2.Studies on the inducing possibility of human visceral larva migrans associated with eating habit of raw liver of domestic animals.

To observe the possibility of human visceral larva migrans due to eating of raw liver of domestic animals, especially of cattle, and also to serve as a good reference for adequate sanitary measures, the investigation survey was carried out from May 1975 to May 1976. From the subjects of a l,048 inhabitants (male 558, female 490) in five localities including two Provinces and three different cities, food habit was studied by questionnaire mannual. Larvae isolated from liver tissues of cattle, and pig were identified. Experimental observation on the chicken and mice infected with Toxocara canis was undertaken to draw a assumption of possibility inducing human visceral larva migrans. The results obtained from the present study are summarized. A part of Korean people has the habit to eat the livers of cattle, fowl, pig and dog raw. Eating rate of raw beef liver was 37.8 percent out of l,048 inhabitants, and its rate was higher markedly in male(57.7 percent) than in female (15. 1 percent), and the highest rate among the group of 31-40 years old. Eating rate of raw liver of fowl was 5.9 percent, pig 5.3 percent, and dog 2.5 percent. Larva recovery rate from beef liver was 11.8 percent out of 195 samples and 72.0 percent of total detected 1arvae were identified as Toxocara(=Neoascaris) vitulorum. From pig liver, larvae of nematoda were found in 6.4 percent out of 109 samples but no larva was detected from 120 fowl livers. Larvae detected from one-half of tissues and organs of infected chicken with about 2,000 Toxocara canis eggs were 8-245 in number, and 85-100 percent of recovered larvae were from their 1iver tissues. Toxocara canis larvae, 45, 31, 42 and 23 in number at 3rd, 14th, 25th and 55th day in one-half of the tissues and organs after infection respectively, were demonstrated from the mice infected with 500 larvae collected from infected chicken liver. Most of the larvae were recovered from the carcass of the mouse. It was approved the larvae isolated from chicken possess infectivity to the mice. Typical eosinophilic granulomatous change was not observed in the liver tissue of the infected chicken at 20th day after infection. As it summarized above, the liver of various domestic animals is the favorite tissue for migration of nematodes larvae. Therefore, the possibility of human visceral larva migrans may be induced due to eating of raw liver of domestic animals.

4.A study to the workers exposed to organic solvents by neurobehavioral tests.

In order to evaluate the confounding factors of neurobehavioral tests and the neurobehavioral effects in the workers exposed to organic solvents, NCTB was carried out on 100 workers. 46 workers had never been exposed to neurotoxic substances, and the others were being exposed to the solvents, mainly toluene. Simple reaction time, digit symbol, Santa Ana dexterity test and persuit aiming were different with age in non exposure group. Simple reaction time was carried out well in males, and digit symbol and persuit aiming were in females. There was no difference at educational level when the subject was educated over 12 years. Santa Ana dexterity and Benton visual test differed according to exposure level to toluene, however simple reaction time didn't. The acute neurotoxic effect was not excluded in this study. But, NCTB could be used to evaluate and prevent neurobehavioral changes in workers exposed to neurotoxic solvents in Korea.

6.Experimental study on the effect of cortisone in mice infected with Toxocara canis: Histopathological findings of granuloma in the liver.

The present study was carried out to examine the effect of "Cortisone" on the worm burden migrated in the organs, and granulomatous change in the liver of mice infected with Toxocara canis eggs. Daily does of 0.5 mg "Cortisone" was administrated subcutaneously to Toxocara-infected mice with different schedule. The results are summarized as follows: More number of Toxocara larvae migrated into the muscle tissue, especially into the carcass through the wall of the large intestine and higher distribution rate and longer persistence of worms were also recognized in the carcass of the experimental group than in the control. Cortisone administration resulted the inhibition of granuloma formation, as well as the resolution of granuloma in the liver. However, the discontinuation of the administration induced the reccurence of heavier grade of inflammation. In non-cortisone treated group (control), cellular infiltration appeared around the worms in the liver on the 10th day of the infection, and typical granulomatosis was followed on the 20th day. As in above results, it is considered cortisone has remarkable anti-inflammatory and immunosuppressive effects in mice infected with T.canis, but histopathological changes on the granuloma formation might be related closely with the duration of Toxocara infection, dosage, duration and timing of cortisone administration.

