2.Coexistence of Darier's Disease and Acrokeratosis Verruciformis of Hopf.

Darier's disease and acrokeratosis verruciformis are inherited with the same pattern and they share similar clinical properties. We present a patient with both Darier's disease and acrokeratosis verruciformis. The patient had had brownish and skin-colored papules, initially on her face and neck, since the age of 25. Similar lesions had gradually spread to various parts of her body and lastly the dorsal aspect of her feet. On dermatological examination, brownish and skin-colored, 2 to 5 mm, keratotic papules were observed on her face, neck, both axillae, inframammary regions, and dorsa of the hands and feet. The nail examination revealed "V" shaped notches and longitudinal ridges on the fingers of both hands. The biopsy taken from the neck showed typical findings for Darier's disease. The biopsy from the dorsa of the left foot revealed the distinctive structures of acrokeratosis verruciformis. In this article the clinical and histopathological findings of these diseases are reviewed based on our patient.

4.Clinical and Histopathological Investigation of Seborrheic Keratosis.

BACKGROUND: Seborrheic keratosis (SK) is one of the most common epidermal tumors of the skin. However, only a few large-scale clinicohistopathological investigations have been conducted on SK or on the possible correlation between histopathological SK subtype and location. OBJECTIVE: The aim of this study was to analyze the clinical and histopathological features of a relatively large number of cases of diagnosed SK. METHODS: Two hundred and seventy-one pathology slides of skin tissue from patients with clinically diagnosed SK and 206 cases of biopsy-proven SK were analyzed. The biopsy-proven cases of SK were assessed for histopathological subclassification. The demographic, clinical, and histopathological data of the patients were collected for analysis of associated factors. RESULTS: The most frequent histopathological subtype was the acanthotic type, followed by mixed, hyperkeratotic, melanoacanthoma, clonal, irritated, and adenoid types; an unexpectedly high percentage (9.2%) of the melanoacanthoma variant was observed. The adenoid type was more common in sun-exposed sites than in sun-protected sites (p=0.028). Premalignant and malignant entities together represented almost one-quarter (24.2%) of the clinicopathological mismatch cases (i.e., mismatch between the clinical and histopathological diagnoses). Regarding the location of SK development, the frequency of mismatch for the sun-exposed areas was significantly higher than that for sun-protected areas (p=0.043). CONCLUSION: The adenoid type was more common in sun-exposed sites. Biopsy sampling should be performed for lesions situated in sun-exposed areas to exclude other premalignant or malignant diseases.

5.A Case of Focal Acantholytic Dyskeratosis Occurring on both the Lip and the Anal Canal.

Focal acantholytic dyskeratosis has a distinctive histological pattern that is associated with various clinical expressions. It rarely occurs on the lip or the perianal area. We report a patient with focal acantholytic dyskeratosis occurring on both the upper lip and the anal canal. Histopathologically, the lesions showed hyperkeratosis, suprabasilar clefting, epidermal acantholysis and dyskeratosis. This case represents the first report of a focal acantholytic dyskeratosis occurring on both the lip and the anal canal.

6.Cutaneous Horn in Premalignant and Malignant Conditions.

Cutaneous horns are conical, circumscribed protuberances formed by densely layered keratin. These lesions originate from basal keratinocytes and may manifest as benign, premalignant, or malignant cutaneous pathology in chronically sun-damaged areas. Complete surgical excision with histologic examination is needed for potential malignancy. In this report, we describe two elderly women presenting with solitary facial cutaneous horns, which were respectively diagnosed as actinic keratosis and squamous cell carcinoma.

7.Clinical Analysis of Benign Eyelid and Conjunctival Tumors.

PURPOSE: To assess the occurrence and characteristics of benign eyelid and conjunctival tumors. METHODS: We histopathologically analyzed 80 consecutive cases of benign eyelid (56 cases) and conjunctival (24 cases) tumors between April 2000 and November 2002. All eyelid and conjunctival tumors were confirmed histopathologically. Age, sexual distribution, occurrence and location were analyzed. RESULTS: The mean ages were 42.32+/-16.27 SD in benign eyelid tumors, and 29.71+/-18.60 SD in benign conjunctival tumors. The sexual distribution revealed 23 males and 33 females in benign eyelid tumors, and 12 males and 12 females in benign conjunctival tumors. About the location, lower lid was most frequent site of benign eyelid tumors, responsible for 27 (48.2%) of the 56 cases, and medial conjunctiva was the most frequent site of benign conjunctival tumors, responsible for 14 (58.3%) of 24 cases. Shave biopsy was performed in eyelid margin tumors. Excisional biopsy was performed in other eyelid tumors and conjunctival tumors. The frequency of benign eyelid tumors was noted in this order: intradermal nevi 25 cases (44.6%), seborrheic keratoses 9 cases (16.1%), and compound nevi 6 cases (10.7%), and frequency of benign conjunctival tumors was noted in this order: compound nevi 7 cases (29.2%), and intradermal nevi 6 cases (25.0%). CONCLUSIONS: From the Results of this study, we hope that this report provides a basic source for diagnosis and therapy of the benign eyelid and conjunctival tumors.

8.Clinical Analysis of the Laryngeal Premalignant Lesion.

BACKGROUND AND OBJECTIVES: It is hard to adequately define the clinical and histologic characteristics of laryngeal premalignant lesions that herald the potential for progression to invasive squamous cell carcinoma especially in the Korean population. The aim of this study was to analyze prognostic factors of laryngeal premalignant lesions in the Korean population. MATERIALS AND METHOD: A retrospective analysis was undertaken in 88 patients with laryngeal premalignant lesions. All were diagnosed with laryngeal keratosis and dysplasia on pathology. RESULTS: 66 patients showed keratosis, 12 patients mild dysplasia, 4 patients moderate dysplasia, and 6 patients severe dysplasia. Of the 88 patients available for follow up (ranging from 6 to 135 months), six patients (6.8%) developed invasive squamous cell carcinoma. Use of CO2 Laser was not associated with cancer progression (p=0.641). In contrast, grade of dysplasia was significantly associated with cancer risk (p=0.032). CONCLUSION: The laryngeal premalignant lesion represents a possible passage to cancer in a limited number of cases, but with a more probability for high degree dysplasia in respect to low degree dysplasia.

9.An unusual collision tumour masquerading as a basal cell carcinoma on the nose.

When two or more cutaneous tumours coexist in a single lesion, it is known as a cutaneous collision or contiguous tumour. Various combinations of collisions have been described. Collision tumours often have misleading clinical and histological presentations, and can be a diagnostic challenge. Chondroid syringomas are mixed cutaneous tumours of dual origin, and like collision tumours, are often confused with the more commonly seen cutaneous lesions. As chondroid syringomas are rare, their involvement in collision tumours is an even more peculiar occurrence. We report an unusual case of a cutaneous collision tumour on the nose involving an intradermal naevus and chondroid syringoma. To the best of our knowledge, this is the first time such a combination is reported.

10.A Case of Irritated Seborrheic Keratosis Associated with a Previous Incision Site.

An 83-year-old woman had undergone an external dacryocystorhinostomy with silicone intubation. Before the surgery, no skin lesions were observed on the incision site. Three months after surgery, the patient complained of a brown-to-black pigmented elevation at her previous skin incision site. A punch biopsy of the pigmented mass was performed. The histopathologic findings confirmed the clinical diagnosis of irritated seborrheic keratosis (SK). SK can occur several months postoperatively and can suddenly increase in size, so surgeons need to carefully check patients' skin prior to surgery. To our knowledge, this is the first reported case of irritated SK discovered on a previous skin incision site.
