2.Primary Malignant Hemangiopericytoma of the Lung: A case report.

Primary pulmonary hemangiopericytoma is a rare, potentially malignant tumor and assumes more malignant course than that originated in the soft tissue. It can be asymtomatic until attaining a large size within the lung. We report a case of malignant hemangiopericytoma of the lung in a 29 year old man. It represented as a sharply demarcated, huge homogeneous opaque mass in the right upper lobe. The cytologic and histologic features were that of malignant hemangiopericytoma.

5.Analysis of the Effectiveness in the Hospital Management of Methicillin-Resistant Staphylococcus aureus by Different Isolation Policies.

BACKGROUND: The purpose of this study is to evaluate the evidence for the effectiveness of different isolation policies in reducing the incidence of methicillin-resistant Staphylococcus aureus (MRSA) infection in hospital in-patients. METHODS: We prospectively evaluated the effectiveness of an isolation policy on transmission of MRSA in the 745-beds hospital. First period, all patients with MRSA (March - July 2000) were not isolated Second period, strict isolation policies were performed (August 2000 - January 2002). All patients with MRSA were isolated in separated room, hand hygiene using alcohol handrub, gowning and g1oving, apply of MRSA notice sticker, criteria in isolation remove, separated disinfection and wastement. Third period, semi strict isolation policies were performed (February 2002 - August 2005). Some patients with MRSA were isolated in separated room and others were admitted in general ward for bed shortage. Only some practices were performed in hand washing and separated disinfection in general ward. RESULTS: The rates of MRSA nosocomial infection per patients during 1st, 2nd, and 3rd surveillance were reported 0.56, 0.23, and 0.42 (P<0.05). Patient-days rate of MRSA nosocomial infection during 1st, 2nd, and 3rd surveillance were reported 0.62, 0.27, and 0.38 (P<0.05). CONCLUSION: With many different isolation policies, it was possible to reduce nosocomial infection of MRSA. In this study, strict isolation policies (2nd period) were the most effective practices in reducing MRSA infection.

6.The Need for Rehabilitation Day Care Program Service of Stroke Survivor's Family.

This study was carried out to find out the basic data required to plan and develop Rehabilitation Day Care Program for the stroke survivor's family in Korea. The subjects comprised of 92 stroke survivor's family who discharged from 4 hospitals in Seoul during the past 2 years. The data were collected from August 3. 1998 to September 18, 1998, through interviews with questionnaires about general characteristics, activities of daily living, depression and service need of rehabilitation day care program at the outpatient clinics by trained nursing graduates. Data were analyzed with descriptive analysis, Pearson's correlation analysis, and Stepwise multiple linear regression analysis using SPSS/WIN 10.0 program. The results obtained are as follows: 1. The mean score of the general need of rehabilitation day care program of stroke survivor's family was 3.10(range 1-4). The highest need among the service categories of the rehabilitation day care program was self-care and restorative activities category(3.30). and health services referral category, recreation category, psychosocial activities category in order. The needs of each category are as follows. In the health services referral category, the need for dental examination and medical examination were highest, followed by the need for physical therapy and occupational therapy. In the psychosocial activities category, the need for family counselling was highest. In the self-care and restorative activities category, the need for ROM exercise training was highest, followed by bowel training, and ambulation training. 2. The need of family for rehabilitation day care program service displayed a correlation with the level of education. ADL. and the level of depression. and a reverse correlation with age, illness intrusiveness, depression, knowledge. subject and object burden and relationship with stroke survivors. 3. The stepwise multiple linear regression analysis revealed following results. For the need for rehabilitation day care program service. 22.6% of the variance was initially explained by level of family's knowledge about caring method for stroke survivors, 8.8% was the level of subjective burden and 5.4% was relationship with stroke survivors. In conclusion, above characteristics should be considered to develop stroke survivors' rehabilitation day care program.

7.Survival Analysis EKG Monitor and Anesthetic Machine in Operating Room.

We have done survival analysis of the EKG monitor(from March, 1976 to February, 1993) and anesthetic machine(from February, 1915 to February, 1993) retrospectively in operating room at Severance hospital. Thirty eight EKG monitors and twenty eight anesthetic machines were included. Also EKG monitors were divided into two groups(one group was made in foreign country and another was made in Korea) and its survival function were compared. All statistics were treated with Product-limited estimate of survival function(Kaplan and Meier, 1958) with BMDP 1L. Mean survival time(+/-S.E) of all EKG monitor was 113.61(+/-5.70) months and the EKG made in foreign country was 106.65(+/-8.22) months and made in Korea was 113.35(+/-8.97) months. But there was no statistical differences between two groups. And mean survival time of anesthetic machine was shown to be 189.45(+/-6.09) months. It is advisable to replace aging machine before malfunction and failure to protect patients from mishaps stem from equipment failure. We conclude that life cycle of equipment for proper time to replace the equipment is not underestimated to reduce the accident by aging machine failure.

8.A Study on the Level of Comfort in Korean Adults: Comparison between Healthy people and in-Patients.

The purpose of this study was to compare the comfort level between healthy people and adult in-patients in Korea. The sample consisted of 248 adults (healthy adults: 124, patients admitted in university hospitals in Seoul and Jinju: 124). The research tool used for the study was General Comfort Questionnaire(GCQ) which was developed by Kolcaba(1992) and was translated by Korean. The research tool consisted of 28 items (6 items on physical dimension, 9 items on psychospritual dimension, 7 items on environmental dimension, 6 items on social dimension). Data were analyzing using the SPSS, yielding t-test, ANOVA. The results are as follows: 1) Mean scores for comfort level in healthy adults were 2.92 on a 4 point scale. 2) Mean scores for comfort level in-patient adult were 2.72 on a 4 point scale. 3) There was a significant difference between the two groups on the comfort level. The healthy adults had higher comfort level than in-patients (t=4.44, p=0.000). 4) There was a significant difference between the two groups on the comfort level of physical and environmental dimension. The healthy adults had higher comfort level than in-patients (t=2.99, p=0.003) and environmental dimension (t=8.81, p=0.000).

9.Social Readjustment and Relating Factors in Stroke Survivors.

Social readjustment is very important in rehabilitation of stroke survivors. The purpose of this study was to examine the level of social readjustment and related factors in stroke survivors. Especially, to find the most useful predictor for social readjustment is a major point of this research. The study included 254 outpatients who were given follow-up care after discharge from. Occupational rehabilitation and resumption of the perceived meaningful social activity prios to the stroke were used to measure social readjustment. The resumptions of first meaningful social activity was the best predictor for life satisfaction in stroke survivors(r=.245, p<.01). The substance of perceived meaningful social activities were job, meeting with friends, hobby and activities for the family. The sum of meaningful social activities (r=.175, p<.01) and occupational rehabilitation (r=.116, p<.05) were significantly related to life satisfaction. There were significant difference in IADL, depression and life satisfaction according to resumption of meaningful social activity. But, occupational rehabilitation was not related to depression. The level of social readjustment was different between occupational rehabilitation and resumption of meaningful social activity. It was 45.7% in the first meaningful social activity and 36.6% in occupational rehabilitation. The related factors with resumption of the first meaningful social activity were that of IADL, depression, illness intrusiveness and cognitive function. And the level of IADL, illness intrusiveness, cognitive function, and age were significantly related to occupational rehabilitation. In conclusion, we suggest that the resumption of the first meaningful activity in stroke survivors is the best predictor of social rehabilitation. Thus, nurses need to work for resumption of meaningful activity as well as occupational rehabilitation.
