3.Maternal and Perinatal Outcomes in Pregnancies Complicated with Placenta Previa Totalis.

OBJECTIVE: To determine the dincal significance of placenta previa totalis. METHODS: Maternal and neonatal medical rerords were reviewed retrospectively. Between March 1990 and June 1997, sixty-nine pregnant women with placenta previa totalis delivered at Asan Medical Center. Diagnosis of placenta previa totalis was confirmed during cesarean section. RESULTS: Mean maternal age at diagnosis was 31.7+ 3.9 years and 3 patients(4.3%) were nullipara. Fifty two patients(75%) had the history of vaginal bleeding during their index pregnancy and seventeen of 52 patients were admitted more than once. Median gestational age at the time of initial bleeding episode was 33.2 weeks(range 23.5-41. 1) and median interval from the first admission to delivery was 11 days(range 1-63), Major placental implantation site was posterior uterine wall(64%, 44/69). Six cases(8%) were complicated with placenta accreta or increta and no case was combined with abruptio placentae. Estimated blood loss at the time of cesarean section was 1,510+/-952ml(mean+/-SD) and 43 patients(62%) were transfused. No case was complicated with disseminated intravascular coagulation. Eight patients(11.6%, 8/69) underwent cesarean hysterectomy because of uncontrollable bleeding. Thirty four patients(49.3%) delivered their babies before 37 weeks of gestation. The mean gestational age at delivery was 36.4+/-3.0 weeks(mean+/-SD). Major neonatal morbidity was respiratory distress syndrome(20.3%, 14/69). Perinatal death rate was 4.3%(3/70). CONCLUSION: Because pregnant women complicated with placenta previa totalis have high probability for transfusion and cesarean hysterectomy, these patients should be managed cautiously and thoroughly. The most frequent neonatal morbidity was respiratory distress syndrome due to preterm delivery.

4.The various orbital infections from odontogenic origin.

Orbital infection or inflammation is a rare but serious complication of an odontogenic infection. Odontogenic infection can spread to the orbit by one or more of several paths. Such extension is potentially dangerous and can lead to loss of vision or worse. 5-cases of orbital infection and inflammation secondary to infection from upper or lower molar teeth, which extended to the subperiosteal or the retrobulbar region of the orbit, are presented in this report. The infections spreaded to the infratemporal and temporal fossa or the ethmoidal labyrinth, and then to the orbit via the inferior orbital fissure or the lamina papyracea. The clinical presentation, differential diagnosis, route of spread, value of serial CT scanning, treatment and possible complications are reviewed.

5.A Case of Astrocytoma in the Third Ventricle.

In children, primary tumors of the third ventricle are uncommon. The majority of the lesion were infiltrating astrocytoma of the anterior portion of the third ventricle. Tumors within the third ventricle were benign, encapsulated, and respectable with good prognosis. But most surgerns have considered this tumors to be inoperable;accordingly shunting procedures followed by radiotherapy has been the treatment of choice. We recently experienced a case of astrocytoma which entirely filled the third ventricle with hemorrhage and reviewed the literatures.

6.One case of Infectious Mononcleosis.

A case of infectious mononucleosis was presented. She was admitted to the hospital with the anterior cervical lymphadenopathy. Erythematous skin rashes on both low extermities, and splenomegaly. Her chief complaints were fever and sore throat. She showed atypical lymphocytes in peripheral blood smear with relative lymphocytosis. Mono-spot test was positive. She received symptomatic therapy and discharged without any complications. So, we report this case and review the brief literatures of infectious mononucleosis with the respect to etiology, clinical course, and histological characteristics of the disease.

7.Does The Mitral Annulus Shrink or Enlarge During Systole? A Real-Time 3D Echocardiography Study.

This study was conducted to explore the geometrical changes of the mitral annulus during systole. The 3D shape of the mitral annulus was reconstructed in 13 normal subjects who had normal structure of the mitral apparatus using real-time 3D echocardiography (RT3DE) and 3D computer software. The two orthogonal (antero-posterior and commissure-commissure) dimensions, the areas (2D projected and 3D surface) and the non-planarity of the mitral annulus were estimated during early, mid and late systole. We demonstrated that the MA had a "saddle shape" appearance and it consistently enlarged mainly in the antero-posterior direction from early to late systole with lessening of its non-planarity, as was determined by 3D reconstruction using RT3DE and 3D computer software.