7.Collective Review of Cases of Spinal Metastases

The authors have reviewed and analysed 238 cases of the apinal metastases. Those cases were treated from January, 1980 to December, 1986 at the department of orthopaedic surgery of Kosin medical center. Among these cases, 37 were received radiotherapy alone ; 10, surgery alone ; 9, combined treatment and the rest of 137 cases were treated symptomatically. In doing the surgical decompression, it was performed in an emergent basis, designed to minimize the progression of the neurologic dificit and to prevent the serious consequences. The effectiveness of the treatment was discussed. And our review concludes the followings : 1. The most frequent primary tumor is breast cancer in woman and lung cancer in man. A sex ratio is more frequent in woman, and peak age is 6th decade. An average survival time is longest in nasopharyngeal cancer(16.6 mos.). 2. The most frequent metastatic deposit is the thoracolumbar spine(28.57%). 3. The prognosis is best in the breast cancer(83.3%), and cervical involvement(66.7%). 4. The prognosis is good for cases with incomplete paraplegia, intact sphincter control, a long duration of pain and neurologic deficit, and the cases with cord compression of gradual onset. 5. The radiotheraphy is the most effective method of treatment.

8.Multiple Metastatic Chordoma only to Muscle & Subcutaneum

Chordoma is relatively rare and slowly growing tumor arising from notochordal remnant. The tumor is locally infiltrative rather than metastatic. Distant metastasis occured in 5% of Gentil & Coley's cases and 43% of Higinbotham et als cases. Usual sites of metastasis sites were lung, liver adrenal gland and lymph nodes. In 1922, Stewart reported first case of extrapulmonary metastatic chordoma in muscle & subcutaneous layer, which was extremely rare. We have experienced a case of wide spread multiple metastasis to muscle & subcutaneous tissue from primary sacrococcygeal chordoma. The case was 26 years old man, who have had multiple metastatic lesions in trunk, upper & lower extremities. They located in muscle and subcutaneous fat layer. In some lesions neurovascular structure was involved. We have treated the metastatic lesions by complete surgical excision, and they were confirmed as soft tissue chordoma by histologic examination.

9.Meniscal cysts of Knee Joint: Report of 2 Cases

A meniscal cyst is not common disease of knee joint. Many cases have been reported but its cause and origin are still pooly understood. We experienced two cases of the cysts on the lateral meniscus. They were treated by arthroscopic partial menisectomy and extraarticular excision of the cyst. The good results were obtained.

10.Clinical Analysis of Unstable Thoracolumbar Fracture and Fracture-dislocation Using Transpedicular Screws and Harrington distration rod

Transpedicular Screw fixation and Harrington distraction rod are effective means of managing unstable thoracolumbar fracture and fracture dislocation. The authors analysed the 52patients who were treated with above 2methods from March 1986 to February 1993. The results were as follows: 1. Most of patients were in 5th decade compare to younger group and most common cause of injury was fall down, but traffic accident is increasing. 2. The most commonly involved sites were T12and L1 vertebrae and most common type of injury is bursting fracture. 3. Postoperative anterior and posterior column of vertebrae body correction rate were 35.0%/34.4% in ISF group and 29.3%/27.9% in Harrington group. Postoperative correction loss rate were 0.13/0.04 in ISF group and 0.06/0.04 in Harrington group. 4. Postoperative correction of kyphosis angle were average 18 in ISF goup and average 15° in Harrington group. Loss of correction angle was average 23° in ISF group and average 4.4° in Harrington group. 5. Range of spinal fixation was average 3.1 segments in ISF group and average 6.2 segments in Harrington group. 6. There was no significant difference in neurologic recovery between ISF group and Harrington group. The patients who had been recovered neurologically more than 1 grade were 28.6% in Harrington group and 29.2% in ISF group. 7. ISF has some benefit in spnal vertebral body height correction and in maintenance of correction compare to Harrington group. Nevertheless disadvantages of ISF group such as long operative time and possibility of cord compression by pedicular screw, ISF has advantages of almost anatomical reduction and the least spinal fusion.
