PURPOSE: This study was done to investigate the effect of foot reflexology on vital signs, general fatigue, foot fatigue, mood, and blood glucose levels in noninsulin dependent patients. METHOD: The Research design of this study was nonequivalent control group quasi-experimental design. 18 patients were assigned to the experimental group, 24 patients to the control group. The data were obtained diaberic patients with ambulatory endocrine outpatients clinic patients from 40 years old to 70 years old. Experimental groups received foot reflex massage for 30minutes three times/week every other days, and Control groups did not received foot reflex massage. The dependent variables were blood pressure, pulse rate. visual analogue scale for general fatigue, foot fatigue, mood. and blood sugar levels. Data were analyzed with chi2test. t-test and repeated measure ANOVA at .0.05 level of significance. RESULTS: There were significant difference in the pulse rate, general fatigue. foot fatigue and mood according to group and time between pre and post foot reflexology. But this research did not prove to decrease blood sugar levels. Conclusions Foot reflexology can imorove pulse rate, general and foot fatigue, and mood status in diabetus patients. So further research need to explore the effect of decreasing of blood sugar levels.

PURPOSE: This study was to investigate the degree of body image, self esteem and quality of life, to identify general and disease of characteristics on influencing this factors with the intention of providing basal data for developing nursing intervention to promote body image, self esteem and quality of life. METHOD: Subjects of this study were 91 grown-up congenital heart patients over 18 years in 2 tertiary hospitals. The data on body image, self esteem and quality of life were collected through questionnaire from March to April 2004. The data were analyzed by descriptive statistics, t-test, Pearson's correlation, ANOVA and tukey test using SAS for Windows 8.1 program. RESULT: 1) The mean scores of body image, self esteem and quality of life were 63.01, 25.29 and 496.79. 2) Body image was correlated with age(p=.0239), educational level(p=.0182), diagnosis(p=.0066), number of operation(p=.0148), cyanosis(p<.0001), complication(p=.0096) and NYHA level(p=.0378). 3) Self esteem was correlated with education level(p=.0026), economic level(p=.0240), number of operation(p=.0113) and cyanosis (p=.0006). 4) Quality of life was correlated with age(p=.0432) and diagnosis(p=.0020), number of operation (p=.0063), duration of last operation(p=.0225), cyanosis(p<.0001), complication(p=.0090) and NYHA level(p<.0001). 5) There was significantly positive relationship between body image, self esteem and quality of life. Subjects with more positive body image had higher self esteem(r=.7897, p<.05) and subjects with higher self esteem had higher quality of life(r=.6091, p<.05).

BACKGROUND: When going into the intensive care unit (ICU), health care workers (HCWs) and visitors are required to put on cover-gowns and change into slippers, which is considered by some hospitals as one of the customary infection control practices. The purpose of this study was to evaluate the efficacy of cover-gowns and slippers in reducing nosocomial infections in the medical ICU (MICU). METHODS: The study was carried out in four periods. In the period-I (January to June, 2002), all HCWs and visitors put on cover-gowns when entering the MICU, but not in the period-II (July to December, 2002). They were required to change into slippers in the period-III (January to April, 2004), but not in the period-IV (May to August, 2004). RESULTS: Nosocomial infection rates during the period-I and period-II were 14.8 and 12.6 per 100 discharges (P>0.05) or 14.1 and 13.6 per 1,000 patient days (P>0.05), respectively. During the period-III and period-IV respective infection rates were 9.4 and 12.6 (P>0.05) or 10.2 and 15.7 (P>0.05). There were no statistical differences in device utilization ratios, device-associated infection rates, and site of infections (P>0.05) between the periods of I and II or between the periods of III and IV. CONCLUSION: These results showed that the intervention of cover-gown and slipper change is a non-effective factor in reducing nosocomial infections in the MICU; therefore, the infection control practice of cover-gown and slipper changes for HCWs and visitors entering the ICU should be eliminated.