8.Analyses of Genetic Alterations in Breast Cancers by Comparative Genomic Hybridization.

Transformation and progression of breast cancer are thought to be caused by an accumulation of complex genetic alterations, but little is known about specific changes. In this study, the author has undertaken a genome-wide screening to detect genetic changes in 20 cases of breast cancer among Koreans, including 16 infiltrating ductal carcinomas, 2 medullary carcinomas, 1 invasive lobular carcinoma, and 1 borderline phyllodes tumor. Comparative genomic hybridization (CGH) was used to screen for DNA sequence gains and losses across all human chromosomes. Simultaneous immunohistochemical staining for c-erbB-2 (Her-2/neu), c-myc, cyclin D1, and p53 protein was done to make comparisons with nuclear grade and that with CGH results. Biotin-labeled tumor DNA and digoxigenin-labeled normal DNA were hybridized to normal metaphase cells. The fluorescence signals were captured by fluorescence microscope after detection by avidin-FITC and anti-digoxigenin rhodamine. Then, the ratio of fluorescence was calculated by an image analyzer. The immunohistochemical staining was done in paraffin-embedded tissue with an LSAB kit and avidin-biotin complex (ABC) method. The CGH results showed gains on chromosomes 8q (40%), 1q (30%), 17q (15%), 20q (15%), 18q (15%), 5p (15%), and 13q (15%). Deletions were on chromosomes 17p (45%) and 22q (20%). High-level amplifications (green/red ratio >1.5) were noted on chromosomes 1p31, 1q, 3q25-qter, 5p, 7q31-qter, 8q, 9p22-qter, 10p, 11p, 11q22-qter, 12p, 12q24, 14q21-qter, 15q23-qter, 17q, 18p, 18q12-qter, 20p, and 20q. By comparison with infiltrating ductal carcinoma, the two medullary carcinomas showed high-level amplification on chromosomes 1p31, 1q, 8q, 10p, 11p and 12p. c-erbB-2, c-myc, cyclin D1, and p53 protein expression was immunohistochemically detected in 9 of 20 (45%), 8 of 20 (40%), 10 of 20 (50%), and 13 of 20 (65%), respectively. The results indicate that the amplification on chromosome 8q, 1q and the deletions on chromosomes 17p and 22q are the most frequent genetic alterations in breast cancers among Koreans. The results reveal a different pattern of genetic alteration from previous studies. The CGH results were not correlated with the immunohistochemical profiles. The amplification pattern of medullary carcinomas was quite different from the pattern of infiltrating ductal carcinomas. The CGH was thought to be very useful in the screening of genetic alterations of solid tumors.

9.Effects of Normocapneic Preoxygenation and Air Breathing during Vital Capacity Inhalation Induction with Sevoflurane on the Ventilation.

BACKGROUND: Sevoflurane has been used to provide an inhaled induction by using a vital capacity breath, which is fast and has few side effects. We compared the clinical effects of a vital capacity inhalation induction (VCII) with sevoflurane in patients of preoxygenation or air-breathing before anesthetic induction. METHODS: After IRB approval, patients were randomly assigned to receive preoxygenation (O2 group, 70 patients) or air breathing (Air group, 70 patients) via SIBI (Single Breath Induction) connectorTM before VCII with 8% sevoflurane in 75% N2O/O2 from primed circuit. The clinical characteristics were compared between two groups in respect to prolongation of breath holding after loss of consciousness (response to verbal command) and side effects (airway, hemodynamic, motor) during VCII. RESULTS: O2 group showed lower incidence (60.0% vs. 87.1%, P < 0.05) and shorter duration (27.1 s vs. 36.4 s, P < 0.05) in prolongation of breath holding than Air group. Otherwise, there were no significant differences in clinical effects between two groups. CONCLUSIONS: We found that preoxygenation reduces the incidence and duration of prolongation of breath holding during VCII with sevoflurane compared with air-ventilation before VCII. We suggest that the prolongation of breath holding might be related to Hering-Breuer response to maximal lung inflation during VCII.
