5%). Postoperatives complication were found in 35(5.6%) of 626 patients, in which mouth opening limitation(3%) was the most common. Above results suggest that early diagnosis and treatment of fracture site, systemic condition and associated injuries are nessary, and cooperative treatment with medical department should be performed.