PURPOSE: The purpose of this study was to identify relationship of urinary symptom, urinary discomfort and quality of life among the bladder cancer patients and benign prostate hypertrophy patients, and to contribute health promotion of such patients and nursing intervention development based on this results. METHOD: Study sample recruited bladder cancer patents(n=49) and benign prostate hypertrophy patients who admitted Seoul National University Hospital from June, 2002 to June, 2003. Both group patients were operated, and prostate hypertrophy patients group (mean 67.8 years old) were older than bladder cancer patients group(60.82 years old). Instruments was composed of general characteristics, urinary symptom scale(19 items), urinary discomfort scale(19 items) and quality of life scale(21 items). Data was analysed SPSS PC + 10 using mean, standard deviation, pearson correlation coefficient. Result as follows: 1. There was a statistically significant difference in occupation between two groups (p=.027). Hypertrophy patients group's age was more older than bladder cancer patients group. 2. The prostate hypertrophy patients group had the significantly higher score in urinary symptom (p=000) and nighttime urination frequency. However, there was no significant difference in incontinence symptoms and the symptoms associated bladder cancer between two groups. 3. The prostate hypertrophy patients group had significantly higher score in urinary discomfort (p=000) than the bladder cancer patients group. However, there was no significant difference incontinence discomfort and the discomfort associated bladder cancer between two groups. 4. The prostate hypertrophy patients group suffered more urinary discomfort than the bladder cancer patients group did. The quality of life the prostate hypertrophy patients group was lower than the quality of life the bladder cancer patients group. Quality of life was no statistically significant difference between two groups (p=000). 5. There was a positive correlation between urinary symptoms and urinary discomfort. However, there was a negative correlation between the quality of life and urination symptoms and discomfort. Conclusions: The prostate hypertrophy patients group had significantly higher score in urinary symptom and urinary discomfort (p=000) than the bladder cancer patients group. The quality of life the prostate hypertrophy patients group was lower than the quality of life the bladder cancer patients group. This means that urinary symptom and urinary discomfort in prostate hypertrophy patient group is more important problem. So, prostate hypertrophy patient group need to control the symptom. Therefore, nurses will be provide the intervention program to improve the bladder function after prostate hypertrophy surgery.

The purpose of this study was to identify the effectiveness of bladder training on self voiding after removal of catheter in female patients with craniotomy, finally to develop a bladder rehabilitation program for cognitive impaired patients. Nonequivalent control group posttest design was used. The population of this study consisted of 34 hospitalized neurosurgical patients, all patients have been received craniotomy. 17 patients were assigned to the experimental group and another 17 patients to the control group. The homogeneity of general characteristics of the subjects was no significant difference. Bladder training program consisted of pre-training education, the bladder training, positive verbal reinforcement. The experimental group has been received bladder training and the control group has been received gravity drainage. The dependent variables, the frequency of voiding trial untill self voiding achieves, the frequency of urinary retention, the amount of residual urine, the occurrence of urinary incontinence, were measured during 3 days after catheter removed. The data analyzed with SPSSWIN : frequency, percentage, t-test and chi-test were used to analyze homogeneity of general characteristics of subjects between the experimental and the control group. T-test, Mann-Whitney U test, and chi-test were used to determine the effect of bladder training. The results of the study were as follows : There was significant difference in the frequency of voiding trial untill self voiding achieves between the experimental group and the control group. There was no significant difference in the frequency of urinary retention between the experimental group and the control group. There was no significant difference in the amount of residual urine between the experimental group and the control group. However, there was significant difference in the amount of residual urine in urinary retention patients. There was significant difference in the occurrence of urinary incontinence between the experimental group and the control group. In conclusion, bladder training program as a nursing intervention was effective in increasing self voiding ability after removal of catheter for craniotomy patients. Therefore, it is recommended to use the bladder training program clinically for the bladder management of cognitive impaired patients.

PURPOSE: The purpose of this study was to investigate the effects of upper extremity exercise training on the motor activity, the ADL and the health related quality of life. METHOD: A non-equivalent pretest-posttest design was used. Study subjects were conveniently selected 40 hemiplegic patients(20 experimental subjects, 20 control subjects) who had been enrolled in two community health centers. After biofeedback training the subjects of experimental group were given constraint-induced movement, involving restraint of unaffected U/E in a sling for about 6 hours over a period of two weeks, while at the same time intensively training the affected U/E. Outcomes were evaluated on the basis of motor activity(amount, quality) of plegic side, DL(ADL, IADL) and health related QOL(SF-36). RESULTS: 1. After 2 weeks of treatment, the amount of use and the quality of motor activity of affected U/E were significantly higher in subjects who participated in exercise training than in subjects in the control group. 2. There were no significant differences in ADL and IADL between experimental and control groups. 3. After 2 weeks of treatment. the health related QOL was significantly higher in subjects who participated in exercise training than in subjects in the control group. CONCLUSION: The above results state that the U/E exercise training could be an effective intervention for improving the motor activity and the health related QOL of chronic hemiplegic patients. Long-term studies are needed to determine the lasting effects of constraint-induced movement.

PURPOSE: The purpose of this study was to investigate the quality of life and related factors of CVA surgery patients. METHOD: The subjects were 64 CVA surgery patients who had discharged and received follow up care at the OPD. Data were collected using William's SS-QOL instrument and IADL. And Data were analyzed with SPSS WIN program in which frequency, percentage, t-test, Pearson's correlation and stepwise regression were used to examine the quality of life and related factors. RESULT: General characteristics related to QOL were marital status and economical status. Depression was the most important factor with influence on QOL in CVA disease patients after surgery. There were further positive correlation between patient's IADL and QOL and between patient's social support and QOL. Meanwhile the patient's depression level was negatively correlated with QOL. CONCLUSION: Depression was the most important factor with influence on QOL in CVA patients after surgery. Therefore active nursing intervention to decrease depression and to improve patient's physical functional status in needed. And the patient's family should be included in all nursing intervention and patient education so that the patient's quality of life is prompted by the maintenance of optimal wellbeing.

The purpose of this study was to identify levels of activity of daily living, self- efficacy, stroke specific quality of life and need for self-help management program for patients with hemiplegia in the home. Data were collected from June to November, 2000 and subjects were 88 poststroke patients who lived in Seoul and Kyunggi-do. The questionnaire consisted of 5 scales: activities of daily living, self-efficacy, stroke specific qulaity of life and need for a self-help management program. Data were analyzed using frequencies, percent, paired t-test, and Pearson's corre- lation coefficient with the SAS(version 6.12) program. The results are as follows ; 1) Most of subjects were partially independ- ent in ADL, but they needed assist once to do dressing, bathing, meal prepara- tion and house keeping work. 2) The mean self-efficacy score was 54.89 (range : 1 to 80) and the individual differences were large. 3) Subjects responded that they were satisfied on the stroke specific quality of life scale totaled 65.8%. This value is comparatively low, especially for social role(51.4%), family functioning(58.3%) and mood (62.2%). 4) The highest needs for self-help manage- ment programs were for physical therapy, stress management, and range of motion exercise and the lowest needs were for elimination management and training, family counseling, and speech therapy. 5) On the demographic variables, sex showed significant differences for the dependent variables. Females had higher scores than males for IADL, self-efficacy, stroke- specific quality of life, and need for self-help management. 6) Age had high negative correlation with ADL, self-efficacy and stroke specific quality of life. Age was also correlated with need for self-help management. In conclusion, there was a high correlation for ADL, Self-efficacy and Quality of life in poststroke patients of home. The patient with a stroke also had a strong need for self-help management programs especially physical therapy and stress management. Therefore rehabilitation programs based on self-efficacy enhancement need to be developed in order to promote independent living for patients with hemiplegia.

This study is to evaluate the decubitus ulcer healing effect of occlusive dressing with hydrocolloid and traditional dry dressing for home care nursing clients. Study design was quasi-experimental pre-post test design. Control group(15 person) was treated decubitus wound with traditional dry gauze dressing, and experimental group(17 persons) was treated decubitus ulcer with occlusive dressing with hydrocolloid and calcium alginate(17 persons). Study period was from 8/1/97 to 12/31/97. Subject subjects was 17 males and 15 females with 2nd phase over decubitus ulcer aged 30 to 70 who have been registered as home care nursing clients in Nurse's Association in Seoul city. Outcome variables were stages, size of decubitus ulcer, pain at dressing time and exudate amount. Study result as follows. Occlusive dressing with hydrocolloid was superior to traditional gauze dressing decreasing ulcer size and phase, and shortening treatment period. Dressing change of occlusive dressing was easier than that of traditional gauze dressing. In conclusion, occlusive dressing was recommended to treat decubitus ulcer in home care patients even if hydrocolloid is expensive to gauze dressing because of treatment effect and easiness of dressing.
